1.Correlation of the Bone Mineral Density with Morphometric Dimensions and Characteristics of Osteoporotic Vertebral Fracture.

This study was aimed (1) to evaluate osteoporotic vertehral fracture using normal morphometric dimensions in the Korean women (2) to test the relative importance of vertebral morphometry and fracture characteristics in predicting lumbar spine hone mineral density (3) to study clinical significance of the patients of 2 standard deviation to 3 standard deviation morphometric value. Normal morphometric data was ohtained from 60 heolthy women and 100 women with osteoporotic vertebral fracture defined by normal morphometry were evaluated. Fracture identification by 3SD cut off value was almost similar with that of qualitative method. Fracture identification by 2SD cut off value showed an increased number of fractures on the mid thoracic and thoracolumbar junction. Distrihution of the osteoporotic vertehral fracture showed double peaks on mid thoracic and thoracolumbar junction. Bone mineral density of third lumhar spine (LSBMD) in the multiple fracture group was significantly decreased from that of the single tracture group(p<0.05). LSBMD in the disperse fracture group was not different from that of the cluster fracture group(P>0.05). Comparing LSBMD of 3SD, 2SD-3SD fracture group and controls, the 3SD fracture group showed lowest value (P<0.01). But LSBMD of 2SD-3SD fracture group showed no difference with that of controls. Fracture threshold was 0.847gm/cm2 in 90th percentile. The correlation of LSBMD with the sum of normalized height of vertebral bodies, the sum of the deformity ratio of the vertebral body and spinal deformity index were all statistically insignificant.

6.The Effect of Chest Physiotherapy on the Amount of Tracheal Secretion and PaO2.

The purpose of this research was to find out the effect of chest physiotherapy on the amount of tracheal secretion and PaO2. After changing position of the neurosurgical patients who had tracheostomy cannula, experimental treatments were applied as bellows, manual chest percussion on group I, chest percussion and manual chest vibration on group II, chest percussion and mechanical chest vibration on group III were done. After these trials, we have analyzed the efficacy of each procedures comparing the group differences in the quantity of tracheal secretion and PaO2. Target samples were sixty patients aged between 20 to 60 who have tracheostomy state and decreased consciousness status that were admitted in NICU of a university hospital from June 1 to August 31, 1999. They assigned randomly into three experimental groups. To compare the effect of each interventions, tracheal secretion quantity was measured and PaO2 was analyzed via arterial blood gas analyzer. The data were analyzed by ANCOVA of 5% significance level using SPSS P/C program. The results were as bellows. 1) The first hypothesis " There is a difference in the quantity of the secretion among Group I, Group II and Group III" was accepted.(F=29.27, p=0.00) 2) The second hypothesis "There is a difference in PaO2 among Group I, Group II and Group III" was rejected.(F=1.71,p=0.19) From this study results, positional change and manual chest vibration including chest percussion were the most effective treatment to get maximum amount of tracheal secretion and it was confirmed that mechanical chest vibration also made much better effect than sole chest percussion method. Therefore, we concluded that the mechanical or manual chest vibration with chest percussion is more effective respiratory care method than the sole chest percussion.

8.The Analysis of Risk Factor and Infection Control of Carbapenem-Resistant Acinetobacter baumannii in a Medical Intensive Care Unit.

BACKGROUND: An epidemiologic study was performed after the outbreak of carbapenem-resistant Acinetobacter baumannii (CRAB) in the medical intensive care unit (MICU) from December 2006 to May 2007. METHODS: A retrospective case-control study was performed using the medical records of the patients. The case and control patients were compared for age, gender, total length of stay in MICU, prior carbapenem use, Acute Physiology and Chronic Health Evaluation II (APACH II) score, presence of central line, effect of mechanical ventilation, and sputum suction. Environmental and hand-washing studies were performed during the outbreak. RESULTS: Ten CRAB-affected patients and 29 controls were enrolled in this study. Univariate analysis showed that the age, total length of stay in MICU, presence of central line, and prior carbapenem use were associated with the CRAB outbreak. However, multivariate analysis showed that only prior carbapenem use was associated with the CRAB outbreak (odd ratio: 8.67, P=0.01). The outbreak disappeared after implementing a combined infection control strategy, including the sequential disinfection of MICU and strict compliance with cross-transmission prevention protocols. CONCLUSION: The use of carbapenem was associated with an increased risk of CRAB infection. This study suggests that the MICU contamination and infection transmission by health-care workers played a major role in the CRAB outbreak. Novel strategies such as restricted use of broad-spectrum antibiotics, strict hand hygiene, strict isolation of the patients, and MICU disinfection may be required to prevent the CRAB outbreak.

9.Options in Intracoronary Radiation Therapy.

Coronary restenosis is still regarded as Achilles' Hill of interventional cardiology despite relentless efforts of many investigators. Recent experimental and clinical studies have suggested that both gamma and beta radiation can be reduce restenosis after angioplasty. Currently, intracoronary brachytherapy for the prevention of restenosis has become a new evolving treatment modality in interventional cardiology. This report discusses a physical aspect of gamma and beta radiation, initial clinical results and delivery systems used in intracoronary brachytherapy. We shall take a brief overview of methods and their advantages in intracoronary brachytherapy. Future work should provide further insight for the best way of treating restenosis.
