1.4 Cases of Hemi-Kock Ileocystoplasty.

Augmentation cystoplasty is effective in the rehabilitation of the contracted bladder of small capacity caused by tuberculosis or interstitial cystitis refractory to more conservative methods of management. Genitourinary tuberculosis is the most common cause of contracted bladder in Korea and it may lead to considerable vesicoureteral reflux and progressive deterioration of renal function. The use of hemi-Kock ileal pouch with an afferent intussuscepted nipple is a suitable solution in patient in whom the bladder outflow tract provides adequate resistance and supratrigonal cystectomy is to be performed with construction of an antireflux mechanism. Herein, we report 4 cases of hemi-Kock ileocystoplasty.

2.A Case of Pityriasis Lichenoides et Varioliformis Acuta.

We report herein a case of pityriasis lichenoides et variloiformis acuta in a 48- year-old man, who has wide spread erythematous macule, papule, vesicle, hemorrhagic vesicle and reddish-brown crust on the neck, trunk, axillae and both extremities. He was treated successfully with tetracycline 2. 0 gm a day for 4 weeks.

3.Experiences in Ureteroileocecal Sigmoidostomy.

Ureteroileocecal sigmoidostomy is a method of continent urinary diversion which can protect the upper urinary tact from urinary reflux and infection, without external collecting devices and concomitant catheterization as in other forms of urinary diversion. Recently, we experienced 7 cases of ureteroileocecal sigmoidostomy in bladder cancer. Of them, 3 cases of immediate complications (retroperitoneal urinoma, wound disruption and ureteral obstruction ) were developed, but they could be corrected with prolonged placement of rectal tube or corrective surgery. And then, ureteral reflux, upper urinary tract infection and incontinence were not observed. So, ureteroileocecal sigmoidostomy is an attractive method of continent urinary diversion in selected cases.

6.The Influence of Taekwon

The bone mineral densities of 60 middle school boys were measured with dual photon absorptiometry to define the influence of Taekwon-Do. They were divided into Taekwon-Do and control group, each comprising 30 boys. The bone mineral densities of the head, trunk, pelvis, both upper and lower extremities, and body total were measured. The results were as follows. 1. The BMD of head was 1.668 ± 0.153 in Taekwon-Do and 1.554 ± 0.153 0.167 in control. It was significantly increased in aekwon-Do(p <0.05). 2. The BMD of trunk was 0.378 ± 0.153 0.044 in Taekwon-Do and 0.353 ± 0.153 0.053 in control. There was no significant difference(p >0.05). 3. The BMD of pelvis was 1.648 ± 0.153 0.112 in Taekwon-Do and 1.444 ± 0.153 0.215 in control. It was significantly increased in Taekwon-Do(p <0.005). 4. The BMD of right and left legs were 1.597 ± 0.153 0.118 and 1.579 ± 0.153 0.125 in Taekwon-Do and 1.425 ± 0.153 0.115 and 1.421 ± 0.153 0.113 in control group respectively. They were significantly increased in Taekwon-Do(p <0.005). 5. The BMD of right and left arms were 1.184 ± 0.153 0.090 and 1.178 ± 0.153 0.115 in Taekwon-Do and 1.056 ± 0.153 0.095 and 1.066 ± 0.153 0.097 in control group respectively. They were significantly increased in Taekwon-Do(p.<0.005). 6. The total body BMD was 0.932 ± 0.153 0.068 in Taekwon-Do and 0.846 ± 0.065 in control. It is significantly increased in Taekwon-Do(p<0.005). In conclusion, Taekwon-Do increases the total bone mineral density and the mineral densities of the head, pelvis and extremities but not that of trunk. It is thought that continued practice of Taekwon-Do throughout adult life might prevent the senile osteoporosis

7.Primary Cardiac Sarcoma in Pregnancy: A Case Report.

Primary cardiac sarcoma is a rare disease in adults. It is also associated with poor prognoses, due to diagnostic delay, therapeutic difficulty, and high metastatic potential. The coincidence of pregnancy and a primary cardiac intimal sarcoma is extremely rare. We report a pregnant woman at 27(+5) weeks gestation who was admitted to the hospital with acute-onset dyspnea. A mass was found on the left atrium by transthoracic echocardiography. Subsequently, the intracardiac mass was removed, and mitral valve replacement and modified DeVega tricuspid annuloplasty were performed. The patient was diagnosed with a undifferentiated sarcoma, and gave birth to a 1,230 g living baby boy by Caesarean section from preterm contraction at 29(+5) weeks gestation. The patient then received systemic chemotherapy. However, 10 months after the initial clinical onset, the patient suddenly died. Surgery is the standard treatment for cardiac tumors, and their removal should always be attempted, even in pregnant women. Although the overall survival rates of the patients are rather poor, palliative cardiac surgery allows the prolonging of pregnancy, until an acceptable fetal viability level is reached.

8.Effects of Leukotriene D4 on Ultrastructure of Nasal Mucous Membrane in Guinea Pigs.

The aim of this study was to evaluate effects of leukotriene D4(LTD4) on the nasal airway. Fifteen guinea pigs were divided into two groups;10 for experimental group and 5 for control group. Two l of 10(-5)mol LTD4 was administered to both nasal cavities in experimental group and same volume of normal saline in control group. Transmission electron microscopy was performed to investigate ultrastructural change of the nasal mucosal membrane in LTD4 administered guinea pigs. Infiltration of neutrophils and eosinophils from the capillaries were seen in the perivascular space. In the capillaries, some endothelial cells degenerated and intercellular spaces of the cells were widened, especially 6 hours later. Degranulated mast cells and neutrophils were found in the lumen of the capillaries. Several red blood cells with low electron density were seen in the shrunken vessels. The giant mitochondriae with inclusion bodies were observed. The results of this study suggest that leukotriene may be a potent mediator to induce biphasic response in the nasal passage of guinea pigs.

9.A Case of Takayasu's Arteritis with Congestive Heart Failure.

A 38 month old girl was admitted with the episode of anorexia, coughing and dyspnea to St. Mary's hospital on Nov. 9, 1977. She had treated as hepatitis at private clinic for one month before admission, however these symptoms were aggrevated with coughing and progressed dyspnea since a week ago. On physical examination, body temperature 36.8C, body weight 11.7kg, heart rate 150/min, BP 130/110mmHg at right arm, 160/90mmHg at left arm and 100/90mmHg at both legs were noted. The right radial pulse was palpated weakly compare to the left radial and both femoral pulses, which were also weak and bruit were heard on the both right carotid artery and abdominal aorta. On cardiac ausculation, no significant murmur was heard except gallop rhythm. The ECG and chest X-ray revealed the left ventricular enlargement and grade I K-W hypertensive retinopathy was found by fundoscopy. On selective left angiographic study mitral regurgitation(grade IV) was proved, and localized obliteration of the right subclavian artery and left carotid artery and diffuse constrictive narrowing of the thoracic and abdominal aorta at the level from D7 to suprarenal(type III) were seen by sequential ascending and abdominal aortorgaphy. The infrarenal abdominal aorta was also hypoplastic. Tuberculin skin test was strongly positive, and VDRL and LE preparation were negative.

10.Change of PSA and PSAD after Antibiotic Treatment in Patients with Prostatitis.

PURPOSE: Serum prostate specific antigen (PSA) is widely used for the early detection of prostate cancer, with biopsies often being performed when the serum concentration exceeds a defined threshold. The concept of prostate specific antigen density (PSAD) was introduced to enhance the specificity of serum PSA for cancer. We investigated the change in serum PSA and PSAD in patients with clinical prostatitis after antibiotic treatment. MATERIALS AND METHODS: A series of 39 patients with chronic prostatitis whose serum PSA and PSAD exceeded 4.0 ng/ml and 0.15 ng/ml/cm3, respectively, were reviewed retrospectively. After they received 4 to 8 weeks of antibiotics, the serum PSA concentration was remeasured. Men with persistently elevated serum PSA concentrations underwent prostate biopsy. RESULTS: The mean pretreatment serum PSA and PSAD was 8.73 ng/ml and 0.24 ng/ml/cm3, respectively. After antibiotic treatment, the mean values decreased significantly, to 4.55 ng/ml and 0.13 ng/ml/cm3, respectively (p<0.05). The mean decrease in serum PSA was 4.19 3.78 ng/ml, a mean relative change 44.3 31.7%. Among 39 men, 22 (56.4%) had their serum PSA and PSAD values return to the normal ranges. Prostate biopsy in 24 patients with persistently elevated serum PSA or PSAD after antibiotic therapy revealed prostate cancer in 4. CONCLUSIONS: Prostatitis is one of the most important factors in serum PSA and PSAD elevation in men without clinically detectable prostate cancer. A decrease in PSA and PSAD after antibiotic treatment can help in avoiding unnecessary prostate biopsies.
