2.The Effect of Topical Dapiprazole on Phenylephrine and Tropicamide-induced Mydriasis.

In order to evaluate the effect of topical dapiprazole, an alpha-adrenergic receptor blocker, in reversing mydrisis by 1% tropicamide and 2.5% phenylephrine, we studied 89 subjects. 0.5% dapiprazole in one eye of each subject after full dilatation of both pupils. The pupillary diameter was calibrated, and thereafter statistical analysis was performed with student paired t-test. Thirty minutes after instillation of dapiprazole, there was a highly significant difference of the decreasing rate in pupil size between the dapiprazole treated eyes and the control eyes(p<0.001). The time for complete reversal of mydriasis was also at least 4 hours shorter in the dapiprazole treated eyes than the control eyes(p<0.006). This study suggests that 0.5% dapiprazole is effective in reversing mydriasis caused by 1% tropicamide and 2.5% phenylephrine, and also effective in making the eyes comfortable in a shorter time.

3.The Effect of Pterygium on Corneal Astigmatism and Visual Acuity.

To evaluate the effect of pterygium on corneal astigmatism and visual acuity, we checked the corneal curvature and visual acuity before and after the operation for pterygium in 126 eye. In the cases of Grade I, the mean change of the horizontal corneal curvature was +0.32D and the mean change of the vertical corneal curvature was +0.07D (P<0.05). In the cases of Grade II, the mean change of the horizontal corneal curvature was +0.54D(P<0.01) and the mean change of the vertical corneal curvature was +0.09D(P<0.05). All of the horizontal and vertical corneal curvature increased after removal of the pterygium, especially the horizontal curvature increased more than the vertical curvature. As the pterygium invades the cornea more extensively, the corneal curvature increased more markedly. After the operation for pterygium the visual acuity was improved 1 to 2 lines at the Han`s visual acuity table in some cases, but there was no statistical significance (P>0.5).

4.Association between betaARK1 Level of Circulating Mononuclear Leukocytes and Left Ventrcular mass in Non-treated Hypertensive Patients.

BACKGROUND: Beta-adrenergic receptor Kinase 1(betaARK1) is a serine/threonine kinase attached, which inhibits the coupling of beta-adrenergic receptor with G-protein. Myocardial betaARK1 level is usually elevated in heart failure and hypertrophy, but it is not known whether the circulating betaARK1 level is related with the degree of cardiac hypertrophy. This study was performed to evaluate the association of the betaARK1 level in circulating mononuclear leukocytes(MNL) in untreated hypertension with left ventricular mass in hypertensive patients. Method: Nineteen non-treated hypertensive patients were included for this study. High blood pressure was confirmed when systolic BP is over 150 mmHg or diastoli BP is over 95 mmHg. Echocardiography was performed to evaluate the degree of hypertrophy by measuring the left ventricular mass index(LVMI) and relative wall thickness(RWT), and test the LV function by measuring the ejection fraction(EF) according to ASE guideline. At the same time, blood was collected from each patient and MNL were isolated by gradient centrifuge with Ficoll-400. Total RNA was purified from MNL and semi-quantitative RT-PCR was performed. After reverse transcription, PCR was done with primers for human betaARK1 and GAPDH as external control. betaARK1 levels were expressed by ratio to GAPDH level and estimated the relations with clinical and Echocardiographic parameters. Result: We studied confirmed 19 hypertensive patients(10 men and 9 women, mean age of 50.6 years). Echocardiographically measured indices(mean+/-SD) were as follows; LVMI(137.3+/-30.6g/m2), PWT(0.53+/-0.09) and EF(54.6+/-8.5%). Ratio of betaARK1 levels to GAPDH was from 0.10 to 0.96 (0.62+/-0.25). betaARK1 levels were correlated with LVMI(correlation coefficient: r=.502, p=.029) and RWT(r=.627, p=.004). But Systolic BP(r=0.009, p=.93), diastolic BP(r=.07, p=.85) or EF(r=.045, p=.84) were not related to level of betaARK1. CONCLUSIONS: The betaARK1 level of circulating MNL was correlated well with the degree of the cardiac hypertrophy estimated by LVMI and RWT. This data suggests that activation of sympatho-adrenal system would exert a major role in developing cardiac hypertrophy and we can expect the decreased responsiveness to catecholamine in the heart of hypertensive patients. betaARK1 in circulating MNL might be used as a predictor or marker for LV hypertrophy in hypertensive patients.

5.The Use of Autogenous Periosteal Grafts for the Periodontal Regeneration in Mandibular Class II Furcation Defects in the Dog.

Autogenous periosteal grafts are an attractive alternative to existing barrier membrane materials since they meet the reqiurements of an ideal material. But no histological data are available on the effectiveness of periosteal membranes in the treatment of periodontal defects. The purpose of this study was to evaluate effect of autogenous periosteal graft on periodontal regeneration histologically. Class II furcation defects were surgically created on the second, third and the fourth premolars bilaterally in the mandibules of six mongrel dogs. The experimental sites were divided into three groups according to the treatment modalities; control group - surgical debridement only; Group I- autogenous periosteal membrane placement after surgical debridement; Group II- autogenous periosteal membrane placement after surgical debridement and bone grafting. The animals were sacrificed at 2, 4 and 12 weeks after periodontal surgery and the decalcified and undecalcified specimens were prepared for histological and histometrical analysis. Clinically all treated groups healed without significant problems. Under light microscope, at 2 weeks, control group showed significant apical epithelial migration and bone remodelling only below the notch area. But for the group I, II with autogenous periosteal graft, less apical migration of epithelium appeared and large amount of osteoid tissue showed above the notch area. Grafted periosteal membrane was indiscernable at 4 weeks, so periosteal membrane might be organized to surrounding tissues. Histometrically, at 4 and 12 weeks, all the test and control groups didn't show significant change of epithelial zone but new attachment level tended to be gained in the test groups than control group. These results suggest that autogenous periosteal grafts should be a good alternative for guided tissue regeneration.

The origin of osteoclast-like giant cell tumor (OGCT) of the salivary gland has been debated because the prototypic cells of osteoclast-like cells and mononuclear stromal cells are largely unexplained in this gland. Bone marrow-derived CD14+ and CD45+ monocyte-derived multipotential cells (CD14+/CD45+ MOMC) may be one of the possible origins of OGCTs of salivary glands, which have never been explored in salivary OGCTs. We present a case of OGCT accompanied with carcinoma ex pleomorphic adenoma in the parotid gland of a 67-year-old Korean female. The tumor presented as a rapidly growing cervical mass comprising a central area of carcinoma ex pleomorphic adenoma and a peripheral circumferential area of OGCT. The immunohistochemical staining pattern was phenotypically consistent with bone marrow-derived CD14+/CD45+ MOMC. This case is the first report of a salivary OGCT in Korea.

8.A Study of Impact on Head and Neck Using Human Volunteer Low-Speed Rear Impact Tests.

Whiplash injury in low-speed traffic accidents are not objectively verified by medical equipment, thereby creating scope for misuse, which has resulted in huge social losses worldwide. The aim of this study was to examine the influence of low-speed vehicular rear-impact collisions on middle-aged men, and to analyze the head and neck injury criteria for the symptomatic human volunteers. Data was examined from the results of 50 dynamic sled tests, originally performed by Hong et al. (2012). In the previous tests, 50 men aged 30~50 years were exposed to an impulse equivalent to a bumper-to-bumper rear collision under medical supervision, and no resulting whiplash injury was identified. In this study, for 6 subjects who experienced dull aches over their bodies, head injury criteria (HIC15) and neck injury criteria (N(km)) were calculated according to the accelerations, forces, and moments at the occipital condyle measured by motion capture system. Although there were no changes in magnetic resonance imaging findings in all subjects at the pre-/post-test orthopedic examination, 6 subjects revealed mild aches around the shoulder, back, or lumbar area, and their symptoms disappeared within 2 days. The head and neck injury criteria, HIC15 (3.086 +/- 2.942) and N(km) (0.077 +/- 0.064) were obtained, and the maximum HIC15 and N(km) were found to be significantly lower than the critical injury assessment reference values (HIC15: 700, N(km): 0.3). Moreover, even though 2 subjects were exposed to the same level of change of velocity (7.9 km/h), each N(km) was significantly different (0.179, 0.057). One can therefore conclude that N(km) can vary according to voluntary movements in the human subject.

9.A survey of the Refractive State of Elementary School Children in Rural Area.

To evaluate the refractive error and glasses wering state, we performed screening eye examination on elementary school children living in rural area. Among 1254 shcool children(male : 542, female : 712), 352(28.1%) children were emmetropia and 902(71.9%) children ametropia. Three hundred ninety two(72.3%) of 542 male students and 510(71.6%) of 712 female students were not emmetropia. There was no statistical significance in sexual difference of refractive errors. As refractive errors, 38.3%(481) were myopia, 27.8%(348) hyperopia, and 5.8%(73( mixed astigmatism respectively. In diopteric distribution of refractive errors, refractive error under +/-2.0D was 72.0%(650/902). Among children with refractive errors, 333(36.9%) children did not require glasses, but rest of childfen needed glasses. Three hundred nine(34.3%) children did not wear glasses inspite of refractive errors, 33(3.7%) children were wearing incorrect glasses and 2 children were wearing glasses unnecessarily. In children, early eye examination is essential for their eye health. Therefore attention to early eye examination and correct glasses wearing seem to be very important especially for child living in rural area.

10.Analgesic and Anti-inflammatory Effects of Ibuprofen, Indomethacin and NS-398 on an Acute Model of Arthritis in Rats.

BACKGROUND: All currently available nonsteroidal anti-inflammatory drugs (NSAIDs) inhibit both cyclooxygenase-1 and cyclooxygenase-2 and exhibit many complications. It has been suggested that the anti-inflammatory and also most of the analgesic effects of NSAlDs result from an inhibition of arachidonic acid metabolites synthesised via cyclooxygenase-2. In the present study, the extent of analgesic and anti-inflammatory effects of ibuprofen (a non-selective cyclooxygenase inhibitor), indomethacin (a selective cyclooxygenase-1 inhibitor) and NS-398 (a selective cyclooxygenase-2 inhibitor) are investigated in on acute model of arthritis in rats by a behavior test and pathologic examination. METHODS: Arthritis was induced with 2% kaolin and 3% carrageenan into the right knee joint cavity under enflurane anesthesia (2 - 4%). Before and after the injection, rats were allowed to walk freely through a pathway, constructed to record weight load by means of 8 weight sensors attached to 8 plates which function independently. Weight bearing, the weight of rat and the diameter of the knee joint were measured serially before and after the injection. At 6 hours after the injection, ibuprofen, indomethacin and NS-398 were injected intraperitoneally (1, 5 and 25 mg/kg/ml). RESULTS: In the control group, weight bearing decreased maximally and the weight bearing ratio increased maximally at 6 hours after inflammation and the diameter ratio increased maximally 1 day after inflammation. There were improvements in weight bearing with ibuprofen, indomethacin and NS-398 in a dose-dependent manner at 8, 10 and 12 hours after induction of arthritis. NS-398 demonstrated better analgesic and anti-inflammatory effects than ibuprofen or indomethacin at a low dose (1 mg/kg). In contrast to NS-398, significant analgesic effects of indomethacin on the behavior test was not shown at a low dose. CONCLUSIONS: These results suggest that the selective cyclooxygenase-2 inhibitor plays an important role as an analgesic and anti-inflammatory drug.
