2.The Effect of Tibiofemoral Alignment on the Results of Total Knee Arthroplasty

There have been several reports that total knee arthroplasty(TKA) was most likely to survive successfully if the coronal tibiofemoral angle was close to 7 degree of valgus, the accepted normal. In order to know whether the postoperative coronal tibiofemoral angle influences the result of total knee arthroplasty, we examined the 152 TKA cases in 106 patients which had been performed at Department of Orthopaedic Surgery, Seoul National University Hospital from Jan. 1987 to Dec. 1991. In our series the average follow-up period was 31 months(range, 13 months to 75 months). Coronal tibiofemoral angles of all cases were checked on weight bearing anteroposterior plain X-rays and were divided into three groups(less than valgus 2° ; valgus 3° to 7° ; greater than valgus 8°). All cases were analyzed on the knee score of Hospital for Special Surgery and the American Knee Society total knee arthroplasty roentgenographic evaluation and scoring system and the cases with radiolucent lines wider than 2mm were checked. Three groups were analyzed comparatively with statistical means(Wilcoxon signed rank test, X² test). There were no significant differences in the clinical results of total knee arthroplasty according to the postoperative coronal tibiofemoral angle. Lateral subluxation of the patella occurred more frequently when the postoperative coronal tibiofemoral angle was above valgus 8°. In the tibial components of group lesser than valgus 2°, the frequency of radiolucent lines wider than 2mm was higher than those of other groups, but its significance should be examined through long term follow-up.

3.A Preliminary Report of Hybrid Total Knee Arthroplasty: Comparative STduy with Cementless Technique

Although long-term studies of total knee arthroplasty(TKA) have confirmed reliable relief of pain and maintenance of function, survivorship at nine to ten years has been inconsistent with late component loosening being the most frequent mode of failure. Tibial component failure has been more frequent than failure of the femoral or patellar component in most TKA series. Encouraged by the early results of porous-ingrowh femoral and patellar components, and aware of retrieval studies showing poor bone ingrowth into tibial components, the “hybrid” method of fixation with an uncemented femoral component and a cemented tibial component, has been introduced and incorporated the potential advantages of both traditional fixation techniques. Between January 1987 and December 1991, forty-five “hybrid” TKAs(group I) were performed in degenerative arthritis and evaluated retrospectively in comparison with the results of nineteen noncemented porous-coated TKAs(group II) in degenerative arthritis. The average follow-up period was 31 months(range, 14 to 60 months) in group I and 26 months(range, 13 to 63 months) in group II. Using the knee rating scale of the Hospital for Special Surgery and the American Knee Society Roentgenographic Evaluation and Scoring System, clinical and radiological comparative study between two groups was performed. Pain scores, range of motion and HSS knee scores improved in both groups, but there was no significant difference between group I and group II(p>0.05). In both groups, the scores of radiolucent lines were significantly higher in zone 1 and 4 of A-P roentgenograph of the tibial component than in other zones(p < 0.05). The tibiofemoral angle was corrected from varus 2.7° to valgus 5.8° in group I and from varus 3.4° to valgus 4.6° in group II. There was no significant differences in component position and scores of radiolucnet lines between group I and group II. In conclusion, there were no significant differences in clinical and radiological results between the “hybrid” and the cementless groups. Results of this study suggest that both the hybrid fixation and the comentless techniques can reliably provide satisfactory clinical and radiological results in properly selected degenerative arthritis patients. But the final outcomes of this study need further evaluation through long term follow-up.

4.Transpalpebral Subperiosteal Forehead Lift.

By the human is getting older, the factors which can the reason of the aging process in the frontal region are divided, static factor and dynamic factor. the static factor is gravity, and the dynamic factor is repeatitive competition of depressosr muscles and elevator muscles. the depressor muscles are corrugator muscle, procerus muscle and orbicularis oculi muscle and the elevator muscle is frontalis muscle. Correction methods of this aging process are divided to non-surgical and surgical method. Non-surgical method are Atecoll and fat injection, and using botulinum toxin. But the effect of these method is temporary and limited, and if the skin laxity is great, this method cannot be used. Surgical methods are laser, chemical peeling, dermabrasion, classical forehead lift which is dissected superficial to galea aponeurosis or subperiosteal plane through coronal or hairline incision, and endoscopy method that the corrugator muscle and procerus muscle are transected by using endoscope, and then the posterior elevation of forehead flap is induced. the endoscopy method is the most popular method in recent years, which has the many advantages of minimal incision, less amount of bleeding and lower complication, but expensive equipment, adaptation and training period are needed. We present the result of 10 patients from May. 1996 to Jan. 1997. After the superior orbital rim exposed through upper eyelid incision, the corrugator muscle was resected while careful attention to the supraorbital n. which was located behind the orbicularis oculi muscle. A communication was made through both sided of medial canthal area, and after the procerus muscle was resected, the fat graft was inserted between them. Finally, we made periosteal incision superiorly, and subperiosteal forehead lift was done without using endoscope.

5.A Nationwide Survey on the Child Day Care and Common Infectious Diseases.

PURPOSE: As the number of children who attend child care centers has increased, concerns has increased about the effect of child day care on childhood illness. This study was conducted to examine the relationship between experience in child care and common infectious diseases in children under 5 years of age. METHODS: Data were collected by surveying 1,000 respondents with children under age 5 through online interviews using a structured questionnaire. The contents of the survey were composed of demographic characteristics, child care facilities usage, experience in infectious diseases, and immunization status. RESULTS: Among the 1,000 children <5 years of age, 78.5% attended a child care facility. Rates of common communicable illnesses were higher in children in child care than for children reared exclusively at home. The predominant communicable diseases which the respondents' children experienced, in order of decreasing frequency, were gastroenteritis (47.1%), otitis media (41.8%) and pneumonia (19.1%). The immunization rate of vaccines that are not included the national immunization program (NIP) (Haemophilus influenzae type b vaccine - 76.6%, hepatitis A vaccine - 63.3%, pneumococcal vaccine - 59.4%, rotavirus vaccine - 43.1%) was lower than that of the NIP vaccines (90.4%). CONCLUSION: Children in child care experience more bouts of common infectious disease, so nationwide policies to prevent or to control the spread of infectious agents in a child-care should be available and appropriate immunization should be emphasized as the most effective method for the control of infectious disease for children.

6.Clinical experience of Dentocutaneous fistula treatment.

Dentocutaneous fistula of the face neck is an infrequent manifestation of chronic dental infection. A periapical dental abscess may be initiated by disease, trauma, or thermal or chemical injury and develops into an extensive necrosis of surrounding tissue. Diagnostic errors can result in multiple excision, biopsies, and ineffective long-term antibiotic therapy. Awarness that periapical dental abscess is the most common etilolgic factor of cutaneous sinus tracts involves the face & neck will facilitate their early diagnosis and prevent needless treatment or anxiety for the patient. From 1994 to 1998, we have performed 6 cases of dentocutaneous fistula. Among of them, three were men and the others were women, age ranged from 18 to 66-year-old, and morbidity period was from 3 weeks to 3 years, the follow-up period ranged from 5 to 18 months (mean period 11 months). In conclusion, an understanding of the pathogenesis of cutaneous fistulae arising from dental infections will lead to proper early diagnosis and treatment without unnecessary surgery.

7.Phylogenetic Analysis of Human Bocavirus in Hospitalized Children with Acute Respiratory Tract Infection in Korea.

PURPOSE: Human bocavirus (hBoV), a recently discovered virus, has been detected in children with respiratory tract infections worldwide. The aim of this study was to analyze the frequency and molecular phylogeny of hBoV in the respiratory samples of children with acute respiratory tract infections in 2010. METHODS: Nasopharyngeal samples were collected from 953 children with lower respiratory tract infections at Severance children's hospital in Korea from January 2010 to December 2010. We applied the multiplex PCR technique for the identification of 12 respiratory viruses from the samples. Among the total specimens, hBoV positive samples were subjected to phylogenetic analysis by sequencing a fragment of the VP1/VP2 gene junction. RESULTS: hBoV was detected in 141 (14.8%) among 953 patients. The 61.7% of hBoV-positive samples were found to co-exist with other respiratory viruses. The results of phylogenetic analysis showed that all 141 hBoV-positive isolates were identified as hBoV 1, revealing a high similarity among the isolates (>98%). CONCLUSION: hBoV 1 with minimal sequence variations circulated in children with acute respiratory infections during 2010. More research is needed to determine the clinical severity and outcomes of the minimal sequence variations.

8.Arthroscopic Treatment of Symptomatic Discoid Lateral meniscus Without Tear.

While there is little controversy about the surgical resection for the torn discoid lateral meniscus, there are some contradictory reports for the treatment of discoid lateral meniscus without tear. Some authors argue that initial treatment should be observation after diagnostic arthroscopy in the cases without tear. Others reported that excellent results were obtained after partial resection of the discoid lateral meniscus without tear or with slight degeneration. We reviewed our series of symptomatic discoid lateral menisci without tear, confirmed by arthroscopic examination. We retrospectively reviewed 8 knees in 8 patients who had had partia! Meniscectomy for discoid latera1 meniscus without tear, confirmed by arthroscopic examination. They were 5 males and 3 females. The average age was 22.8 years (range, 7 to 57 years). The average duration of follow-up was 2 years and three months. The patients had continuing pain and locking or snapping preoperatively. Preoperative MRI had grade Il intrameniscal signal in all the cases. The patients were evaluated hy Lysholm pain score and total Lysholm score. There were 7 complete types and one incomplete type. The average Lysholm pain score and the total Lysholm score improved from 13.1, 67.8 preoperatively to 28.8. 97.9, at the last follow-up, respectively. Mechanical symptoms including click and 1ocking were disappeared in all, and no complication was obser ved. We suggest that symptomatic discoid lateral meniscus should he resected e.ven though there is no tear visible on arthroscopy, especially when the MRl shows grade lI intrameniscal signal.

9.Patellofemoral Complications Following Total Knee Arthroplasty: Comparative Study between the Group with Lateral Retinacular Release and the Group without Lateral Retinacular Release.

Results of 170 total knee arthroplasties(TKAs) performed from 1986 to 1995, followed up for at least one year, were reviewed. Lateral retinacular release was performed in 76 knees(45%) and not performed in 94 knees(55%). We compared the results of TKAs combined with lateral retinacular release with those without lateral retinacular release. The clinical results of TKAs were evaluated by the Knee Scoring System of Hospital for Special Surgery(HSS). The average follow-up period was 34 months. The clinical results assessed by HSS score, pain score and range of motion improved postoperatively in both groups, but there was no significant difference between the two groups. There were 8(4.7%) with patellar subluxations in 170 TKAs, one(1.3%) in the group with lateral retinacular release and 7(7.5%) in the group without lateral retinacular release(t-test; p<0.05). Analysis of lateral tilt of patella on roentgenographs also showed a significant difference between the two groups. 7 knees(9.2%) with lateral tilt were observed in the group with lateral retinacular release and 22 knees(23.4%) with lateral tilt in the group without lateral retinacular release(t-test; p<0.05). There was no osteonecrosis or fracture of patella in either group. In summary, lateral retinacular release might improve the patellar tracking and lower the incidence of patellar subluxation and lateral tilt of patella. Lateral retinacular release alone did not seem to cause patellar osteonecrosis and subsequent patellar fracture which were detectable in plain roentgenographs. So, we suggest that lateral retinacular release can be performed without great risk of potential patellar complications in TKAs requiring lateral retinacular release for proper patellar tracking.

10.A Case of Ruptured Aneurysm of the Sinus of Valsalva into the Left Ventricle with Complete AV Block in Behcet's Syndrome.

Behcet's disease is an inflammatory condition of multiple organ systems in witch recurrent oral and genital ulcers are the most typical signs. Less common clinical features include cerebral vasculitis, arterial aneurysm, deep vein phlebitis, aseptic meningitis, and discrete bowel ulcers. The most serious complication of Behcet's disease is arterial involvement especially ruptured arterial aneurysm. The aneurysm of sinus of Valsalva in Behcet's disease is a rare condition. There is no report about it in Korea. We report a case of a aneurysm of the sinus of valsalva with Behcet's disease. The patient had complete AV block and a right coronary sinus of Valsalva aneurysm which ruptured into the left ventricle. The diagnosis was made with transesophageal echocardiography. The patient was implanted with permanent pacemaker for relief of congestive heart failure due to complete atrioventricular (AV) block. He discharged and he is still follow-up in outpatient clinic.
