2.The Surgical Treatment of Osteoporotic Vertebral Collapse Caused by Minor Trauma.

With an aging population, osteoporotic vertebral collapse is an increasingly common condition. This compression fractures has been considered a benign entity, quite responsive to conservative treatment. In a rare patients, however, a major neurologic complication and painful kyphosis despite conservative treatment can develop. Therefore, the purpose of this present study is to analyze the surgical results of 14 patients with severe back pain, an increasing kyphosis and neurologic deficits caused hy osteoporotic vertebral collapse following minor trauma, who were treated surgically. Presenting signs and symptoms included severe back pain with progression of kyphosis in 6 patients and increasing neural deficit in 8 patients. Of 14 patients, eight patients had an intravertebral cleft sign (vacuum sign). Indications for surgery included increasing kyphotic deformity, intractable pain, or increasing neurologic deficit. There was no correlation between intravertebral cleft sign and neurologic deficit. However, patients who had intravertebral cleft sign had not well respond to conservative treatment. As treatments, combined anterior and posterior fusion in 8, anterior fusion in 4, posterior instrumentation, and wide decompressive laminectomry in one patient, respectively, were carried out. The final correction of the deformity averaged 0.3 degrees. Therefore. correction of kyphosis was not favorably maintained because of variable surgical methods, and sinking of graft bone or instrumentation into the osteoporotic vertebral bodies. However, pain was reduced significantly in all patients. In addition neurological symptoms improved in 7 patients. One patient underwent reoperation with nnterior inierbody tusion together with anterior instrument because of an increasing kyphosis, neurologic. iymptoms and scvcre hack pain following wide decompressive laminectomy. There was no complication relatecl to instruments. The authors strongly helieved that surgical intervention has highly satisfactory results in patients who have intravertehral cleft sign with persistent back pain despite conservative treatment, and proressive or persistent neurologic deficits following osteoporotic vertebral collapse.

3.Aging Effect on Intraocular Pressure and Associated Factors.

Background : There is a substantial body of evidence that intraocular pressure increase with age among western population. However, Japanese study showed that intraocular pressure was inversely related with age. So we investigated whether intraocular pressure is related with age for Koreans. Methods : Between March, 1996 and February, 1997, 6201 subjects underwent comprehensive health examinationinthe Kyung Hee medical screening center in Seoul, Korea. The intraocular pressure was measured with pneumatonograpyh. At the same time blood pressure, body weight, height, serum osmolarity, fasting blood glucose, T3, T4, TSH(thyroid stimulating hormone), hematocrit measurements were performed. We selected 250 case randomly and investigated whether the distribution of C/D ration(cup/disk ratio), smoking status, alcohol drinking amount are uniform by aged group. So we measured C/D ratio by fundusphotograph and investigated smoking and drinking habits by questionnaires. Results : In the randomly sampled 250 cases, the amounts of smoking and alcohol consumption were lower in the elderly group. But, the C/D ration didn't show any difference between young and elderly group. In a multivariable analysis, both right and left intraocular pressure showed significantly inverse correlations with aged and visual acuity. Male had significantly higher intraocular pressure than female. Both right and left intraocular pressure showed significant positive correlations with body mass index, fasting blood glucose, hematocrit and systolic blood pressure. Diastolic blood pressure, serum osmolarity, T3 and T4 were not associated with intraocular pressure. TSH had significant positive correlation with right intraocular pressure, but no correlation with left intraocular pressure. Conclusion : Age was independently and inversely related with intraocular pressure for Koreans.

4.Intravascular Ultrasound Assessment of the Coronary Intervention.

BACKGROUND: Coronary arterigraphy has been used as a tool to assess the degree of coronary artery narrowing and the result of balloon angioplasty, which frequently underestimates the degree of atherosclerosis. Intravascular ultrasound(IVUS) can give more delicate information about plaque morphology and the result of coronary intervention. We compared qualitaive and quantitative measurement between IVUS and coronary angiography after coronary intervention. METHODS: We used 30 or 20 MHz intravascular ultrasound catheter which was connected to Hewlett Packard Sonos 1500 Intravasscular equiment in 5 coronary balloon angiopasty and 3 Palmaz-Schatz stent implantation cases. Sites of intervention were at the left anterior descending coronary artery in 7 patients and at the left circumflex artery in one patient. Quantitative coronary angiography(QCA) was done by CAAS II system. We measured referenc diameter(RD), minimal lumen diamter(MLD), Lumen and vessel cross sectional areas(LCSA,VCSA) obstraction area(OA) and plaque area(PA) and also analysed plaque morphology. RESULTS: 1) IVUS is more sensitive in the detection of eccentricity, Calcification and dissection. 2) Before intervention, the mean reference diameter was 2.87+/-0.42mm,3.07+/-0.39mm,% diameter stenosis was 52.4+/-11.6%,65.3+/-9.22% and MLD was 1.32+/-0.24mm, 1.07+/-0.23mm in IVUS and QCA, respectively, which were no statistical significance between these parameters(p>0.05). After intervention, MLD and OA increased significantly(p<0.01) com pared with basal values but there were no significant difference between MLD and OA between 2 measurements(p>0.05). Plaque area measured by IVUS decreased from 9.84 to 7.26mm2 without statistical significance(p>0.05). 3) There was a good correlation in the measurement of the reference segments before intervention but this correlation was much lower after intervention in the reference and stenosis segments between 2 methods(r=0.8723 vs 0.6538, p<0.01). CONCLUSION: IVUS is considered as a sensitive tool in the detection of calcification, eccentricity and dissection and in evaluationg the results of the coronary intervention.

5.Transplantation of Cultured Allogenic Chondrocyte-Collagen Gel Composite into the Articular Cartilage Defect of Rats.

As one of many cell-many cell-based cartilage repairing methods, transplantation of chondrocyte-embedded-collagen gels in cartilage defect was performed for more satisfactory regeneration of cartilage. The authors performed this study to investigate whether the TGF-beta1 treatment of chondrocytes can do some additional synergistic effect on the transplantation of chondrocyte-embedded-collagen gels for crtilage repair. Chondrocytes were isolated from the articular cartilage of newborn Sprague-Dawley rats. Chondrocytes cultured for 10 days in monolayer were embedded in the 0.45% type I collagen gel. Full-thickness cartilage defect was made in the patellar groove of adult Sprague-Dawley rats. Chondrocytes culdefect was made in the patellar groove of adult Sprague-Dawley rats. The cartilage defects were treated with the following methods in a total of 200 animals, which were assigned to 5 different groups of 40 rats. In the control group, the deffect was left without any treatment, in group I, the defect was filled with collagen gel only, in group II, with collagen gel coontaining 10 ng/ml concentration of TGF-beta1, in group III, with collagen gel containing chondrocytes, and in group IV, with collagen gel containing chondrocytes and TGF-beta1. At 1, 2, 4, 8, 12 weeks after the operation, eight rats of each group were sacrificed, and their distal femurs were harvested for the histologic and biomechanical tests. The section s were stained with hematoxilin and eosin. Alcian-blue, and Safranin-O. Regenerated cartilage was analyzed by the semiquantitative histological grading system. Point indentation test was performed as a biomechanical evaluation, and the stiffness was calculated. The results of the histological grading system revealed that the scores gradually increased with time in all groups, and the scores of group III and IV were higher than those of control, group I and II. The biomechanical study showed that the stiffness gradually increased to reach a plateau level in each group. In control, group I and II, the stiffness increased up to the eighth week and remained around the increased level at the twelfth week, and did not show any statistically significant difference between the groups. In group III and IV, the stiffness was higher than in control group, and increased markedly at the fourth week and the increased level was maintained onwards. The results of this study showed that the transplantation of chondrocyte-embedded-collagen gels enhanced the healing process, and the treatment of TGF-beta1 demonstrated at least partially significant improvement.

7.A Case Report of Primary Aldosteronism with Sustained Hypertension After Adrenalectomy.

We experienced a case of primary aldosteronism due to adrenal adenoma. The patient was 45-year old female, whose main complaints were weakness, headache, and fatigue. By use of abdominal CT scan, ultrasonogram, adrenal angiogram and by result of clinical laboratory findings, we conculuded the diagnosis of adrenal adenoma, and confirmed it by surgical and pathologucal findigs. The clinical sympyoms disappeared after left adrenalectomy, and laboratory findings of renin, aldosterone, electrolutes were normalized but hypertension persisted, so she has been managed by antihypertensives.

8.A Case of Well Differentiated Liposarcoma of Spermatic Cord.

Liposarcoma of the spermatic cord is very rare and preoperative diagnosis is difficult. Well differentiated liposarcoma is in general less aggressive than other type of liposarcoma and seldom metastasizes. Radical orchiectomy with wide local excision appear to be adequate treatment modality and offer better prognosis. We report a case of well differentiated liposarcoma in 51-year-old male with brief review of the literatures.

9.Four Cases of Paramedian Thalamopeduncular Artery Infarction.

Cerebral infarction in the territory of the paramedian thalamopeduncular artery (PTA) causes various lesions in the upper midbrain and thalamus resulting in widespread disturbances in neurological function. However, the exact topography and variations in the territories of the PTA remain unknown. We report four patients with MRI-proven infarctions in the PTA territory. One patient had lesions in the unilateral thalamus and midbrain, the other two had lesions in the bilateral paramedian thalamus and unilateral midbrain, and the remaining patient had lesions in the unilateral thalamus and bilateral midbrain. Clinical manifestations depended on the variations of the size and extent of infarctions. Theanatomical variations of the PTA are discussed and suggested.

10.A Case of Widespread Follicular Mucinosis Associated with Mycosis Fungoides.

We presented here a case of a 53-year-old male patient who was confirmed as follicular mucinosis associated with mycosis fungoides. He showed erythematous infiltrative papules and plaques on the face, scalp, trunk and extremities. Some of the lesions were observed as mycosis fungoides. A biopsy specimen showed both findings of follicular mucinosis and mycosis fungoides, such as reticular degeneration of follicles, deposit of mucin, Pautrier's microabscess, epidermotropism, rather dense perifollicular or perivascular infiltrates, and the presence of significant proportions of admixed eosinophils and plasma cells. To our best knowledge, this is the first reported case of follicular mucinosis associated with mycosis fungoides in Korea.
