2.A Case of Chondrodermatitis Nodularis Chronica Helicis.

A 51-year-old man had small, tender, skin colored firm nodule on the helix of the both ears for eight months. He was found to have classic features of chondrodermatitis nodularis chronica helicis, namely, tender nodule less than 1 cm in length with central crusting, localization to helix, age, men, and histologic findings. The histologic examination of the biopsy specimen showed the presence of thinned epiderrnis on center. acanthosis on the adjacent epidermis, with hyperkeratosis and parakeratosis, and cleft between epidermis and dermis. In the dermis, highly vascularized connective tissue and chronic inflarnmatory infiltrate were visible with perichondrial fibrosis. Elastic tissue stain showed degeneration of elastic fibers. After complete excision, no recurrence was obsered until now.

3.Effects of Infant Massage Therapy on Transcutaneous Bilirubin, Feeding, and Excretion in Neonates with Jaundice Undergoing Phototherapy

PURPOSE: This paper reports the results of a massage therapy on transcutaneous bilirubin (TcB) in the forehead and inguinal regions, feeding intake, urine amount, and defecation frequency for neonates with jaundice who are receiving phototherapy. METHODS: A non-equivalent control group, pre-post, quasi-experimental design was used. Participants were 33 neonates with jaundice admitted for phototherapy and were randomly assigned to one of two groups—an experimental group (n=16), which received 3 days of routine plus 15 minutes of massage three times a day, and a control group (n=17), which received the routine care only. Data were analyzed using χ²-test and Mann-Whitney U test with SPSS 21.0 Win program. RESULTS: On the second day of hospitalization, the experimental group showed significantly lower TcB in the inguinal region (Z=−2.12, p=0.017); on the third day, the experimental group showed significantly lower TcB in the inguinal region (Z=−2.31, p=0.011) and higher defecation frequency (Z=−2.22, p=0.013) compared with the control group. CONCLUSIONS: The study findings suggest that massage for jaundice neonates receiving phototherapy is an effective intervention to reduce TcB in the inguinal region and to increase defecation frequency.

A 51-year-old female had disseminated, symmetrically distributed, brownish black pigmentation and papillary hypertrophy on the intertriginous and flexural areas for 6 months, and gastric adenocarcinoma was confirmed during the evaluation of internal malignancy, The cutaneous finding of acanthosis nigricans was a significant cutaneous marker of internal malignancy in this case.

On the contrary to congenital anomalies of intestinal rotation in pediatric patients, those in adults are generally nonsymptomatic and of little consequence. Occasionally, however, an adult may have midgut nonrotation and complain of chronic or recurrent abdominal pain. Intestinal nonrotation can be divided into complete or partial failure of rotation and into abnormalities affecting the proximal segment, the distal segment or both. We report herein a 43-year old female patient with symptomatic partial, cecocolic nonrotation.

Gallstone ileus is a well-known complication of cholelithiasis, but is relatively rare. Most ectopic gallstones are located in the small bowel; they are rarely found in the stomach and duodenum. We describe the imaging findings of a case of intragastric gallstone, as well as a case in which duodenal obstruction was caused by a large gallstone (Bouveret's syndrome).

9.Risk Factors for Nosocomial Urinary Tract Infection in the Intensive Care Unit with a Positive Urine Culture and Foley Catheterization.

PURPOSE: The purpose of this study was to identify the risk factors for a nosocomial urinary tract infection in intensive care units with a foley catheterization which showed a positive urine culture. METHOD: Three-hundred eighty-seven patients were included in the study. A retrospective review of the electrical medical record system's databases and medical record sheets in hospitalized patients from January 2003 to December 2003 was used. The collected data was analyzed by descriptive statistics, t-test, chi-square test and logistic regression analysis. RESULT: The frequency of the participants' nosocomial urinary tract infection was 72.9%. Significant risk factors for a nosocomial urinary tract infection were 'age', 'place of catheter insertion', 'frequency of catheter change', and 'duration of catheterization'. These variables explained 18.4% of variance in the experience of nosocomial urinary tract infection in intensive care units with foley catheterization. CONCLUSION: Medical personnel can decrease the incidence of a nosocomial urinary tract infection by recognizing and paying attention to the duration of catheterization, frequency of catheter change, and place of catheter insertion. As a result, specific and scrupulous strategies should be developed to reflect these factors for decreasing nosocomial urinary tract infections.

10.A Study on Changes of Primary Caregivers' Fatigue, Depression and Life Satisfaction by Using Dementia Day Care Service.

PURPOSE: This study is to examine how dementia day care service affects fatigue, depression and life satisfaction of caregiver. METHODS: The study was conducted using a convenient sampling method from 6 Dementia Day Care Center in Incheon. Thirty nine primary caregivers answered the questionnaires. Pre-test was done before demented elders start using the dementia day care center and post-test was done five months after. Data were analyzed by using descriptive statistics and paired t-test. RESULTS: There was a significant difference in caregivers' fatigue level after using dementia day care service(t=2.188, p=.035). Results of subcategories were as follows; There was a significant difference in caregivers' physical fatigue level(t=2.270, p=.029) and psychological fatigue level(t=2.277, p=.029) after using dementia day care service. However, there was not significant difference in caregivers' neurological fatigue level(t=1.312, p=.197). There was a significant difference in caregivers' depression level(t=3.066, p=.004) and life satisfaction(t=-2.131, p=.040) after using dementia day care service. CONCLUSION: The results indicated that dementia day care service is helpful for family with demented elders. Therefore it is necessary to expand dementia day care center in terms of its size and numbers in order to support increasing number of demented elders and their families.
