1.A Case of Chondrodermatitis Nodularis Chronica Helicis.

A 51-year-old man had small, tender, skin colored firm nodule on the helix of the both ears for eight months. He was found to have classic features of chondrodermatitis nodularis chronica helicis, namely, tender nodule less than 1 cm in length with central crusting, localization to helix, age, men, and histologic findings. The histologic examination of the biopsy specimen showed the presence of thinned epiderrnis on center. acanthosis on the adjacent epidermis, with hyperkeratosis and parakeratosis, and cleft between epidermis and dermis. In the dermis, highly vascularized connective tissue and chronic inflarnmatory infiltrate were visible with perichondrial fibrosis. Elastic tissue stain showed degeneration of elastic fibers. After complete excision, no recurrence was obsered until now.

2.Congenital Agenesis of Odonteid Process: A Case Report

Anomalous development of the odonteid precess is an uncommon clinical occurence that can seriously impair the integrity af the atlantoaxial articulation. Absence of the odontoid process may be either congenital or acquired, but clinically the etiology is of little importance as the clinical signs and symptoms and the treatment are identical in both. We experienced a case of congenital absence of the odontoid process and good result was obtained from the posterior atlantoaxial fusion with hale-cast.

3.Mechanical and Morphological Properties of the Growing Long Bone: A Torsion Study on Rabbit's Femur

It is essential to define the mechanical and morphological characteristics of bone in undrestanding the mechanism and treatment of fractrues of bone. We already reported the results of bending study in 1983 and tension study in 1987 to assess the mechanical and morphological changes of bone according to the growth. Following these studies, we performed torsion study on growing long bone to have better understanding of the characteristics of bone. In order to assess the mechanical and morphological properties of fracture of growing bone by torsion, we divided 100 Newzealand white rabbits(40 1-month-old rabbits, 20 3-months old rabbits and 40 5-months-old rabbits) into 6 experimental groups according to their maturity and side : Group 1(right femur of 1 month old), Group 2( left femur of 1 month old), Group 3(right femur of 3 months old), Group 4(left femur of 3 months old), Group 5(right femur of 5 months old), Group 6(left femur of 5 months old). Each group of femora were loaded in torsion with varing distance and the following results were obtained: 1. Fracture shapes were composed of the spiral component and the vertical component which connected the two ends of the spiral component. 2. Younger bones revealed greater deformation angle and apex angle but less obliquity than older bones. 3. When the gap between the grips were lengthened three times, there was a very signi ficant increase of deformation angle and obliquity but apex angle was decreased and the fracture angle showed little change. 4. It was necessary to measure the apex angle and obliquity in the morphological description of fractures by torsion. 5. We observed when a certain degree of angular deformity was applied on a bone at a certain age, the fracture was occured through the tension failure regardless of types of loading forces.

4.A Case of Malignant Acanthosis Nigricans Associated with Gastric Adenocarcinoma.

A 51-year-old female had disseminated, symmetrically distributed, brownish black pigmentation and papillary hypertrophy on the intertriginous and flexural areas for 6 months, and gastric adenocarcinoma was confirmed during the evaluation of internal malignancy, The cutaneous finding of acanthosis nigricans was a significant cutaneous marker of internal malignancy in this case.

5.Serum 17-Hydroxyprogesterone Levels in Term and Preterm Infants.

PURPOSE:The incidence of congenital adrenal hyperplasia(CAH) is 1/5,000- 1/20,000 births and thus the importance of the neonatal screening test is being emphasized. However, the reference value for the term and preterm infants has not yet been established and false positive values are frequent due the immature hypothalamic-adrenal axis of the preterm infants or the stress-induced adrenal dysfunction. Therefore, we analyzed the 17-hydroxyprogesterone(17-OHP) concentration in terms of gestational age, birth weight, and postnatal state to establish the reference range for the Korean term and preterm infants. METHODS:We analyzed the results of the CAH screening test retrospectively, which was performed on 737 neonates(624 fullterm neonates, 113 premature neonates) born between January 1998 through July 1998 in Inje University College of Medicine Sanggye Paik Hospital. Mean gestational age and birth weight of infants were 38.2+/-2.6 weeks and 3,116+/-674kg respectively. 17-OHP screening test was performed on 4.9+/-3.8days after birth by obtaining blood samples from the heelstick of neonates. 17-OHP concentration was measured by the ELISA kit(ICN Co.) and repeated the procedure if the result was higher than 35ng/ml. RESULTS: 1) 17-OHP concentration of the preterm infants was significantly higher than that of the fullterm infants(19.1+/-12.3ng/ml vs 11.7+/-7.8ng/ml, P=0.001). 17-OHP concentration was inversely proportional to gestational age. 2)17-OHP concentration was inversely proportional to birth weight(r=0.22, P>0.01). 17-OHP concentration according to birth weight was as follows.:below 1,500g was 26.7+/-11.7ng/ml, 1,500 to 2,000g was 18.0+/-13.9ng/ml, 2,001 to 2,500g was 17.9+/-10.5ng/ml, 2,501 to 3,000g was 12.1+/-7.9ng/ml, 3,001 to 3,500g was 11.5+/-8.1ng/ml, above 3,500g was 11.4+/-7.5ng/ml. There was a significant decline in the 17-OHP concentration as the birth weight increased. 3) 17-OHP concentration was gradually decreased as sampling date increased. 4) The gender of the infants did not influence the 17-OHP concentration(male 13.0+/-9.1 vs female 12.7+/-9.0). 5)17-OHP concentration were significantly higher in sick preterm infants than healthy preterm infants. 6)Six cases, whose 17-OHP concentration were greater than 35ng/ml, were all preterm and low birth weight infants. Reexamination after one week showed the value within normal range. No CAH cases were diagnosed in the study. CONCLUSION: 17-OHP concentration was inversely proportional to gestational age and birth weight. Therefore, reference ranges of 17-OHP concentration should be subdivided according to gestational age and birth weight. Further research about perinatal risk factors affecting the 17-OHP concentration will be required.

6.Inflammatory Linear Verrucous Epidermal Nevus: Report of Two Cases.

One patient is a 6-year old female, who has had intensely pruritic linear papuloplaque.", on the left side of the perineal and perianal areas for three years. The other patient, is a. 23-year old male, who has had iritensely pruritic linear papuloplagues on the left upper extremity. This condition started at 1 month of life. In both cases, microscopie examination of the lesions showed psoriasiform and eczematoid appearance. In the latter case, spongioform pustule and Munro's microabscess were also found. We believe that these cases probably represent instances of the entity that. Altman and Mehregan have called inflammatory linear verrueous epidermal nevus, and unusual clinical feature in our patients was the involvement of the left groin in case 1 and of the left upper extremity in case II, which are somewhat rare sites of involvement.

7.Topical Zinc Sulfate Therapy in Herpes Simplex.

Zinc sulfate, which was used as zinc sulfate ophthalmic solution in herpetic keratitis, has been found to have the therapeutic effects in cutaneous and mucosal herpes simplex. We studied ta evaluate the therapeutic effect of zinc sulfate in patients with herpes simplex. Nine patients with primary herpes simplex and twenty five patient with recurrent herpes simplex were included in this study. Twenty five patients(six with primary type and nineteen with recurrent type were treated with topical applicatiion of 0.3% zinc sulfate solutiion seven to eight times a day and nine patients(three with primary type and six with recurrent type)were applied with placebo vehicle in the same way. Maintenance treatment was then given tc all these patients, first once a week for one month, and then twice a month indefinitely. The effect was assessed by the comparison of the duration of disappearance of symptoms, drying of vesicles and crust formation, and desiccation and sloughing-off of cursts, and recurrence rate between zine sulfat,e group and placebo group. The resuilts were summarized as follows : 1. In the average time of the disappearance of symptoms, drying of vesicles and crust formation, a,nd desiccation and sloughing-off of crusts, the resu1t observecl in zinc, sulfate group was shorter than that of placebo group, respectively ( p < 0.01). 2. In recurrence rate, 11.1% observed in zinc sulfate group was much lower than 80.0%i in placebo group(p . 0.01). These findings suggest that topical zinc sulfate is an effective agent for treatment and prevention of recurrence af herpes simplex infection.

8.CT Differentiation of Renal Tumor Invading Parenchyma and Pelvis: Renal Cell Carcinoma vs Transitional Cell Carcinoma.

PURPOSE: The differentiation between renal cell carcinoma(RCC) and transitional cell carcinoma(TCC) is important due to the different methods of treatment and prognosis. But occasionally it is difficult to draw a distinction between the two diseases when renal parenchyme and renal collecting systems are invaded simultaneously. MATERIAL AND METHOD: We reviewed CT scans of 37 cases of renal cell carcinoma and 12 cases of transitional cell carcinoma which showed involvement of renal parenchyma and renal sinus fat on CT. Retrospective analysis was performed by 3 abdominal radiologists. Check points were renalcontour bulging or reniform shape, location of mass center, intact parenchyme overlying the tumor, cystic change, calcification, LN metastasis, vessel invasion, and perirenal extention. RESULT:There were renal contour bulging due to the tumor mass in 33 out of 37 cases of renal cellcarcinima, wherea and nine of 12 cases of transitional cell carcinoma maintained the reniform appearance. This is significant statiscal difference between the two(p<0.005). Center of all TCCs were located in the renal sinus, and 24 out of 35 cases of RCC were located in the cortex(p<0.005). Thirty-six out of 37 cases of RCC lost the overlying parenchyma, whereas 4 out of 9 cases of well enhanced TCC had intact overlying parenchyma(p<0.005) RCC showed uptic change within the tumor mags in 31 cases which was significanity higher than the 4 cases in TCC(p<0.05). CONCLUSION: CT findings of renal cell carcinoma are contour bulging, peripheral location, obliteration of parenchyma, and cystic change. Findings of transitional cell carcinoma are reniform appearance, central location within the kidney, intact overlying parenchyma, and rare cystic change.

10.The Study on the Effect of Nicorandil in Angina Pectoris.

Clinical studies were performed that the patient with angina pectoris having no responses to Ca++ antagonist and beta-blocker had been taken nicorandil 5mg bid daily with, beta-blokade and Ca++ antagonists for 3 weeks. The results after the use of nicorandil were as follows; 1) The clinical symptoms after the use of nicorandil were improved in 20(80%) of 25 patients, specially marked improved in 16 of 25 patients. There was no significant changes of BP and heart rate after the use of nicorandil. 2) The results of CBC, urinalysis, serum chemistry exa. and chest P-A were within normal range before and after the use of nicorandil. 3) After the use of nicorandil, ST segments depressed in 13 of 25 patients before use of nicorandil was elevated in 8(61.6%) of 13 cases after the use of that. QT interval, P-R interval and T wave in 25 cases were within normal range before and after the use of that. The M mode echocardiography showed the decreased movement of ventricular septum in 19 of 25 patients before the use of Nicorandil, and there was no changes after the use of that. 4) The adverse effects after the use of nicorandil to 25 patients were as follows: headache in 3(12%), nausea and vomiting 2(8%), palpitation 1(4%), upper abdominal discomfort 1(4%), and facial flushing 1(4%).
