2.The Effect of Estradiol on the Osteoporosis Induced by Oophorectomy in the Rat

Post-menopausal osteoporosis is known to be related to estrogen deficiency. The purpose of this experimental study is to investigate whether or not estrogen administration can prevent or retard the process of osteoporosis induced by oophorectomy in rats. Fifty-two Sprague-Dawley rats, aged five months, were separated into three groups. Group 1 (4 rats) was control group that had sham operation without oophorectomy. Goup 2 (32 rats) was another control group that had bilateral oophorectomy alone. Group 3 (16 rats) was experimental group that had estradiol administration from the fourth week through the eighth week after bilateral oophorectomy. In group 1, all the rats were killed at the fourth week after sham operation. In group 2 and group 3, four rats in each group were killed weekly until the eighth week after oophorectomy. All the animals were subjected to radiological, histological studies on the tibia, ilium, skull, and the sixth lumbar vertebra. Also, histomorphometric measurements were 1. Osteoporotic changes were evident from the fifth week after oophorectomy and increased with time on the radiologic examination in group 2. In contrast, osteoporotic changes were evident from the second week after oophorectomy and progressed with time on the histological and histomorphometric studies in group 2. 2. After administration of estradiol, osteoporotic changes which include thinning of the cortex, decrease of the amount of trabeculae were partially restored on the radiological, histological studies in group 3. 3. Histomorphometric study revealed that the trabecular area percent (the proportion of total area of the trabeculae from the whole metaphyseal area), decreased progressively through the eight week after oophorectomy in gorup 2. Whereas the trabecular area percents of group 3 were higher than those of group 2, but still lower than initial value before oophorectomy. Based on the above observations, estradiol administration appeared to delay the progression of the osteoporotic changes induced by oophorectomy in rats.

3.Pleomorphic Xanthoastrocytoma: A Case Report.

Pleomorphic xanthoastrocytoma is histologically characterized by marked cellular pleomorphism of lipid-laden neoplastic astrocytes and bizarre giant cells showing mitotic figures and high cellularity. Inspite of its ominous-looking microscopic features, howerver, the prognosis is usually favorable. This tumor develops mainly in the supratentorial area of young people and frequently involves the leptomeninges. We experienced a case of pleomorphic xanthoastrocytoma in 18 year-old-male. In addition to the cellular pleomophism, the prominent reticulin fibers surround the individual tumor cells or the tumor cells nests. Immunohistochemical staining and electron microscopy revealed glial fibrillary acidic protein(GFAP) expression and pericytoplasmic basal lamina in the tumor cells.
4.Quantitation of C-reactive Protein Levels and Erythrocyte Sedimentation Rate after Spinal Surgery.

OBJECTIVES: In order to standardize the CRP, ESR and WBC count after three types of spinal surgery and get clinical usefulness from these laboratory dadas after spinal surgery and clincal usefulness of laboratory datas. MATERIAL AND METHODS: Three groups of patients(microdisectomy, anterior and posterior fusion) were chosen for the study. CRP, ESR and WBC levels were prospectively determined before surgery and at days 1-5, 10, 14,21 and 42 after surgery. RESULTS: In all patients, preoperative normal CRP level(<10mg/L) increased, reaching peak levels on the second day after anterior fusion(84.6mg/L), and at the third day after microdiscectomy(54.5mg/L) and posterolateral intercorporal fusion(152.2mg/L), with normalization in 5-10 days. Preoperative normal ESR level increased to peak level on the forth day after microdiscectomy(33.0mm/hr), and increased double peak levels after anterior(postoperative days 2&5) and posterolateral intercorporal fusion(postoperative days 3&5). ESR followed by a slow and irregular decrease, and at 42 days after surgery often remained elevated. WBC increased to variable peak level within normal limit in all patients. CONCLUSIONS: The rapid decline in CRP will probably be interrupted by a second rise or persisting elevation if infection occurs. CRP is presumably a better test than ESR for early detection of postoperative infection.

6.Preventing Lateral Skin Numbness after Medial Unicompartmental Knee Arthroplasty.

BACKGROUND: The authors report the results of preserving the infrapatellar branch of the saphenous nerve during unicompartmental knee arthroplasty to prevent lateral skin numbness. METHODS: All 100 cases had medial compartmental osteoarthritis and a minimally invasive technique had been used. The mean follow-up duration was two years and eight months (range, 24 to 42 months). RESULTS: The classification according to the location of this nerve was observed as either Mochida Type I with 76 cases (76%), Type II with 16 cases (16%), and unclassified type with 8 cases (8%). In Type I, the nerve was saved in 62 cases (82%), but could not be preserved in Type II because of the surgical procedure. These results showed that the mean distance from the joint line to the nerve of Type I was 9.13 mm (range, 4 to 15 mm) and the nerve passed inferiorly. CONCLUSIONS: This study showed the location of this nerve can be predicted ahead of the procedure, which will help preserve it during the surgery.

Intraneural ganglion of the tibial nerve is very rare. We have experienced a case of the intraneural ganglion cyst in the tibial nerve and medial plantar nerve, which was located at the level of the ankle and foot. A 29-year-old male suffered from lumps causing numbness around the medial aspect of the sole and great toe. But there was no weakness and change of the motor function. We excised the cystic mass successfully and the neurologic symptoms disappeared.

8.Transplantation of Cultured Allogenic Chondrocyte-Collagen Gel Composite into the Articular Cartilage Defect of Rats.

As one of many cell-many cell-based cartilage repairing methods, transplantation of chondrocyte-embedded-collagen gels in cartilage defect was performed for more satisfactory regeneration of cartilage. The authors performed this study to investigate whether the TGF-beta1 treatment of chondrocytes can do some additional synergistic effect on the transplantation of chondrocyte-embedded-collagen gels for crtilage repair. Chondrocytes were isolated from the articular cartilage of newborn Sprague-Dawley rats. Chondrocytes cultured for 10 days in monolayer were embedded in the 0.45% type I collagen gel. Full-thickness cartilage defect was made in the patellar groove of adult Sprague-Dawley rats. Chondrocytes culdefect was made in the patellar groove of adult Sprague-Dawley rats. The cartilage defects were treated with the following methods in a total of 200 animals, which were assigned to 5 different groups of 40 rats. In the control group, the deffect was left without any treatment, in group I, the defect was filled with collagen gel only, in group II, with collagen gel coontaining 10 ng/ml concentration of TGF-beta1, in group III, with collagen gel containing chondrocytes, and in group IV, with collagen gel containing chondrocytes and TGF-beta1. At 1, 2, 4, 8, 12 weeks after the operation, eight rats of each group were sacrificed, and their distal femurs were harvested for the histologic and biomechanical tests. The section s were stained with hematoxilin and eosin. Alcian-blue, and Safranin-O. Regenerated cartilage was analyzed by the semiquantitative histological grading system. Point indentation test was performed as a biomechanical evaluation, and the stiffness was calculated. The results of the histological grading system revealed that the scores gradually increased with time in all groups, and the scores of group III and IV were higher than those of control, group I and II. The biomechanical study showed that the stiffness gradually increased to reach a plateau level in each group. In control, group I and II, the stiffness increased up to the eighth week and remained around the increased level at the twelfth week, and did not show any statistically significant difference between the groups. In group III and IV, the stiffness was higher than in control group, and increased markedly at the fourth week and the increased level was maintained onwards. The results of this study showed that the transplantation of chondrocyte-embedded-collagen gels enhanced the healing process, and the treatment of TGF-beta1 demonstrated at least partially significant improvement.

9.Detection of Human Papillomavirus DNA by In Situ Hybridization Using Biotinylated DNA Probes in Codylomata Acuminata.

Human Papillomaviruses(HPVs) are regarded as sexually transmitable and closely associated with precancerous lesions or invasive cancers of the vulva, vagina, and cer V1X. In situ hybridization, besides showing higher sensitivity, allowed the identification of specific viral types. We studied 12 cases of condylomata accuminata for HPV typing by in situ hybridization techenique. Ten cases(83.3 % ) showed positive for HPV type 6/11, and 2 cases(16.7% ) were positive for HPV type 16/18.

10.Development of 99mTc-Transferrin as an Imaging Agent of Infectious Foci.

PURPOSE: Purpose of this study is to synthesize 99mTc-labeled transferrin for infection imaging and to compare it with 67Ga-citrate for the detection of infectious foci. MATERIALS AND METHODS: Succinimidyl 6-hydrazino-nicotinate hydrochloride-chitosan-transferrin (Transferrin) was synthesized and radiolabeled with 99mTc. Labeling efficiencies of 99mTc-Transferrin were determined at 10 min, 30 min, 1 hr, 2 hr, 4 hr and 8 hr. Biodistribution and imaging studies with 99mTc-Transferrin and 67Ga-citrate were performed in a rat abscess model induced with approximately 2 x 10(8) colony forming unit of Staphylococcus aureus ATCC 25923. RESULTS: Successful synthesis of Transferrin was confirmed by mass spectrometry. Labeling efficiency of 99mTc-Transferrin was 96.2+/-0.7%, 96.4+/-0.5%, 96.6+/-1.0%, 96.9+/-0.5%, 97.0+/-0.7% and 95.5+/-0.7% at 10 min, 30 min, 1 hr, 2 hr, 4 hr and 8 hr, respectively. The injected dose per tissue gram of 99mTc-Transferrin was 0.18+/-0.01 and 0.18+/-0.01 in the lesion and 0.05+/-0.01 and 0.04+/-0.01 in the normal muscle, and lesion-to-normal muscle uptake ratio was 3.7+/-0.6 and 4.7+/-0.4 at 30 min and 3 hr, respectively. On image, lesion-to-background ratio of 99mTc-Transferrin was 2.18+/-0.03, 2.56+/-0.11, 3.08+/-0.18, 3.77+/-0.17, 4.70+/-0.45 and 5.59+/-0.40 at 10 min, 30 min, 1 hr, 2 hr, 4 hr and 10 hr and those of 67Ga-citrate was 3.06+/-0.84, 4.12+/-0.54 and 4.55+/-0.74 at 2 hr, 24 hr and 48 hr, respectively. CONCLUSION: Transferrin is successfully labeled with 99mTc, and its labeling efficiency was higher than 95% and stable for 8 hours. 99mTc-Transferrin scintigraphy showed higher image quality in shorter time compared to 67Ga-citrate image. 99mTc-transferrin is supposed to be useful in the detection of the infectious foci.
