2.A Case of Diffuse Aspiration Bronchiolitis in a Dysphagic Infant.

Diffuse aspiration bronchiolitis is defined as a clinical entity characterized by a chronic inflammation of bronchioles caused by recurrent aspiration of foreign particles. Clinical symptoms are bronchorrhea, bronchospasm, and dyspnea, and chest radiographs show the presence of regional or disseminated srnall nodular shadows and hyperlucency. Chest CT should help in detecting diffuse nodular shadows of bronchiolitis. Pathologic findings of diffuse aspiration bronchiolitis are characterized by localization of chronic mural inflammation with foreign body reaction in bronchioles. Recurrence of small amounts of aspiration might play an important role in the pathogenesis of diffuse aspiration bronchiolitis. We report a case of diffuse aspiration bronchiolitis in a 4-month- old female infant who had recurrent aspiration due to dysphagia and presented with recurrent fever, dyspnea and wheezing. She showed typical radiologic and histologic findings compatible to diffuse aspiration bronchiolitis. She was improved with treatment of nasogastric tube feeding. We emphasize the importance of recognizing this disease entity and differentiating it from pulmonary diseases associated with bronchospasm. (J Korean Pediatr Soc 2000;43:842-845)

3.The Actual State of Industrial Accidents in Small-medium Manufacturing Industries.

PURPOSE: This study was conducted to assess reported industrial accidents and non-reported industrial accidents that took place in 63 small and medium sized manufacturing industries located in Daegu and Gyeongsangbuk-do in the year of 2009. METHODS: During the period from January to December 2009, the number of industrial accidents that happened each month was examined according to the characteristics of industry, industrial accidents, workers with industrial accidents, treatment, as well as causes and treatment of the non-reported. RESULTS: The total incidence rate of industrial accidents was 3.48%, of which only 0.31% reported. The incidence rate in packing and distribution among manufacturing workplace, and in the industry with 50~99 employees, was higher than the other groups. Of the non-reported, over 80% in required under 10 days of medical treatment, and 56% in required cost of less than 100,000 won. Kappa values were 0.771 concurrence in opinions for causes of non-reported, and 0.571 concurrence in opinions for management termination of non-reported between the employees and employer. CONCLUSION: It is suggested that more supportive policy and precise, nationwide survey on the actual state of industrial accidents, including non-reported cases, be implemented to efficiently manage industrial accidents.

6.Vaccinia of the Lids.

Ocular vaccinia usually is seen as a disease of the lids but primary involvement of other portions of the eye, such as conjunctiva and cornea, is uncommon. The disease can result from autoinoculation and contamination after contact with the vaccination vesicle. Ocular vaccinia is most likely to occur in patients who have not had previous successful vaccinations and in those previously vaccinated who do not have adequate antibody levels against vacclIua. The case reported in the following was a 2 year old girl who noted ocular vaccinia in her right upper and lower eyelids in 5 days after smallpox vaccination.

7.The Congenital Coloboma of the Choroid.

We report of 3 cases of atypical coloboma of choroid. The two of these were one male and one female. A man aged 40 discovered atypical coloboma of choroid bilaterally with red-green color blindness. One of his son aged 6 discovered atypical coloboma of choroid bilaterally with nystagmus. One of his daughter aged 11 discovered of atypical coloboma of choroid bllaterally with alternating exotropia.

8.Experimental Strebismus Surgery with Tissel in Rabbit Models.

We performed experimental surgery with Tisseel in rabbit models, to determine whether Tisseeling can replace the classic suture technique. 30 albino rabbits were divided into two groups-one group was Tisseel group(15 rabbits) and the other was Vicryl group(15 rabbits). The superior rectus and inferior rectus were recessed and resected 5mm. Both groups were killed 2,4, and 6 weeks after the surgery respectively and the eyes were enucleated. The tensile strength and pathological findings were compared between the two groups. From this study, we found that Tisseeling had good tensile strength and minimal tissue reaction. Thus Tisseeling is considered to be good method in strabismus surgery.

9.Validity and Reliability of a Clinical Performance Examination using Standardized Patients.

PURPOSE: The purpose of this study was to test the validity of a modified clinical performance examination (CPX) for preclinical students in nursing. METHOD: 70 nursing students in their second semester of the junior year at C University participated in CPX. Scenarios and checklists were developed by our research team from September to October 2005. Six stations were organized. Evaluation included physical examination of a patient with lung cancer, education on usage of a metered dosage inhaler, and lobectomy postoperative care. Students were randomly assigned to a station. RESULT: There was a difference in the CPX scores according to stations. The agreement of scoring between trained faculty members and SPs was more than moderate (r=.647). The correlation between the CPX score and the average grade in the previous semester and between the CPX score and the average grade of a paper and pen test of the pulmonary system of adults was low (r=.276; r=.048). CONCLUSION: Traditional CPX is generally recommended, however, modified CPX is appropriate for preclinical students in the current Korean Nursing school setting if there are additional scoring systems to balance the testing level at each station.

10.Discussion on Expansion and Establishment of Home Nursing Care.

PURPOSE: The purpose of study is to develop a home nursing care project model suitable for Korea's heath care system and policy directions for expansion and establishment of home nursing care. METHOD: The first, status of home nursing care program in medical institution and public health center was evaluated respectively in view of structure, process and performance. And then, issues of the program were analyzed in view of accessibility, safety, and sufficient supply. The second, demand projection of home nursing care according to income level and technical level of service that is needed to the subject was tried. In addition, the level of supply for home nursing care at present was estimated. The third, home nursing care program in advanced countries (Japan and U.S.A.) was examined. RESULT AND CONCLUSION: Community-based home nursing care program is developed into public-operation model and private-operation model from above the sub-subject result. Functional network for referral system among related institutions is built up to meet various needs, regardless of accessibility to distance and economy. And prior settlements and policy directions for expansion and establishment of home nursing care are suggested.
