1.Cerebral Arterial Embolism Treated by Intra-Arterial Infusion of Urokinase Which Was Occurred during Percutaneous Balloon Mitral Valvuloplasty.

Cerebral arterial embolism which resulted in total occlusion of right proximal middle cerebral artery was occured in 33 years old female patients with severe mitral stenosis and atrial fibrillation during percutaneous mitral balloon valvuloplasty(PMV). The occluded artery could be detected by transfemoral carotid angiography and recanalized immediately by intra-arterial infusion of urokinase. There were no cerebral hemorrhage by CT scan after intra-arterial urokinase infusion and the patients discharged without any neurologic sequale 7 days after thrombolysis. We report a case of successful recanalization with intraarterial infusion of urokinase in patient with cerebral arterial embolism occurred during PMV.

2.Comparison of Surgical Results Between Bilateral Rectus Muscle Recession and Lateral Rectus Muscle Recession and Medial Rectus Muscle Resection in Exotropia Over 40 Prism Diopters.

It is the purpose of this study to compare the postoperative results of 168 patients with exotropia of basic and simulated divergence excess type over 40 prism diopters; 86 patients had bilateral rectus muscle recessions and 82 patients underwent lateral rectus muscle recession and medial rectus muscle resection. The patients in the former group had averaged 13.7 years of age and 8.2 months of postoperative follow-up period. Those in the latter group averaged 22.9 years and had been followed up for 7.1 months. The postoperative success rates are as follows: 66.7%(57/86) in the bilateral rectus muscle recession group and 68.1%(58/82) in the lateral rectus muscle recession and medial rectus muscle resection group(P>0.05). And the success rates of those older than 16 years without amblyopia are 60.0%(18/30) in the former group and 75.0%(24/32) in the latter group(P>0.05). The lateral rectus muscle recession and medial rectus muscle resection group showed somewhat higher rates of success even though it proved statistically insignificant.

3.Keratoacanthoma in Co-existence with Nevus Sebaceus.

Nevus sebaceus of Jadassohn has been referred to as a organoid nevus that classically evolves through three stages and may be associated with a range of skin tumors, and of these, syringocystadenoma papilliferum and basal cell carcinoma have been commonly reported to occur. The occurrence of keratoacanthoma is a very rare event in nevus sebaceus. Herein we report on a 23-year-old man with nevus sebaceus of the cheek. He represents a keratoacanthoma arising within nevus sebaceus.

4.Correlation of Histologic Types With Clinical findings and Transrectal Ultrasonographic Pictures in Benign Prostatic Hyperplasia.

Tissues obtained by transurethral prostatectomies were evaluated histologically and correlated with the clinical findings and the transrectal ultrasonographic pictures of 104 patients with benign prostatic hyperplasia (BPH). The patients were divided into 3 groups according to the following histology. predominantly glandular types (30 patients), predominantly stromal types (35 patients) and mixed types which have equal proportions of glandular and stromal hyperplasia (39 patients). The average age was highest in the glandular group. Postoperative obstructive symptom scores were significantly lower in the stromal type as compared to the glandular and the mixed type. The maximal flow rates at three months following the operations were highest in the stromal type. In comparison of the histologic findings with preoperative transrectal echographic pictures, the transverse and the longitudinal diameter of the transitional zone were significantly lower in the stromal type than in the glandular and the mixed type. The weight of transitional zone was significantly smaller in the stromal type than in the glandular and the mixed type. The echogenicity showed hyperechoic and isoechoic pattern in the stromal and the mixed type and hypoechoic and isoechoic pattern in the glandular type. The echographic homogeneity, the frequency of prostatic calculi and cyst formation, and the margination of the transitional zone did not show any significant difference among each group. In conclusion, the histologic types of BPH have correlations with the clinical findings and the transrectal ultrasonographic pictures. Furthermore precise interpretation of the histologic types of BPH by transrectal ultrasonography will be helpful in decision making about the treatment modality and in prediction of the clinical progress after some treatment for the patients with BPH.

The importance of tick-borne diseases is increasing because of climate change, with a lack of long-term studies on tick-borne pathogens in South Korea. To understand the epidemiological characteristics of tick-borne diseases, the monthly distribution of field ticks throughout the year was studied in South Korea between May 2014 and April 2018 in a cross sectional study. The presence of various tick-borne pathogens ( During the study period there were 11,717 ticks collected and 4 species identified. In conclusion, due attention should be paid to preventing tick-borne infections in humans whilst engaged in outdoor activities in Spring and Autumn, particularly in places where there is a high prevalence of ticks.
6.Risk Factors of Dislocation Occurring after Acetabular Component Revision.

Purpose: To analyze the causes and to prevent dislocations of the hip joint that occur in patients who underwent revisions of total hip arthroplasties by changing only the polyethylene liners and femoral heads, subsequent to primary total hip arthroplasties. Materials and Methods: We evaluated 28 patients who underwent acetabular component revisions of total hip arthroplasties subsequent to primary total hip arthroplasties. The average age of the patients was 55 years old and all 28 patients had operations through the posterolateral approach. In 17 of the cases, the acetabular cups, polyethylene liners, and femoral heads were changed; in 10 cases, just the polyethylene liners and femoral heads were changed; and in 1 case, only the polyethylene liner was changed. Results: We observed 7 cases of hip dislocations that occurred among a total of 28 cases after revisions of total hip arthroplasties. In all 7 cases, the polyethylene liners and femoral heads were changed, but not the acetabular cups. In additional, in all 7 cases of dislocation, the patients were non-compliant and started weight-bearing prematurely after revisions of the total hip arthroplasties Conclusion: We observed new dislocations in the 7 cases in which the polyethylene liners and femoral heads were changed. The main reasons were inadequate soft tissue tension and hip joint laxity. Therefore, it is necessary to increase the neck length, use elevated rim polyethylene liners, readjust the acetabular cups to their optimal positions, and apply hip abduction braces to patients early after revision of total hip arthroplasties in order to lessen the danger of dislocations.

7.Influencing Factors on Infant Mothers' Happiness

PURPOSE:
This study aimed to provide preliminary data for a nursing intervention plan to promote happiness of mothers with infants. Therefore, factors affecting happiness of mothers with infants were investigated.
METHODS:
Subjects were 142 mothers with children aged 1 to 3 years, in daycare centers and kindergartens located in Busan metropolitan city. Data were collected using self-reported questionnaires.
RESULTS:
Pressures pertaining to the parental role and distress related to subfactors of parenting stress, leisure activity (among subfactors of fathers' involvement in parenting), and marital satisfaction were significant predictors of happiness for mothers with infants. These variables explained 47.6% of the differences in happiness of mothers with infants.
CONCLUSION
Based on the above results, mothers with infants are happier when pressure and distress from parenting decrease and leisure activities resulting from fathers' involvement in parenting and marital satisfaction increase. Therefore, to increase the happiness of mothers with infants, the development of a nursing intervention program is necessary. Specific goals of the program should include promoting leisure activity (as fathers become more involved in parenting), decreasing distress from parenting, and improving marital satisfaction levels.
8.A Case of Huge Retroperitoneal Lipoma.

Retroperitoneal lipoma probably constitute the group of the most massive solid abdominal tumors; their etiology is unknown; their symptoms insidious; and recurrence and sarcomatous change frequent. The clinical diagnosis was made by clinical symptoms, physical findings and radiologic evaluation such as intravenous urography, ultrasonography, computed tomography, angiography and so on. Abdominal CF scan provided clinically the most useful information regarding the state of tumors and also their effect on adjacent structures and shows homogenous appearing fat tissue with a low attenuation coefficient identical to that found in the subcutaneous fat. We present a case of treatment for a huge retroperitoneal lipoma in a 46-year-old man.

Gallstone ileus is a well-known complication of cholelithiasis, but is relatively rare. Most ectopic gallstones are located in the small bowel; they are rarely found in the stomach and duodenum.
