1.Common Variations of Renal Vessels in Donor Kidneys.

To prevent extensive manipulation of the kidney and profuse bleeding, the exact information of the renal vessels is very important. On the basis of the intraoperative observations of 127 donor kidneys and preoperative angiography, we studied the renal vascular variations. Both kidneys had single renal arteries in 70.1% of the cases and multiple renal arteries were found in 29.9% of the cases. 89% of the cases had single renal arteries and single renal veins, and 4.8% of the cases were drained by multiple renal veins on operative field. Left renal veins had three branches, adrenal vein, gonadal vein and lumbar vein in 84% of the cases. Only adrenal vein and gonadal vein were identified in 13% of the cases. Anomalies of the IVC were identified in two cases. One case was left postrenal IVC and the other case was persisting left IVC. Right renal veins had no branch in 88.9% of the cases and gonadal vein was identi5ed in 11.1% of the cases in our study. In surgical procedures about the renal area, familiarity with common variations of the renal vessels and anomalies of IVC is very important for the surgeon.

2.Two Cases of Fetus Papyraceus in Twin Pregnancy.

The fetus papyraceus is a remarkably mumified fetus and the complication is rare in multiple pregnancy. If one fetus of multiple pregnancy died in uterus remote from term and other fetus did not, the fetus with placenta is compressed by the membrane of living fetus in uterus and then the dead fetus becomes a fetus papyraceus. Recently we had experienced two cases of fetus papyraceus in twin pregnancy at our department of Obstetrics and Gynecology, Keimyung University School of Medicine. We present these cases with brief literatures.

Renal cell carcinoma in the pediatric age group has been considered as a rare clinical disease entity but perhaps not as rare as once thought. This neoplasm should be considered in the differential diagnosis of hematuria and/or abdominal masses in children. Herein a case of pathologically confirmed pediatric renal cell carcinoma with multiple cysts in liver is presented with a brief review of literature.

Coronavirus disease 2019 (COVID-19) is caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which is affiliated with the β-coronavirus subgroup, which includes SARS-CoV and Middle East respiratory syndrome coronavirus (MERS-CoV), but is far more infectious than the 2. Because it is potentially life-threatening to infants and pregnant women with weak immune systems, clinical manifestations and vertical transmission of COVID-19 are matters of interest. Staff of the obstetrics department of university hospitals in Daegu and of the Daegu metropolitan government designated Daegu Fatima Hospital for the delivery of pregnant women with suspected and confirmed SARS-CoV-2 infection. Thirteen pregnant women with laboratory-confirmed COVID-19 were identified. Among them was a 28-year-old pregnant woman who had recovered from COVID-19 and had given birth to a healthy girl at 38 weeks of gestational age. We present our uncommon experience with a brief review of literatures.

6.Risk factors of delirium in elderly inpatients.

BACKGROUND: This study was done to investigate the clinical risk factors of delirium in general hospital's elderly inpatients. METHODS: A case-control study design was used. Forty cases and age -sex matched 120 contols were identified using hospital discharge data from June 1995 to May 2001. The controls were selected during the same period. The clinical records of each subject were reviewed using discharged disease code to select cases. Subsequent7, the data were recorded from clinical records. RESULTS: The subjects admitted with delirium tended to have increased age (P=0.007), hypoxia(P=0.018), low hemoglobin(P =0.011),sleep disturbance (P=0.001), more ADL total dependence ( P=0.018), diagnoses with infections (P=0.006), diabetes (P=0.046), and hypertension (P=0.041). Above risk factors seemed to be clinically associated with delirium. CONSLUSION: These findings indicate that rapid infection control, maintenance of good sleep hygiene, well-controlled diabetes and hypertension, and correction of hypoxia may help to prevent delirium in general hospital's elderly inpatients.

7.Treatment of a Coronary Arterial Stenosis in a Child with Kawasaki Disease by Percutaneous Transluminal Coronary Angioplasty: Case Report and Literature Review.

Percutaneous transluminal coronary angioplasty(PTCA) is rarely performed on patients with coronary arterial stenosis that resulted from Kawasaki disease. We experienced a 3 year 10-month-old male with a history of Kawasaki disease who developed a few numbers of fusiform aneurysm on the right and left coronary artery. We examined and followed up the patient for 21 months using eletrocardiography, echocardiography, scintigraphy, and coronary angiography. The angiography was performed at 4 months initially and repeated 21 months after the onset because of a perfusion defect at scintigraphy. A significant stenotic lesion was found on the right coronary artery. Twenty-one months after the onset, the stenotic lesion was successfully dilated after percutaneous transluminal coronary angioplasty and luminal patency was maintained for over 1 year. We report this case and a review of literatures.

8.A case of clear cell carcinoma that is unrelated to diethystilbestrol of the uterine cervix.

Clear cell carcinoma of the uterine cervix is rare type of the uterine cervical adenocarcinoma. Although uterine cervical adenocarcinomas presently represent 20% to 30% of cervical cancers in the industrialized countries, the clear cell carcinoma of uterine cervix is very rare malignancy that accounts for 4% to 9% of the adenocarcinoma of uterine cervix. This malignancy occurs in two distinct age groups; those younger than 24 years and those older than 45 years. In younger patients, most of these malignancies are mainly related to prenatal diethylstilbestrol (DES) exposure, but older patients are unrelated to in utero DES-exposure. We experienced a case of clear cell carcinoma of the uterine cervix of 34 years old housewife who was not related to prenatal DES-exposure. We presented a case with a brief review of related literatures.

9.The Effect of Intraurethral Instillation of PGE1 Solution in Patients with Erectile Dysfunction.

PURPOSE: Intracavernosal self injection therapy is now being widely used to treat patients with erectile dysfunction. However, there is a large number of patients who give up the self injection program due to the fear of injection. Therefore, less invasive route of drug administration is highly recommended. We investigated the efficacy of intraurethral instillation of Prostaglandin El (PGE1 ) solution in the patients who showed full erection with intracavernosal injection of PGE1. MATERIALS AND METHODS: Twenty-nine impotent patients who showed full erection with intracavernosal injection of PGE1 were included in this study. We estimated the grades and durations of the penile erection after intraurethral instillation of PGE1 RESULTS: Sixteen out of 29 patients(55.2%) showed full erection with intraurethral instillation. Two patients(6.9%) showed urethral pain which disappeared within 24 hours. But there was no systemic side effect CONCLUSIONS: Intraurethral instillation of PGE1 appears to be safe, well tolerated, and less invasive treatment modality Thus it can be selected as an alternative treatment of impotence in selected cases.
