1.A Case of Congenital Smooth Muscle Hamartoma.

Congenital smooth musele hamartomas appear at birth as hypertrichotic patches or plaques with or without hyperpigmentation. Histologic characteristic is hyperplasia of dermal smooth muscle bundles. We report a case of congenital smooth muscle harnartoma in 5-year-old female, who showed localized excessive-hairy, skin-colored patch on the lateral surface of left. elbow, which were found at birth. Biopsy specimen showed hyperpigmentation of the basal layers of epidermis, melanophages in upper dermis, and hyperplasia of smooth muscle bundles in lower dermis.

2.Systolic Time Intervals in Korean Diabetics.

Recently, the proportion of cardiovascular disease as a cause of death in diabetics is in ever increasing tendency. Present study is designed to evaluate the change of myocardial performance by the effect of microangiopathy of diabetics. The measurements of systolic time intervals were obtained from simultaneous high speed recordings(100mm/sec) of electrocardiography, phonocardiography, and carotid pulse tracing in diabetics(male 26, female 31). 1. As the diabetic retinopathy got severe, shortening of LVET and prologation of PEP were noted, but QS2 showed no significant change. 2. PEP/LVET ratio fo diabetics without diabetic retinopathy was significantly higher than that of normal control(p<0.01). 3. PEP/LVET ratio of diabetics with diastolic blood pressure under 95mmHg was significantly higher than that of normal control group(p<0.01), and PEP/LVET ratio of diabetics with diastolic blood pressure over 96mmHg was significantly higher than that of normal control and that of diabetics with diastolic blood pressure under 95mmHg(p<0.01). 4. PEP/LVET ratio of diabetics with proliperative retinopathy was significantly higher than that of normal control and that of diabetics with mild diabetic retinopathy(p<0.01), but this result was not conclusive because of the small case number.

3.A Case of Malignant Atrophic Papulosis.

A 15-year-old female patient had malignant atrophic papulosis(Degos' disease) that affected the skin and probably intestinal tract. She had multiple erythematous papules with porcelain-white atrophic centers on the trunk and extremities. Also she suffered from intermittent nausea, vomiting and abdominal pain. She received appendectomy for abdominal pain 2 months ago. Histopathologic studies showed atrophic epidermis and necrobiotic dermis. There were endothelial thickening, fibrinoid necrosis of bload vessels, perivascular lymphocytic infiltation in the lower dermis. She was treatd with acetyl salicylic acid and dipyridamole, but there was no improvement.

4.A case of tuberculosis verrucosa cutis.

We report a case of tuberculosis verrucosa cutis in a 34-year-old male patient who presented with asymptomatic verrucous plaque over the right buttock, right inner thigh and right inguinal area. The Mantoux test revealed a strong positive reaction. The histologic picture consists of hyperkeratosis, acanthosis and papillomatosis overlying an inflammatory infiltrate in the upper dermis. Tuberculoid granulomas without caseation necrosis are seen in the dermis. Tubercle hacilli were not demonstrated. The patient was treated with INAH, Rifampicin and Ethambutol for 6 months. To date, the skin lesions have markedly improved.

5.Sleep Problems in Autism Spectrum Disorder.

Autism Spectrum Disorder (ASD) is characterized by persistent deficits in social communication and restricted, repetitive patterns of behavior and interest. Sleep problems are not uncommon in children with autism spectrum disorders. Symptoms of insomnia are the most frequent sleep problems in individuals with ASD. Sleep problems can cause significant difficulties in the daily life of children with ASD and their families. Genetic factor, deregulations of melatonin synthesis, extraneous environmental stimuli and psychiatric and medical conditions may cause sleep problems. The first line treatment of sleep problems in ASD includes managements for potential contributing factors and parent education about sleep hygiene care for child and behavioral therapy. Supplementation with melatonin may be effective before considering other medications, such as risperidone, clonidine, and mirtazapine.

7.Surface Replacement Arthroplasty of the Hip

The concept of surface replacement arthroplasty of the hip is to replace the diseased joint surface with artificial plastic and metal cup in the acetabular and femoral head respectively, and to restore the normal anatomy and gain biomechanical function. Ten cases of Wagner type's surface replacement arthroplasty performed in Department of Orthopedic Surgery, Seoul National University Hospital since July 1979 are reviewed, and their minimum follow up is two months. In terms of the Pain control and improvement of range of motion in the hip, surface replacement arthroplasty showed about the same result compared with conventional total hip. In our cases there was no one complicatian, even though it is too short to evaluate the result of this operation. Wagner recommends surface replacement arthroplasty to perform through anterior approach, but 2 cases of our ten patients that have relatively good range of motion of the hip could be operated on this surgery by anterolateral approach (oblique skin incision) without trochanteric osteotomy, because of high incedence of myositis ossification with anterior approach. It is our conclusion that surface replacement arthroplasty of the hip may be used as an interim procedure to gain time in young patient who needs conventional total hip replacement which reveals increasing complication with the lapse of time.

10.Three Cases of Eosinophilic Pustular Folliculitis Without Eosinophilia.

We present three cases of eosinophilic pustular folliculitis without eosinophilia in a 19-year-old female, a 27-year-old male, and a 51-year-old male. Multiple erythematous plaques with papules and pustules were observed on the face in two patients and on the face, trunk, and extremities in the third patient. Histopathologic findings showed hair follicles and the sebaceous glands infiltrated with mixed inflammatory cells composed of eosinophils and neutrophils, characteristic of eosinophilic pustular follicultitis. However, peripheral blood eosinophilia was not present in any of our three cases.
