2.Assessment of Neurobehavioral Performance among Welders Exposed to Manganese.

OBJECTIVES: A cross sectional study was carried out in the welders exposed to manganese and control group. The aim of this study was to assess the adverse effects of manganese on central nervous system of welders. METHODS: The study group consisted of 49 welders and the reference group, 49 workers matched with welders for age and educational level in automobile manufacturing company. Concentration of manganese in welding hume, in blood, and cumulative exposure index(CEI) were determined for each subject. For each group, computerized neurobehavioral test including Simple Reaction Time, Digit Symbol, Digit Span, and Finger Tapping Speed of Swedish Performance Evaluation System were applied. RESULTS: Exposure level of manganese in welders were very low(mean CEI 0.069). Welders exposed to manganese had slower response speed, slower motor steadiness, and shorter memory span than control group. But these differences were not statistically significant. The performance of Finger Tapping Speed tended to lower as environmental and biological exposure index got higher. Other neurobehavioral tests were found to have a little association with the exposure indices. CONCLUSIONS: There was no significant difference of neurobehavioral performance between welders and control group. This study indicates that Finger Tapping Speed measuring motor steadiness might be a one of useful tests for early detection of adverse effects of welding on central nervous system.

4.Expression of the Low Molecular Weight Cyclin E is Early Event in Colorectal Carcinogenesis.

PURPOSE: Cyclin E is essential for the transition from the G1 to S-phase of the cell cycle, and plays important roles in carcinogenesis in many cancers. Especially, low molecular weight cyclin E is overexpressed in breast cancer and its level of expression correlates well with the progression and prognosis. Although the cyclin E level is amplified, and overexpressed, in many cancers, including colorectal cancer, the role of low molecular weight cyclin E in colorectal cancer remains to be studied. Therefore, the expression of low molecular weight cyclin E in various stages of colorectal tumors was studied. MATERIALS AND METHODS: The expression of low molecular weight cyclin E was analyzed in 45 tumors, and compared with paired normal mucosa from the same patients (6 adenomas, 11 stage A, 14 stage B and 14 stage C colorectal cancers) by Western blot analysis. The expres sion of low molecular weight cyclin E was also analyzed in normal colon mucosa from 12 healthy normal controls. RESULTS: The low molecular weight cyclin E was expressed exclusively in all stages of colon tumors, but not in the normal mucosa from the same patients or in the normal controls. However, there was no correlation between tumor progression and the degree of expression of low molecular weight cyclin E. CONCLUSION: The expression of low molecular weight cyclin E is suggested to be an early event in colorectal carcinogenesis.

7.Posterior Stabilization with Halifax Interlaminar Clamps in Unstable Cervical Spine Injuries.

Fifty-two patients with unstable cervical spine injuries underwent operations between January 1, 1990 to June 30, 1997 in our institution and this is retrospective study on clinical results about these patients. Patients with cervical instability resulted from injury posterior column, being included in White's criteria, were treated with Hafifax interlaminar clamps for cervical instability. We followed up forty-two cases for more than one year. All of the forty- two cervical fixations showed immediate and long term stability and there were no neurological complication. Neu rologic improvements were seen in thirty three, but thirteen cases were unchanged. These results indicate that Halifax interlaminar clamps are consider to be a safe and effective method for posterior stabilization of va rious causes of cervical spine instability.

8.Remnant Preservation is Helpful to Obtain Good Clinical Results in Posterior Cruciate Ligament Reconstruction: Comparison of Clinical Results of Three Techniques.

BACKGROUND: The purpose of the present study was to compare the clinical results of 3 posterior cruciate ligament reconstruction techniques according to the time from injury to surgery and remnant PCL status and to evaluate the efficiency of each technique. METHODS: The records of 89 patients who underwent primary PCL reconstructions with a posterolateral corner sling were analyzed retrospectively. Thirty-four patients were treated by anterolateral bundle (ALB) reconstruction with preservation of the remnant PCL using a transtibial tunnel technique in the acute and subacute stages of injury (group 1). Forty patients were treated with remnant PCL tensioning and an ALB reconstruction using the modified inlay technique in the chronic stage (group 2), and fifteen patients were treated with double-bundle reconstruction using the modified inlay technique (group 3). The double-bundle reconstruction was performed if there was a very weak or no PCL remnant. RESULTS: The mean side-to-side differences in posterior tibial translation on the stress radiographs were reduced from 10.1 +/- 2.5 mm in group 1, 10.6 +/- 2.4 mm in group 2, and 12.8 +/- 3.2 mm in group 3 preoperatively to 2.3 +/- 1.4 mm in group 1, 2.3 +/- 1.5 mm in group 2, and 4.0 +/- 2.5 mm in group 3 at the last follow-up (p < 0.001, p < 0.001, and p < 0.001, respectively). Statistical analyses revealed that group 1 and group 2 were similar in terms of side-to-side difference changes in posterior tibial translation on the stress radiographs; however, group 3 was inferior to group 1 and group 2 at the last follow-up (p = 0.022). The clinical results were not significantly different among the three groups. CONCLUSIONS: Excellent posterior stability and good clinical results were achieved with ALB reconstruction preserving the injured remnant PCL in the acute and subacute stages and remnant PCL tensioning with ALB reconstruction in the chronic stage. The PCL injuries could be surgically corrected with different techniques depending on both the remnant PCL status and the interval between the knee trauma and operation.
