2.Appendiceal Mucocele with Lower Gastrointestinal Bleeding.

The appendiceal mucocele is very rare disease of 0.2% incidence. About 24% of patients are asymptomatic and symptomatic patients present with pain in the right lower quadrant of abdomen in 64%, plapable maas in the right lower quadrant of abdomen in 50%, and rarely, melena, hematochezia, anemia, diarrhea, malaise, and abdominal distension. The gastrointestinal bleeding may be presented in the patient with intussusception, but the massive bleeding is generally absent. We report a case of appendiceal mucocele accompanying with gastrointestinal bleeding and review of literature.

3.Concurrent Malignant Lymphoma of the Colon and Small Bowel as the Primary Origin.

Primary gastrointestinal lymphoma primarily confined to gastrointestinal tract is relatively rare diaease. Although the lymphoma primarily involved colorectum or small bowel was commonly reported than primary gastric lymphoma in Korea, the concurrent primary lymphoma of colon and small bowel is very rare in reported cases. We report a case of combined primary malignant lymphoma of the jejunum and cecum, who was admitted due to melena and anemia. She had right hemicolectomy and end-to-side ileotransverse colostomy at 2 months ago, due to primary colon lymphoma. The laparotomy was performed and identified hard, 2 x 3 cm sized masses on distal 25 cm, 80 cm and 150 cm from the Treitz ligament. The histology of these small ma showed histiocytic lymphoma as same as cecal specimen. In general, the small bowel lymphoma shaws poor prognosis than gastric or colorectal lymphoma due to diffieulty in diagnosis and late symptoms, but this problem could be resolved through the knowledge about primary gastrointestinal lymphoma and the development of diagnostic methods.

4.A Case of Leiomyosarcoma in Stomach.

Leiomyosarcomas of the stomach are unusual tumors, accounting for 1% to 3% of all malignant tumors and 17% to 20% of all smooth muscle tumors of the stomach. It is difficult to diagnosis these tumors preoperatively because of its vague symptoms and signs or producing symptoms only late. Despite their rarity and difficulty in diagnosis, thier diagnosis is important due to more favorable prognosis aasociated with these tumors than that of gastric carcinoma. Recently, endoscopy is being used with increasing frequency, but because these are submucosal lesions the diagnosis of thease tumors cannot be easily made through the endoscope. We experienced a case of relatively full-brown leiomyosarcoma in gastric fundus. Endoscopy showed 6x6cm sized, definite margined, protruding mass on gastric fundus, with central deep ulceration coated with blood clots. The mucosa overlying the mass was intact, smoothly effaced and the bridging fold was also visible. Postoperative ultrasound showed large protruding mixed echogenic solid mass with contral deep excavation due to ulcer, and intact serosa, so suggestive of endogastric tumor. We believed endoscopy combined with endoscopic ultrasound is probably useful method in early diagnosis of gastric submucosal tumors, especially exogastric type or early small tumors

A case of persistent Cushing's syndrome in a 25 year-old male is presented. Inspite of bilateral total adrenalectomy twice during two years, there was no clinical improvement. Adrenal scanning revealed no adrenal remnants. ACTH secreting pituitary microadenoma is demonstrated by computed tomographic brain scan and hormonal studies. Selective transsphenoidal microadenomectomy results in clinical and endocrinological improvement.

7.Time Course of the Suppression Effect on Transient Evoked Otoacoustic Emissions by Prolonged Contralateral Acoustic Stimulation.

BACKGROUND AND OBJECTIVES: Although the suppressive effect of the medial efferent acoustic reflex is well known, the time course of this effect over prolonged periods has yet to be fully evaluated. We assessed time-dependent change in the suppression of transient evoked otoacoustic emissions (TEOAEs) by the medial efferent acoustic reflex over a relatively long period. SUBJECTS AND METHODS: We measured TEOAEs in the right ear before contralateral acoustic stimulation (CAS), and then measured serial TEOAEs in the right ear at four intervals during a total of 16 minutes of continuous CAS, followed by three more recordings after termination of CAS. RESULTS: TEOAE amplitudes were reduced with CAS during a certain period (from the immediate period to 10 minutes depending on frequency) and subsequently recovered. TEOAE suppression values in the mean amplitudes for overall frequency were 0.76 dB at the initial recording, 0.35 dB at 5 minutes, 0.44 dB at 10 minutes, and 0.33 dB at 15 minutes during CAS. The initial suppression value was significantly larger than other suppression values of 5, 10, and 15 minutes (p<0.05). In recordings obtained after CAS, TEOAE amplitude exceeded pre-acoustic amplitudes at 1 kHz, 1.5 kHz, and 2 kHz. CONCLUSIONS: The present results show the existence of the medial efferent acoustic reflex and demonstrate the time course that TEOAE suppressions present initially after CAS, showing fatigue over time. Overshooting of TEOAE was observed in recordings at several frequencies after termination of CAS.

9.Analysis of Correlation between Cognitive Function and Speech Recognition in Noise.

BACKGROUND AND OBJECTIVES: Speech in the noise test has been known to reflect the distortion component of hearing impairment originated not only from the cochlea but also from the central auditory system. For communication with language, a proper cognitive function of the cerebral cortex is essential. In the present study, we aimed to evaluate the correlation between the cognitive function and the speech recognition ability against background noise. SUBJECTS AND METHOD: Thirty one listeners with sensorineural hearing loss in both ear participated in this study. We performed pure a tone audiometry, a speech recognition test with and without multitalker babble noise, a computerized cognitive function test and the Mini-Mental Status Examination-Korea. RESULTS: Speech recognition score in noise showed significant correlations with the results of both computerized cognitive function test and Mini-Mental Status Examination-Korea, whereas, the pure tone average and speech recognition score in the quiet background did not. CONCLUSION: These results support that speech recognition ability in background noise reflects the cognitive function.

10.A Case of Acute Suppurative Labyrinthitis Complicated by Chronic Otitis Media.

With the development of antibiotics, tympanogenic labyrinthitis complicated by otitis media rarely occurs nowadays. Only few cases of acute serous or suppurative labyrinthitis in otitis media patients have been reported previously in the literature. Acute suppurative labyrinthitis caused by direct bacterial invasion into the inner ear leads to potentially severe, irreversible hearing loss, vertigo and spontaneous nystagmus toward the healthy side. Magnetic resonance image might be helpful to recognize the inflammatory lesions of the labyrinth. Treatment for tympanogenic labyrinthitis includes an appropriate use of antibiotics and establishment a draining route. Recently, we experienced an unusual case of acute suppurative labyrinthitis complicated by chronic otitis media without cholesteatoma. We also review and discuss the clinical manifestations, radiologic findings and treatment strategy of this disease.
