4.Hearing Thresholds in Electronics Sound Inspectors Exposed to Continuous and Impulsive Type Noises.

OBJECTIVES: To investigate the relation between different types of exposure to noise and a classic sign of noise-induced hearing loss (NIHL); the audiometric notch. METHODS: A cross-sectional epidemiological survey was carried out in electronics sound inspectors exposed to continuous and impulsive type noises. The noise levels in the working environments, and the audiometric hearing threshold levels were measured. RESULTS: Electronics sound inspectors were exposed to impulsive noise ranging from 74.1 to 88.9 dBA. The hearing loss induced by impulsive noise was greater and was characterized by maximum loss at 6 kHz audiometric frequency, as opposed to the notch at 4 kHz that is typical of continuous noise. CONCLUSIONS: The results suggest that the probable effect of impulsive noise on hearing should be considered even when the exposure is within the 85 dB equivalent A-weighted level. To diagnose NIHL it is important to elicit a detailed and accurate history of exposure to noise. Although the notch at 4 kHz is a well established clinical sign and may be valuable in confirming the diagnosis, the 6 kHz notch is variable and of limited importance.

Ankle arthrodesis has been regarded as a good operative treatment for post traumatic and rheumatoid arthritis of ankle, and ankle arthroplasty has been also developed with several types, from 1970, by Lord & Marotte. Ankle arthroplasty is more functional & less cosmetic troublesome than arthrodesis, and with more developed prosthesis design, more good result will come. The authors operated 6 cases of total ankle arthroplasty for posttraumatic and rheumatoid arthritis of ankle, which were also indications of arthrodesis, and introduce the results and operative problems of ankle arthroplasty.

The purpose of present study is to compare the effect of treatment using Guidor(R) as a barrier membrane in conjunction with platelet-derived growth factor and insulin like growth factors on experimental dehiscence defects. Following the resection of premolar crowns, roots were submerged. After 12 weeks of healing period, experimental dehiscence defects of 4mm in height and 4mm in width were surgically created on the mid-facial aspect of the lower premolar roots in each of 4 adult dogs. After root planning and demineralization of the root surface with citric acid, the control groups received 4% methylcellulose gel only, the test group I received 4% methylcellulose gel and were covered by Guidor(R) and the test group II were treated with PDGF and IGF and 4% methylcellulose gel with Guidor(R) coverage. Histological and histomorphometric analysis following 8 weeks of healing revealed the following results. 1. The new bone formation showed no statistically significant difference in all groups with 0.59+/-0.82mm(14.03+/-19.60%) for control, 0.70+/-0.39mm(16.30+/-9.01%) for group I, 0.87+/-0.76mm(18.74+/-16.03%) for group II. 2. The new cementum formation showed no statistically significant difference in all groups with 0.54+/-0.48mm(16.38+/-14.57%) for control, 0.95+/-0.38mm(23.43+/-9.30%) for group I, 1.01+/-0.75mm(22.10+/-16.11%) for gorup II. 3. The root resorption showed statistically significant differences betweenthe control group and all test groups(p<0.05) with 2.11+/-0.53mm(52.93+/-12.32%) for control, 0.63+/-0.27mm(15.32+/-7.05%) for group I, 0.89+/-0.33mm (19.26+/-7.11%) for group II. On the bases of these results, there were no statistically difference between treatment using resorbable membrane and resorbable membrane in conjunction with PDGF and IGF in the dehiscence defects, where it was difficult to maintain space. The use of membrane seemed to be more effective in the inhibition of root resorption.

9.Comparison of Propofol and Thiopental / Halothane for Short Duration Ophthalmic Surgical Procedures in Children.

Experiences with propofol in pediatric anesthesia are limited. We undertook a study to evaluate the quantity of induction and recovery from anesthesia with propofol compared to thiopental/halothane. Fifty children received 2mg/kg of propofol as a loading dose followed by a maintenance dose of 9mg/kg/hr for first 30minutes and followed by 6mg/kg/hr there- after. Fifty children received 5mg/kg of thiopental, and maintensnce was provided with halothane (0.5%-1.5%). The interval between the end of the administration of propofol or thiopental/halothane and extubation, as well as discharge to the ward,was significantly shorter with propofol (5.43 versua 10.72 min and 10.30 versus 18.20 min, respectively). Pain on injection site were seen more frequently with propofol, whereas nausea and hiccup were only observed with thiopental. During the first 6 hour after the surgical procedure, an- algesics were needed significantly more often in the thiopental group. In conclusion, propofol used as a single anesthetic for total intravenous anesthesia is a satisfactory agent for oph- thalmic surgery of short duration in children.

10.Atypical Noise-induced Hearing Loss As a Workers'Impairment Criteria.

OBJECTIVES: The problem of noise-induced hearing loss (NIHL) was objectively and systematically investigated by diagnosing three workers who complained of tinnitus and hearing disturbance. METHODS: Atypical hearing loss is diagnosed as a work-related disease by using general medical methods, such as environmental survey, neurological examination, otoscopy, pure-tone audiometry, speech audiometry, otoacoustic emissions (OAE), auditory-evoked potentials, and interview on the history of past disease, family and occupation. RESULTS: Three results were found according to the work-related hearing loss as follows; (1) hearing loss that is caused by exposure to continuous noise of less than 85 dB(A) or impact noise of greater than 135 dB peak, (2) mixed hearing loss that has progressed due to noise with past otitis media, and (3) atypical hearing loss that showed remarkable differences between air and bone conduction due to tinnitus. CONCLUSIONS: The criteria for work-related hearing loss should be carefully investigated by considering exposure to impact noise, the effect of continuous noise on mixed hearing loss patients, and the hearing loss caused by tinnitus.
