1.Pleomorphic Xanthoastrocytoma: A Case Report.

Pleomorphic xanthoastrocytoma is histologically characterized by marked cellular pleomorphism of lipid-laden neoplastic astrocytes and bizarre giant cells showing mitotic figures and high cellularity. Inspite of its ominous-looking microscopic features, howerver, the prognosis is usually favorable. This tumor develops mainly in the supratentorial area of young people and frequently involves the leptomeninges. We experienced a case of pleomorphic xanthoastrocytoma in 18 year-old-male. In addition to the cellular pleomophism, the prominent reticulin fibers surround the individual tumor cells or the tumor cells nests. Immunohistochemical staining and electron microscopy revealed glial fibrillary acidic protein(GFAP) expression and pericytoplasmic basal lamina in the tumor cells.

3.Prevalence and evaluation of asymptomatic microscopic hematuria in adults.

To knew the prevalence and nature of asymptomatic microscopic hematuria in a community. we evaluated 13,678 cases(M; 12.198 cases, F; 1,480 cases) that visited to the Department of Health Service. Kang Nam General Hospital from April to October, 1992 for annual physical examination. For the detection of hematuria urine dipstick test was done. and then urine microscopy was done in cases with positive dipstick test (262 cases). Hematuria was defined as 3 or more RBCs/HPF on microscopic examination of the urinary sediment(189 cases). Repeated urinalysis was done in cases with 3-4 RBCs/HPF (30 cases). And in cases with 5 or more RBCs/HPF on urine microscopy, excretory urography(105 cases) was primarily checked and in addition, renal ultrasonography. cystoscopy. RGP. urine culture. urine cytology. serum IgA, C3. C4 were performed for investigation ofThe results were as follows: I. Prevalence of asymptomatic microscopic hematuria was 1.38% and significantly increased with age (p<0.01). 2. Urologic abnormalities associated with hematuria were identified in 23 cases (21.9 %)-6 cases of renal stone. 4 cases of BPH. 4 cases of UPJ stricture, 3 cases or cystitis, 2 cases of ureteral stone. 1 case or ureteral stricture. nephroptosis. renal cyst. renal tuberculosis. S. In 82 cases, urologic abnormalities could not be found and then serum IgA. C3 C4 checked in 36 cases among these. IgA was increased in 21 cases (58.3%). From the above result. the causes of asymptomatic microscopic hematuria in adults could not be found in 78.1% and serious urologic diseases tumor and so on) were almost not found. We conclude that in order to obtain the detection of accurate prevalence and various urologic diseases. repeatedurine dipstick test and long-term followup in larger populations would be needed, and that screening test for the evaluation of hematuria would consist of performing excretory urography only in relative young adults and excretory urography with selective urine cytology after age 50 years.

5.Work-related Cerebro-Cardiovascular Diseases in Korea.

Cerebro-cardiovascular disease (CVD) is one of compensable occupational diseases in Korea as in Japan or Taiwan. However, most countries accept only cardiovascular diseases (ischemic heart diseases) as compensable occupational diseases if any, but not cerebrovascular diseases. Korea has a prescribed list of compensable occupational diseases. CVD was not included in the list until 1993. In the early 1990s, a case of cerebral infarction was accepted as occupational disease by the Supreme Court. The decision was based on the concept that workers' compensation system is one of the social security systems. In 1994, the government has established a diagnostic criterion of CVD. The crude rate of compensated cerebrovascular disease decreased by 60.0% from 18.5 in 2003 to 7.4 in 2008 per 100,000 workers, and that of compensated coronary heart disease decreased by 60.5% from 3.8 in 2003 to 1.5 in 2008 per 100,000 workers. The compensated cases of CVD dramatically increased and reached its peak in 2003. Since many preventive activities were performed by the government and employers, the compensated cases have slowly decreased since 2003 and sharply decreased after 2008 when the diagnostic criterion was amended. The strategic approach is needed essentially because CVDs are common, serious and preventable diseases which lead to economic burden.

6.Injury of Renal Artery Branches by Blunt Trauma: Arteriographic Findings and Transarterial Embolotherapy.

PURPOSE: The purpose of this study was to describe the anglographic finding and clinical result of transarterial embolotherapy(TAE) in patients with injuries of renal artery branches by blunt trauma. MATERIALS AND METHODS: The study was based on retrospective analysis of seven cases, in which TAE was attempted for the control of traumatic renal arterial bleeding. All procedures were performed via the transfemoral approach. TAE was performed with stainless steel coil in two cases, Gelfoam in one case, and Gelfoam and stainless steel coil in four cases. RESULTS: Angiographic findings of vascular injuries were pseudoaneurysm in four cases, extravasation in two cases and arteriocalyceal fistula in one case. All procedures were performed successfully without complication. CONCLUSION: Pseudoaneurysm is a common anglographic finding in patients with injury of renal artery branches and TAE is considered a safe and effective method for treating such cases.

7.A case of true hermaphroditism with 46, XX/47, XXX karyotype.

True hermaphroditism is characterized by the coexistence of testicular and ovarian tissue. Authors experienced a case of true hermaphroditism which was confirmed an ovary in the left abdomen and testis in the right scrotum with a karyotype of 46, XX,47, XXY and report this case with the brief review of literatures.

8.Prostate specific antigen as a tumor marker for adenocarcinoma of the prostate.

Serum prostate specific antigen (PSA) and prostate acid phosphatase (PAP) were measured in 106 patients including 9 patients with prostate cancer, 63 patients with BPH. and 34 normal control group free of prostate disorders. The mean PSA value was 61.14+/-43.87 ng/ml for prostate cancer, 6.07+/-5.57 ng/ml for BPH and 1.42+/-0.98 ng/ml for normal control group, and the mean PAP value was 9.81+/-10.16 ng/ml for prostate cancer, 1.58+/-2.36ng/ml for BPH and 1.00+/-0.17 ng/ml for normal control group. The positive rates of PSA were 78% for prostate cancer, 22% for BPH and 0% for normal control group, and those of PAP were 67% for prostate cancer, 6% for BPH and 0% for normal control group. The sensitivity and specificity for PSA were 78%, respectively. We conclude that PSA may be a useful tool for detection and therapeutic monitoring of the prostatic cancer.

9.The Status of Occupational Accidents and Injuries and the Implications.

Since 1964, when the official statistics on occupational injury rates had been produced for the first time, the rates have showed a steady decline until 2000. However, the injury rate has been stagnant since 2000 although the fatal injury rate has decreased 40% in the same period. The non-fatal injury rate per 100 employees was 0.63 while the fatal injury rate per 100,000 employees was 9.74 in 2010. Traumatic injuries accounted for 86.8% of all injuries, followed by non-traumatic injuries at 8.4% and injuries by traffic accidents at 4.6%. Injuries caused by being caught in objects, hit by flying or falling objects and collisions have decreased while those caused by slip and fall on same level and fall from the height have increased. The decrease in injuries caused by being caught in objects may be the result of prevention efforts because 68.6% of them occurred in the manufacturing industry where most efforts for prevention have been invested. The increase in injuries involving slips and falls on same level may be caused by the growing number of employees in the service industry where 51% of the cases occurred. The construction industry accounted for 40.2% of all fatal injuries, and falls from the height caused 54.3% of the fatality. The non-fatal injury rate of Korea is lower than that of industrialized countries while its fatal injury rate is comparatively higher. It is probably caused by many unclaimed cases of mild non-fatal injuries whereas most fatal injuries were filed. Another problem is that the injury rate does not include injuries that occur to workers who are not eligible for the compensation scheme. This information can be obtained through a surveillance system or the national survey. Therefore, the attention of occupational physicians should focus on injuries as well as diseases occurring at work, and also on all working population whose injuries and diseases are not covered by the compensation scheme.

10.Two Cases of Surgical Intervention in Persistent Localized Choriocarcinoma.

Gestational trophoblastic tumors including choriocarcinoma bave become one of the most curable human malignancies with an overall cure rate exceeding 90%. Although systemic chemotherapy is the initial treatment for chorio- carcinoma, some patients with chemotherapy-resistant choriocarcinorna can be treated by integration of cbemotherapy, surgery and radio- therapy. We report two cases of persistent localized choriocarcinoma which was treated by surgical intervention.
