1.A Clinical Observation on Renal Tuberculosis.

A clinical observation was made on 46 cases of renal tuberculosis who were admitted to the Department of Urology, Eul Ji General Hospital, Seoul, Korea, during the period from January 1978 to December 1982. Following results were obtained. 1. The most common symptoms were urinary frequency (47.8%), gross hematuria (43.5%) flank pain (32.6%) and dysuria (21.7%). 2. Tubercle bacilli were demonstrated in 37% of patients by Ziehl-Neelson staining. The most common findings of urine were hematuria (73.9%), Pyuria (54.4%) and proteinuria (32.6%). 3. Excretory urographic findings in 46 cases of renal tuberculosis were non visualization (45.7%), calyectasis (28.3%), delayed visualization (15.2%), ureteral deformity (10.9%). 4. The most common finding of cystoscopy was hemorrhagic patches (44.4%), followed by sealed off ureteral orifice (s) in 30.6%, ulceration and tubercle formation in 25.0%. 5. Patients were managed by chemotherapy only in 47.8%, chemotherapy with nephrectomy in 43.5%, chemotherapy with reconstructive surgery in 8.7%.

2.Expressions of the Tumor Associated Proteins and Their Correlation with the Pathologic Features in Childhood Hepatoblastoma.

Hepatoblastoma is a rare malignant liver tumor found in children. Its biological characteristics and prognostic factors have not been well known. We investigated 29 cases of hepatoblastoma, registered in university hospitals in Seoul from 1984 to 1996. By the immunohistochemical method, p53, Waf-1 (p21), bcl-2, heat shock protein 70 (hsp70), c-jun, transforming growth factor-alpha (TGF-alpha) expressions were studied. Those data were compared with clinico-pathologic features; age, sex, tumor size, tumor stage and histologic subtypes. Expression of p53 and bcl-2 were each observed separately in single cases. Expression of c-jun was more frequently noted in patients at higher stages. Expression of TGF-alpha decreased in the order of pure fetal, mixed, embryonal and small cell anaplastic subtypes. Cumulative survival rate was lower in females than in males and in patients with a higher tumor stage. According to histologic subtypes, survival rates decreased in the order of pure fetal, mixed, embryonal and small cell anaplastic subtypes. Survival rate was lower in patients with c-jun expression. Group of TGF-alpha labelling index under 19 showed a lower survival rate than that over 19. In conclusion, we found that tumor associated proteins, c-jun and TGF-alpha, are closely related to the prognosis of hepatoblastoma but p53 and bcl-2 may not be related to it.

3.Three-dimensional morphological evaluation of the hard palate in Korean adults with mild-to-moderate obstructive sleep apnea.

OBJECTIVE: The purpose of this study was to evaluate differences in three-dimensional (3D) morphology of the hard palate between Korean adults with and without mild-to-moderate obstructive sleep apnea (OSA) using cone-beam computed tomographic (CBCT) data. METHODS: The protocol for the two-dimensional (2D) and 3D mathematical modeling was established by analyzing CBCT images of 30 adults with OSA and 30 matched controls without OSA, using MIMICS software. The linear and angular measurements were also determined using this software. The measurements were repeated for 30 palates, by the same operator, to assess reliability. RESULTS: The palates of OSA patients were higher in the posterior part and narrower in the anterior-superior part than those of the control group (p < 0.05). The nasal cavities of patients with OSA were narrower (p < 0.05) than those of controls. The increasing angle of the first molar palatal root is a compensation of the upper dental arch to improve occlusion. However, for most palatal measurements, there were no significant differences between the OSA and control groups (p > 0.05). The results of 2D and 3D mathematical models were consistent for linear and angular measurements, indicating that 2D and 3D mathematical modeling of the palate is a reliable methodology. CONCLUSIONS: OSA is a multifactorial disease; the palates of adults with mild-to-moderate OSA do not have specific morphological features distinct from those of healthy controls.

4.Carcinoma of the Prostate Gland Accompanied by Severe Chronic Anemia.

We report a case of Undifferentiated carcinoma of the prostate in a 66 years old male resulted in severe chronic anemia (Hb value: 2.9gm/100dl, Hct:9%). The patient was treated with multiple blood transfusions, diethylstilbestrol and cystostomy with good improvement of general condition.

5.Three Cases of Horseshoe Kidneys with Complications..

The horseshoe kidney is the most common of all renal fusion anomalies 3 cases of horseshoe kidneys with complications are reported. 2 cases of horseshoe kidneys has had unilateral giant hydronephrosis as complications and undergone unilateral nephrectomy with division of isthmus and another one has had unilateral renal stone with no hydronephrosis, which is under observation.

6.Clinical Effects of Recombinant Human G-CSF (Neutrogin) on Neutropenia Induced by the Cancer Chemotherapy for Gynecologic Malignancy.

This study is undertaken to investigate the clinical effecte of Neutrogin(recornbinant human granulocyte-colony stimulatiing factor) in 25 patients with ovarian caneer who received two courses of CAP chemotherapy. In the first courae of chemotherapy as a contml course, all patients were treated with CAP chemotherapy alone and during the second course, Neutrogin was given at a dose of 2ug/kg/day subcutaneouely for 14 days 24 hours after completion of chemotherapy. Neutrogin significantly increaaed the nadir count of leukocyte and abaolute neutrophil and decreased tbe number of days per patient on which the absolute neutrophil count was 1,000/mm3 or less. The mean recovery time required for neutrophil to greater than l,500/mm3 after nadir were significantliy decreased in the seeond comse as compared with the first course. Each one patient exriened mild side effects of Neatrogin, such as fever, tene pain and malaise and rernovered without tretment. These results showed that Neutrogin is extremely effective and useful to treat chemotherapy indueed leukopenia and to accelerate the recovery from these complications.

8.A Study on the Clinical Features and the Predictors of Febrile Seizure Plus.

PURPOSE: We intended to investigate the clinical features and the predictors of febrile seizure plus(FS+), not suspected of generalized epilepsy with febrile seizure plus (GEFS+). METHODS: We selected 24 patients, aged more than 6 years old, who were admitted to Myongji Hospital of Kwandong University from January, 2001 to March, 2005 due to febrile seizures. All of those had neither a family history of febrile seizures or epilepsy nor abnormalities in the EEG or MRI. We examined sex, age of seizure onset, type of seizure, duration of seizure, the time until seizure after fever and frequency of febrile seizure. We investigated also the statistical significances of these clinical features by comparing with patients of simple febrile seizure(FS) as a control group. RESULTS: The male to female of the FS+ group was 2:1, which had no statistical significance compared with the FS. The age of seizure onsets in the FS+(39.4+/-5.0 months) was significantly older than FS the group(24.1+/-2.1 months)(P<0.05). In the types of seizures, non-generalized tonic clonic seizures occurred significantly more often in the FS+ group(20.9%) than the FS(8.4%)(P<0.05). The frequency of febrile seizures also was high in the FS+ group(n=3.2+/-0.8) than the FS group(n=1.1+/-0.1)(P<0.05). The duration of seizures and the time until seizure after fever did not have statistical significance. CONCLUSION: We suggest that FS+ can be characterized by late onsets of initial seizures, more frequent non-generalized tonic clonic seizures and recurrent febrile seizures. Therefore, FS+ should be suspected for patients more than 4 years old with non-generalized tonic clonic seizures or recurrent febrile seizures of more than 4 times.

9.A new type of clear orthodontic retainer incorporating multi-layer hybrid materials.

Clear thermoplastic retainers have been widely used in daily orthodontics; however, they have inherent limitations associated with thermoplastic polymer materials such as dimensional instability, low strength, and poor wear resistance. To solve these problems, we developed a new type of clear orthodontic retainer that incorporates multi-layer hybrid materials. It consists of three layers; an outer polyethylenterephthalate glycol modified (PETG) hard-type polymer, a middle thermoplastic polyurethane (TPU) soft-type polymer, and an inner reinforced resin core. The resin core improves wear resistance and mechanical strength, which prevent unwanted distortion of the bucco-palatal wall of the retainer. The TPU layer absorbs impact and the PETG layer has good formability, optical qualities, fatigue resistance, and dimensional stability, which contributes to increased support from the mandibular dentition, and helps maintain the archform. This new type of vacuum-formed retainer showed improved mechanical strength and rate of water absorption.

10.Three cases of Testicular Tumors.

We have experienced 3 cases of testicular tumors since last 2 years, from June, 1981 to May, 1983. two cases were embryonal Ca. occurred in 22 years old male and 2 1/2 years old male respectively and another one was seminoma case occurred in 33 years old male. All of them were undergone inguinal orchiectomy and the seminoma case has been taken anti-tumor chemotherapy with radiation therapy and still alive. The adult embryonal Ca. case expired 3 months after operation and the child embryonal Ca. case is alive under observation.
