Seminal vesicle cysts combined with ipsilateral renal agenesis represent a rare urologic anomaly. This urogenital malformation is explained as a developmental failure in the same mesodermal region. We report a case of seminal vesicle cyst, ipsilateral dilated ureter and renal agenesis which was treated by surgical excision. And we searched the literature to review the clinical presentation, diagnosis and treatment options of this anomaly.

2.Urodynamic and Histological Changes in a Sterile Rabbit Vesicoureteral Reflux Model.

This study aimed to investigate pressure changes of renal pelvis and histological change of kidneys in a surgically induced sterile rabbit vesicoureteral reflux (VUR) model. Five rabbits served as a control group, 7 as the sham-operated group, and 8 served as the VUR group. Three weeks later, urodynamic studies were performed, and histological examinations evaluated degree of inflammation, fibrosis, and tubular damage in the kidneys. At a low infusion rate, renal pelvic pressure in the VUR group was stable until late filling phase and then increased slightly. At a high infusion rate, the renal pelvic pressures of the sham-operated and control groups were stable until late filling phase and then increased slightly, whereas the renal pelvic pressure in the VUR group steadily increased from mid filling phase. Focal thinning of the tubular epithelium and interstitial widening were observed in certain cortical areas of refluxing kidneys, without inflammatory cell infiltration. Obvious changes in the mean diameters of distal tubules and extracellular matrix volume fractions were observed in two highly refluxing kidneys. High pressure reflux with bladder instability may result in renal cortical changes.

3.The Value of Prostate Specific Antigen, Digital Rectal Examination, Transrectal Ultrasound, and Transrectal Ultrasound-Guided Biopsy in Prediction of Final Pathologic Diagnosis in Prostate Cancer.

PURPOSE: We evaluated the clinical utility of preoperative serum prostate specific antigen (PSA), digital rectal examination (DRE), transrectal ultrasound (TRUS), and findings of TRUS-guided biopsies in predicting the final pathologic diagnosis in patients who underwent radical prostatectomy. MATERIALS AND METHODS: The medical records of 53 patients with prostate cancer who underwent radical prostatectomy from september 1995 to June 2000 were reviewed. Pathologic variables evaluated on sextant biopsies included total length and percent of cancer on one core, number of cores involved and Gleason score. Clinical variables included PSA and PSA density (PSAD). Also subjects were divided into two groups; organ-confined group versus non organ-confined group. RESULTS: On final pathologic examination, 38 patients (72%) had organ-confined, 11 patients (21%) had margin positive, 6 patients (11%) had capsular penetration and 6 patients (11%) had seminal vesicle involvement. None had pelvic lymph node metastases. Chi-square analysis demonstrated significant correlations between PSA, PSAD, number of cores involved, total length of cancer on one core and organ-confined prostate cancer. When PSA level was 11 or more, PSAD was 0.34 or more, biopsies had Gleason scores of 7 or more, number of cores involved was two or more, and total length of cancer on one core was 0.4cm or more, possibility of cancer being non organ-confined increased CONCLUSIONS: This study demonstrates that PSA, PSAD, Gleason score in sextant biopsy, number of cores involved, total length of cancer on one core are clinically useful predictors of organ-confined disease. This may help both patients and clinician in selecting the most appropriate therapeutic approach.

4.Renal Cell Carcinoma with Deep Vein Thrombosis.

The association between cancer and deep vein thrombosis or pulmonary embolism has been recognized by most clinicians. Adenocarcinoma of the pancreas, female reproductive organs, breast, colon, lung, and prostate have been found to be associated with the highest risk of deep vein thrombosis. However, the isolated deep vein thrombosis is a rare phenomenon in renal cell carcinoma. Only two cases of deep vein thrombosis in renal cell carcinoma were reported in the literature. The tumor thrombi of renal cell carcinoma is found in renal vein and inferior vena cava, with the incidence of 20-49% and 5-10%, respectively. We report two cases of renal cell carcinoma which are associated with the deep vein thrombosis due to the obstruction of inferior vena cava by the tumor thrombi.

5.Histologic Alterations in the Ipsilateral and Contralateral Testes and Epididymides of Rats following Unilateral Torsion and Detorsion of the Testes.

PURPOSE: This investigation was undertaken to determine the damage to the testes and epididymides following torsion and detorsion of the testes. MATERIALS AND METHODS: The right testes of 8-week-old male rats(n=30) were subjected to torsion for 10 min. At 0, 1, 4, 8, and 24 hours, and 1 week after the repair of a torsion, the ipsilateral and contralateral testes and epididymides were harvested. The mean number of spermatids per tubule, the mean seminiferous tubular diameter(MSTD), and the germinal epithelial cell thickness(GECT) were used to evaluate changes to the testes. The histologic changes to the epididymal ductal epithelium were also evaluated. RESULTS: The mean number of spermatids per tubule, GECT, and MSTD were significantly decreased in the 24-hour ipsilateral detorsion group, but minimal changes to ipsilateral testes were observed in the 1-week detorsion group. There was no evidence of histologic changes to the testes in any of the contralateral detorsion groups. The interstitial fibroblast proliferation and hemorrhage of the ipsilateral epididymis were found in the 4-hour detorsion group and increased in the 8-hour detorsion group. Interstitial fibroblast proliferation was prominent in the ipsilateral epididymis of the 24-hour detorsion group, but was only occasionally observed in the contralateral epididymides. Shortening of the tubular epithelial cell height and tubule dilatation were observed in the ipsilateral and contralateral epididymis 1 week after detorsion. CONCLUSIONS: Torsion/detorsion damage was found earlier and at a higher intensity in the epididymides than in the testes. This finding may be due to the protection afforded by the blood-testis barrier.

PURPOSE: To evaluate the efficacy and safety of an intracavernous injection of lyophilized papaverine/phentolamine/alprostadil (Standro(R)) for the treatment of erectile dysfunction (ED) in Koreans. MATERIALS AND METHODS: 249 men (>20 years old), with ED (>6 month duration), were enrolled from 14 clinical centers. The intracavernous 'TMs' were titrated in a stepwise fashion at the clinic, from 0.05-0.25ml (17.64mg papaverine, 0.6mg phentolamine, and 6mug alprostadil per ml), with increment of 0.02-0.05ml, according to the etiology and severity of the ED and the patients' ages. RESULTS: Of the 249 men, 238 completed the dose titration, and progressed to home treatment of 3 months duration. Of these 238, 193 (psychogenic 13.0%, organic 75.5%, mixed 11.5%) completed the home treatment (4 or more self-injections), with the other 45 dropping out (lost to follow-up in 24, patient refusal in 9, no chance to have intercourse in 7 and omitted recording of patient diary in 2). The success rate per trial (a total number of sufficient erection for vaginal intromission/a total number of injections) and per patient (number of patients who had one or more sufficient erections for vaginal intromission/the enrolled patients at beginning or 193 patients), and the satisfaction rate per patient (number of patients who had both patient and partner satisfaction with erection/193 patients) were 74.1, or 91.2 and 75.1%, respectively. The adverse reactions were prolonged erections in 3, urethral pain in 1 and penile skin edema in 2. Three patients complained of penile pain during an erection, but there was no dropout due to the pain. No significant changes in laboratory tests were found after the home treatment. CONCLUSIONS: A 'TM' seems to be effective and safe for an intracavernous injection for the treatment of men with erectile dysfunction.
