2.Dermatological Side Effects of Anti-tumor Necrosis Factor Alpha Therapy.

As anti-tumor necrosis factor alpha (anti-TNFalpha) agents are progressively being used in various medical conditions, dermatological adverse events have been encountered more frequently. To understand such dermatological conditions that have been documented while undergoing anti-TNF therapy, we reviewed relevant literature, including case reports and case series. Reported dermatological conditions included infusion and injection site reaction, cutaneous infection, psoriasiform eruption, dermatitis, allergic rash, lupus-like lesion, vasculitis, lichenoid reaction, granulomatous reaction, hair loss, cutaneous infection, and cutaneous neoplasm. These events had varying strengths of causal association and severity therefore, drug discontinuation may or may not be required.

3.A Catheter Fragment in External Iliac Artery Cutted during Femoral Artery Cannulation: A case report.

Arterial cannulation for constant monitoring of arterial pressure and blood gase analyses has become commonplace in the care of the critically ill patients. The radial artery is often regarded as causing a negligible complication risk because of extensive collateral arterial flow in the hand. One of other alternative sites for arterial cannulation is femoral artery. Femoral artery cannulation has a high degree of success in very small, critically ill children. It should be considered an acceptable alternative to small-vessel cannulation when the latter is not technically achievable, or in the unstable patient where rapid establishment of reliable artery access is necessary. Arterial cannulation may cause many complications: arterial catheters can directly injure the vessels, resulting in thrombosis, occlusion, distal embolization or ischemia. Local insertion site complications, such as hematoma, hemorrhage, and infection may occur. Arterial catheter may also be a source of systemic sepsis. We report an unusual case of unintentional release of a catheter fragment into the external iliac artery in a 7-month (7.8 kg) male patient with tetralogy of Fallot, which was inadvertently inserted during right femoral artery cannulation. The catheter fragment was successfully retrieved with the Amplatz Goose Neck microsnare under fluoroscopy without any problems.

4.Sonographic-Pathologic Correlation of Gallbladder Adenoma: Adenoma versus Adenoma with Dysplasia.

PURPOSE: To correlate SOhographic and pathologic findings of gallbladder adenoma, and to evaluate the clinical significance of sonographic findings. MATERIALS AND METHODS: Ultrasound findings of twenty gallbladder adenomas were retrospectively reviewed to evaluate the size, shape and echogenicity of the adenoma, and was correlated with the pathological finding. RESULTS: Among 14 patients, 11 patients had single lesion and 3 patients had multiple lesions. Three patients showed 2, 3 and 4 adenomas, respectively. Nine of 20 lesions showed focal dysplasia pathologically. Among the nine adenomas with dysplasia, two adenomas showed focal cancerous change. The nine adenomas showing focal dysplasia measured 25.6mm (14-35mm) in mean diameter, while the mean diameter of adenomas without dysplasia was 8.7 mm (3-13mm). The echogenicity of adenoma with focal dysplasia were hyperechoic in 8, isoechoic in 1. The echogenicity of adenomas without dysplasia were hyperechoic in 7, isoechoic in 4. Sessile(7/9) and papillary shape(6/9) were predominant in adenoma with dysplasia, but smooth shape(8/11) and stalked type(9/11) were predominant in adenoma without dysplasia. Two adenomas with focal cancerous change showed histological transition from cancer to dysplasia and to adenomatous tissue. In adenoma with dysplasia, the diameter more than 14 mm on sonography was statistically significant (p<0. 005). Also age of patient was significantly different between the two groups (p<0.01), while echogenicity and associated stone were not statistically significant. CONCLUSION: As gallbladder adenoma more than 14ram in diameter on US is suggestive of dysplasia on pathology, so, close follow up US or surgery is recommended.

Lipoleiomyomas of the uterus are extremely rare benign neoplasm that can be difficult to differentiate from benign cystic ovarian teratomas. Only a few pure lipomas of the uterus have been documented. We presented a case of lipoleiomyoma of the uterus, in which sonography and computed tomography were per formed, but because of its rarity, and confusion with more common uterine leiomyoma and ovarian dermoid tumor, preoperative diagnosis was not suggested. To our best knowledge, this is the first case of uterine lipoleiomyoma in Korea.

8.The Significance of Clinical Symptoms in Dry Eye Patients and Statistical Analysis of the Patiants.

From April 1 to July 30, 1989, we studied 188 subjects(male 25, female 163) with suspected dry eye based on their clinical symptoms and their response to conventional treatment with artificial tear. Schirmer Test and Tear break-up time measurement(B.U.T.) were performed on each patient. There have been many reports that Schirmer test and B.U.T. measurement can be influenced by many enviromental factors and testing methods so the question of their relative value has not yet been sloved. the purpose of this study was to evaluate the importance of clinical symptoms and signs in diagnosing dry eye. Demographic aspect of dry eye patients in Korea was also analysed. The results were as follows: 1. The sex distribution was 163 females(87%) and 25 males(13%). There were absolutely more females than males. 2. The ages ranged from 15 to 83 years old. The peak age was from 41 to 50 years(29%). 3. Among 163 female patients, the majority 103(63%) were menopausal and postmenopausal females. 4. Among major chief clinical complaints, foreign body sensation showed the highest frequency with 67 patients(35%). 5. Major aggravating factor of the disese was prolonged use of eye which showed highest frequency with 122 patients(65%). 6. There were no statistically significant differences in the mean value of BU.T. and Schirmer test between right left eye(p<0.05). 7. In the majority of the cases 153(82%) the B.U.T. and Schirmer test value were 8 sec and 8mm or below respectively.

A 33-year-old woman who was diagnosed as medullary carcinoma by fine needle aspiration of thyroid mass is presented. The smear revealed dispersed pattern and small clusters of cells without follicular or papillary structures. The nuclei were round, oval or spindle shaped and eccentrically located. The cytoplasm was abundant and showed red-stained cytoplasmic granulation and tail-like projection with indistinct border. Clumps of amorphous, light-green material were intermingled with tumor cells.

10.A Case of Hypersensitivity Reaction Induced by Abacavir in an AIDS Patient.

Abacavir is a nucleoside reverse transcriptase inhibitor that is commonly used in HIV-infected patients. A well-known and potentially life-threatening side effect of abacavir is allergic hypersensitivity reaction. A screening test for the HLA-B*5701 allele is currently used to predict the risk of hypersensitivity reaction to abacavir. This test, however, may be less useful in Korea, because of the low prevalence of HLA-B*5701. A 52-year-old male with HIV infection was referred to our hospital because of suspected side-effects of antiviral agents and lymph node enlargement of the neck. He suffered from a fever, generalized edema, skin rash of the whole body, and difficulty breathing after starting antiviral agents. Suspected as a hypersensitivity reaction resulting from drug side-effects, prescription of abacavir was stopped. The patient subsequently recovered. The presence of the HLA-B*5701 allele was confirmed by polymerase chain reaction-sequencing based typing (PCR-SBT).
