3.Cardiovascular Diseases in Korea.

Statistical observation was made on the 1949 patients with cardiovascular diseases who had been treated in Dept. of Internal Medicine, Seoul national University Hospital during the period of four years, from jan. 1972 to Dec. 1975. 1) The patients wth cardiovascular diseases accounted for 20.2% out of total 9,638 medical inpatients. The incidence for the consecutive years was not apparently variable. 2) The diseases occured most frequently in the sixth decades and the incidence decreased in the order of fifth and seventh decades. 3) The disease was more prevalent in male than female with ratio of 1.3 of 1. 4) The hypertension was the most common disease among cardiovascular diseases, accounting for 67.5% of patients with cardiovascular diseases and 13.6% of total medical inpatients. The disease occurred most frequently in the sixth decades and the incidence decreased in the order of fifth and seventh decades with male preponderance. 81.8% of patients with hypertension was essential hypertension and 18.2% was secondary hypertension. Secondary hypertension was caused by parenchylmal renal diseases in most patients and there were only several patients with renovascular and endocrine hypertension. 40.2% of patients with essential hypertension had no complications, 38.6% had cerebral vascular accidents, 12.1% had uremia and 9.1% had congestive heart failure. The patients with malignant hypertension was 9.6% of total patients with hypertension. 5) Rheumatic heart disease was 14% of patients with cardiovascular diseases, being second in incidence. It was more prevalent in female than male with ratio of 1 male to 1.3 female and affected the mitral valve in most of the cases, whereas involvement of aortic valve was rare. 6) The coronary heart disease was 5.3% of patients with cardiovascular diseases, being third in incidence. It occured most frequently in seventh and sixth decades and affected male two times more frquently than female. 7) The congenital heart disease was 2.9% of patients with cardiovascular diseases, of which atrial septal defect was highest in incidence. 8) The cor-pulmonale was 1.4%, pericarditis 1.4%, endocarditis 0.9%, postpartum heart disease 1.2%, myxedematous heart disease 0.2%, arrhythmia 2.1%, peripheral vascular disease 1.1% and miscellaneous 0.6% of patients with cardiovascular diseases. 9) Postpartum heart failure and endocarditis was remarkably decreased in incidence recently and there were one patients with beriberi heart disease and one patient with syphilitic heart disease during those years of this study.

5.Acropigmentation Symmetrica of Dohi Treated with the Q-switched Alexandrite Laser.

A few cases of reticulate acropigmentary disorders have been reported in Korea. Most of them were reticulate acropigmentation of Kitamura, but not acropigmentation symmetrica of Dohi. A 21 year-old male patient diagnosed as acropigmentation symmetrica of Dohi had been previously treated with various treatment methods unsuccessfully. Re-treatment with the Q-switched alexandrite laser(755 nm, 100nsec) showed a temporary improvement but a relapse of the lesion was experienced again. We describe a case of acropigmentation symmetrica of hi treated with the Q-switched alexandrite laser.

6.The Effect of Estradiol on the Osteoporosis Induced by Oophorectomy in the Rat

Post-menopausal osteoporosis is known to be related to estrogen deficiency. The purpose of this experimental study is to investigate whether or not estrogen administration can prevent or retard the process of osteoporosis induced by oophorectomy in rats. Fifty-two Sprague-Dawley rats, aged five months, were separated into three groups. Group 1 (4 rats) was control group that had sham operation without oophorectomy. Goup 2 (32 rats) was another control group that had bilateral oophorectomy alone. Group 3 (16 rats) was experimental group that had estradiol administration from the fourth week through the eighth week after bilateral oophorectomy. In group 1, all the rats were killed at the fourth week after sham operation. In group 2 and group 3, four rats in each group were killed weekly until the eighth week after oophorectomy. All the animals were subjected to radiological, histological studies on the tibia, ilium, skull, and the sixth lumbar vertebra. Also, histomorphometric measurements were 1. Osteoporotic changes were evident from the fifth week after oophorectomy and increased with time on the radiologic examination in group 2. In contrast, osteoporotic changes were evident from the second week after oophorectomy and progressed with time on the histological and histomorphometric studies in group 2. 2. After administration of estradiol, osteoporotic changes which include thinning of the cortex, decrease of the amount of trabeculae were partially restored on the radiological, histological studies in group 3. 3. Histomorphometric study revealed that the trabecular area percent (the proportion of total area of the trabeculae from the whole metaphyseal area), decreased progressively through the eight week after oophorectomy in gorup 2. Whereas the trabecular area percents of group 3 were higher than those of group 2, but still lower than initial value before oophorectomy. Based on the above observations, estradiol administration appeared to delay the progression of the osteoporotic changes induced by oophorectomy in rats.

7.A case of porokeratosis induced by topical PUVA in a vitiligo patient.

A 20-year-old female, who had been treated with topical PUVA therapy for her vitiligo, developed characteristic skin lesions of disseminated superficial artiric porokeratosis(DSAP). All of her three sisters had vitiligo and had been treated with topica PUVA, hut only one developed DSAP lesion. Her mother had had DSAP lesions on expcsec areas for 20 years. DSAP shows an autosomal dominant mode of inheritance, and is charaterized by multiple ciark brownish keratotic papaules surrounded by slightly elevatied order. The exacerbation of the lesion by exposure t.o UV lignt indicates that the regulatingene for DSAP is related to sunlight. 1-lerein we reported a case of DSAP induced by topica] PUVA therapy in one patient during management of familial vitiligo in 3 sisters.

8.Clinical Study of Conservative Management of the Acute Epidural Hematoma.

The authors present 31 patient with acute epidural hematoma who had been admitted from January, 1987 to June. 1989 managed by consevative treatment. The results were as follows ; 1) The age of patients was distributed evenly, most patients were male (26 cases) and the most common mechanism of injury was traffic accidents(14 cases). 2) Sites of hematoma were mostly the temporal and parietal area and the maximum thickness of hematoma was not different from each site. 3) On admission, all patients presented GCS score above 13 except 2 cases. 4) All patients did not undergo delayed up, due to the enlargement of the hematoma or neurological deterioration. 5) The maximum thickness of hematoma was under 20 mm in all patients. 6) The hematoma was resolved within 30 days in all cases. This was confirmed with brain CT scan.

9.A case of amyopathic dermatomyositis.

A 50-year-old female, who had been suffered from diabetes rielitus, showed typical cutaneous findinhs of dermatomyositis with only transient and mild proximal muscle weakness at sometime during the course of disease. She had Gottrons papules, Gottrons sign, heliotrope rash, macular violsceous erythema involving neck, anterior chest, back, extensor site of forearms and arms and dorsa of hands. However, he serum CPK, LDH and sGOP levels were within normal limits. Her fasting and 2 hours potprandial blood glucose levels were elevated. There were no histological abnormality in muscle biopsy. Histopathologic findigs of the skin lesion on her back were that of poikiloderma atrophicans vasculare. We diagnosed our patient as Rebeccas type 2 amyopathic dematomyositis which is chracterized by skin disease with subjective myalgias and musle weakness, but without laboratory evidence of muscle disease.
