1.Expression of p53 and Matrix Metalloproteinase-2 Proteins in Colorectal Adenocarcinoma.

The p53 gene is believed to play an important role through the mutation and overexpression in the progression of various human malignant tumors. The type IV collagenase (matrix metalloproteinase: MMP-2) initiates the degradation of the extracellular matrix, and consequently may play a role in the tumor invasion and metastasis. To investigate the correlation between clinicopathologic features of the colorectal adenocarcinomas and benign tumors and expression of p53 and MMP-2 proteins, we performed an immunohistochemical study on 40 colorectal adenocarcinomas, 20 adenomas and 20 hyperplastic polyps by using the antibodies to p53 and MMP-2 proteins. The positive expression rate of the p53 protein in adenocarcinomas was 62.5% and significantly higher than in benign tumors. The positive expression rate of the MMP-2 protein was 47.5% in adenocarcinomas, but there was no expression of MMP-2 protein in benign tumors. The difference in p53 and MMP-2 expression rates between malignant and non-malignant tumors was statistically significant. The positive expression rate of p53 protein in the non-metastatic and metastatic adenocarcinomas was 59.1 and 66.7%, respectively. The positive expression rate of MMP-2 protein in the non-metastatic and metastatic adencarcinomas was 45.5 and 50.0%, respectively. The correlation between several clinicopathologic features and expression of p53 and MMP-2 protein was not statistically significant, but the rate of positive MMP-2 immunoreactivity showed a statistically significant difference between Astler-Coller stage B1 C1 group and B2 C2 group of adenocarcinoma (p=0.0431). We concluded that the expression of p53 and MMP-2 protein contributes to the cancer development and MMP-2 may play a certain role in the invasiveness of the colorectal tumor. p53 and MMP-2 protein expression is not correlated with lymph node metastasis.

2.A Case of Unruptured Non - communicating Rudimentary Horn Pregnancy Progressed to the Third Trimester of Pregnancy.

The uterus with rudimentary horn occurs as a result of a lack of development during fetal life of the middle and lower parts of one of the Miillerian ducts, in which there is a failure of fusion of the two ducts, The incidence of this uterine anomaly is rare. Many cases of rudimentary uterine horn are not discovered since they do not become involved in a pregnancy, and thus remain symptomless and uncomplicated, and the diagnosis usually being made only with pregnancy with rupture. In this presentation, we describe a case of diagnosis and management of unruptured non-communicating rudimentary uterine horn pregnancy at the third trimester. The patient diagnosed by ultrasonography and MRI, was hospitalized for further evaluation and delivery under the impression of unruptured non-communicating rudimentary horn pregnancy. A living male infant weighing 1,930 gm was delivered by cesarean section at 33+l weeks due to impending preterm labor. Some articles concerned this subject were reviewed briefly to discuss relevant method of diagnosis, treatment, and clinical characteristics.

3.A Case Report of Symptomatic Torus Palatinus.

PURPOSE: Torus palatinus is a bony prominence at the middle of the hard palate. The size varies from barely discernible to very large, from flat to lobular. This oral exostosis is not a disease or a sign of disease, but if large, may be a problem. So, we present the clinical and histopathologic features and applied therapy and provide a comprehensive review of the rare case of the symptomatic exostoses. METHODS: A 37-year-old woman had slowly growing exophytic nodular mass of the bone that arises the midline suture of the hard palate. The patient was concerned about discomfort associated with movement of her tongue and about frequent irritation of the palatal mucosa during mastification of the hard food. The patient had a large, unilobulated torus palatinus. It extended from the area adjacent to the canine to a point beyond the junction with the soft palate. The mass was oblong in shape, measuring about 3cm long, 2cm wide, and 0.8cm in height. RESULTS: Before surgical intervention a CT was obtained for the sake of estimating the thickness of the bone between the exostoses and the maxillary antrum and floor of the nose. The surgical procedure was performed with the patient under general anesthesia. Removal of the exostosis was performed after midline mucoperiosteal incision with osteotome and diamond burr. Histologic finding revealed decalcified dense bony tissue, the presence of lacunae, and normal osteocytes. CONCLUSION: Surgical removal is recommended when one or more of the following condition exist: interference with the construction of prosthesis, interference with oral function, irritation or pathology of the overlying tissue, inability of the patient to maintain proper oral hygiene, and fear of malignancy or other psychologic trauma. We report a rare case of the torus arising in hard palate with symptoms.

4.Balloon Valvuloplasty of Pulmonary Stenosis in Patients Younger than 6 Months of Age.

PURPOSE: Balloon valvuloplasty(BVP) is the treatment of choice for valvular pulmonary stenosis (PS). However, this procedure was usually performed in children older than 2 years. The purpose of the present study was to assess the safety and efficacy of BVP in young infants. METHODS: Retrospective analysis of the medical records of 25 infants younger than 6 months of age who received BVP for PS including critical PS between July 1991 and September 1997 were evaluated. RESULTS: The arterial oxygen saturation before and after procedure was 76.7% and 90.0%, respectively(P<0.001). Transvalvar pressure gradients measured with continuous wave Doppler echocardiography changed from 103.2+/-27.1mmHg to 34.8+/-14.4mmHg(P<0.001) and 29.3+/-15.9 mmHg after 6 months(P=0.075). The systolic pressure ratio of right and left ventricle before and after procedure was decreased from 1.40+/-0.4 to 0.74+/-0.3(P<0.001). Successful gradient relief was achieved with initial BVP in 21 out of 25 infants. There was one procedural death and one emergency surgery after BVP. Four of the 23 remaining patients required repeated BVP. CONCLUSION: Balloon valvuloplasty in young infants is a safe and effective procedure. In patients with symptomatic severe PS, balloon valvuloplasty should be recommended at an early age.

5.Comparison of Optical Coherence Tomography Characteristics among Three Subtypes of Exudative Age-related Macular Degeneration.

PURPOSE: To compare the characteristics of optical coherence tomography in eyes with treatment-naïve typical neovascular age-related macular degeneration (typical nAMD), polypoidal choroidal vasculopathy (PCV), and retinal angiomatous proliferation (RAP). METHODS: One hundred fifty-three eyes newly diagnosed with exudative AMD were retrospectively collected. All study eyes were classified into three subtypes: typical nAMD, PCV, and RAP. Subfoveal choroidal thickness (SFCT) was measured using enhanced depth imaging optical coherence tomography (EDI-OCT). Central macular thickness (CMT) and other OCT features including intraretinal cystoid fluid and subretinal fluid were also evaluated in all eyes. SFCT, CMT and other OCT features were compared among the three subtypes of exudative AMD. RESULTS: Seventy-four eyes with typical nAMD, 55 eyes with PCV, and 24 eyes with RAP were included. SFCT was significantly thickest in PCV and thinnest in RAP (p < 0.001). RAP showed the thickest CMT and the highest frequency of intraretinal cystoid fluid (p = 0.004, p < 0.001, respectively). CONCLUSIONS: In patients with exudative AMD, different characteristics of OCT were observed according to the three subtypes. Identification of OCT characteristics could help differentiate the subtypes of exudative AMD.

6.Syphilitic Granulomatous Pancreatitis: A case report.

Syphilitic granulomatous pancreatitis is an extremely rare condition,and can occur in the generalized acquired syphilitic patient in tertiary or secondary phase. The most serious problem with granulomatous pancreatic lesion is clinical or radiological misdiagnosis as cancer. We experienced a case of syphilitic granulomatous pancreatitis arising in 54 year old female patient. She was treated for syphilis 20years ago. But she and her husband are still strong positive to VDRL and TPHA. On abdominal computed tomography and endoscopic pancreatico- duodenography, there was an obstructive mass of low density in the distal common bile duct or pancreatic head. Under the preoperative diagnosis of pancreatic head carcinoma, Whipple's operation was done. On gross examination, the pancreas was fibrotic, and the common bile duct was well preserved without tumor mass. Microscopically, numerous intralobular noncaseating epithelioid cell granulomas with multinucleated giant cells are identified. They surround thick-walled, small to medium sized arteries and involve vascular wall with luminal narrowing or obliteration, which are characteristic findings of the syphilitic granuloma. The remaining parenchyme shows fibrosis, acinar atrophy or destruction with dense infiltration of lymphohistiocytes, plasma cells with granuloma formation. Although the Warthin-Starry stain reveals no spirochetes, the serologic result and pathologic findings are compatible with syphilitic granulomatous pancreatitis.

7.Avacular Necrosis of the Femoral Head: MR Imaging with Histologic Correlation.

PURPOSE: To correlate MR findings with histologic findings in avascular necrosis(AVN) of the femoral head. MATERIALS AND METHODS: MR imaging was performed with 8 femoral head specimens using T1- and proton density weighted coronal SE sequences, and compared with contact radiography and histologic sections. RESULTS: In each specimen, necrotic zone in the superior portion of femoral head, repair zone located inferior to the necrotic zone, and rim adjacent to normal bone marrow could be defined. Necrotic zone showed high signal intensity on both T1- and proton density- weighted images in 3 cases which were composed of necrotic bone and marrow, and low signal intensity on both sequences in 2 cases which were composed of necrotic bone marrow with amorphous cellular debris. Mixed high and low signal intensities were seen in 3 cases. The repair zone showed low signal intensity on Tl-weighted image and high signal intenisty on proton density weighted image in 5 cases which were composed of thickened trabecular bone and meSenchymal tissue and also showed intermediate signal intensity on T1 weighted image and high signal intensity on proton density weighted image in 3 cases which were composed of osteoid, chondroid and undifferentiated mesenchymal cells. Rim shown as the low signal intensity on T1 weighted image in all cases was corresponded to viable thickened trabecular bone. CONCLUSION: MR imaging would be the best modality in the diagnosis of avascular necrosis of fernoral head and when used in conjuction with degree and location of signal intensity, the prediction of histologic finding may be possible.

8.A Case of Giant Ethmoid Osteoma with Orbital Extension.

Osteoma is a tumor of mature bone showing slow growth of well-differentiated bone. It occasionally produces symptoms of space occupying lesion and symptoms of ethmoid osteoma occur earlier than that of osteoma of the frontal sinus. Osteoma of orbital area is uncommon and it may produce ophthalmologic symptoms and cosmetic problems. Treatment remains controversial: open procedures are typically being used, however, small sized ethmoid osteoma are removed by intranasal procedure. Recently, the authors have experienced a case of giant ethmoid osteoma with orbital extension, which was removed with intranasal endoscopic and external ethmoidectomy approach. Hence, we report this case with a review of literature.

9.Simple double eyelid operation (oriental blepharoplasty) using Y needle.

The eyes, the saying goes, are the windows of the soul. It's the first thing you notice about a person. Therefore, many people want to possess beautiful eyelids. Surgical formation of a palpebral fold and sulcus divides the lid into two well-defined segments (palpebral and pretarsal), producing the double eyelid desired by many Oriental women as well as an increasing number of man recently. Upper lid blepharoplasty is the Oriental eye is one of the variations of standard upper lid blepharoplasty. In Oriental double eyelid surgery, there have been two approaches to form a superior palpebral fold: the buried suture(nonincision) method and the full external incision method. Conventionally, the nonincision technique has been shown to produce little postoperative edema. However, the probability of the fold disappearing is high, and this technique cannot be performed in patients with fatty eyelids. Conversely, the incision technique has contrary characteristics. Recently, partial incision(or semi-open) technique which is combination of mentioned methods is used, this technique is removal of pretarsal tissue, muscle, and/or orbital fat around 2 or 3 incision site to facilitate tarsus-dermal adhesion. Our method is on the basis of this technique, furthermore, compared with conventional semi-open method, Y(Yang's) needle assisted double eyelid operation is more easy, convenient, saving-time method and provide satisfactory results.

10.Clinical Characteristics of Papillary Thyroid Carcinoma in Children and Adolescent.

PURPOSE: Thyroid carcinomas are rare in childhood and adolescence, and the papillary thyroid carcinoma is the most frequent hisotological type. This study was undertaken to investigate clinical characteristics of the papillary thyroid carcinoma developing in childhood and adolescence. METHODS: From 1980 to 2006, 53 children and adolescents were treated for the papillary thyroid carcinoma in our department. Clinical characteristics were reviewed retrospectively. The clinical course of 9 patients was not evaluable because of inadequate follow-up and the remaining 44 patients were included in the final survival analysis. RESULTS: The median age at diagnosis was 14.9+/-3.9 years. There were 9 males and 44 females. There was a significant female preponderance (1:7.8) (P=0.035) in pubertal age group, while there was no sexual difference in prepubertal age group (1:1.25). 33 patients (62.3%) had local invasion to the perithyroidal tissue, 38 patients (71.7%) had regional lymph node metastasis, and 12 patients (22.6%) had pulmonary metastasis. Unilateral lobectomy, subtotal or total thyroidectomy was performed in all patients. 19 patients (35.8%) with local or distant metastasis were additionally treated with radioiodine ((131)I) ablation therapy. After initial treatment, 41 patients were alive and free of thyroid carcinoma, 3 patients were alive with persistent thyroid carcinoma. At median follow-up of 9.9+/-5.8 years, 10 patients (24.4%) among 41 patients were recurred but all 44 patients were alive, 5 with disease and 39 disease-free. CONCLUSION: Childhood Papillary thyroid carcinoma was more frequent in pubertal female. And it was associated with more locally aggressive, more frequent metastasis and had higher recurrence rates.
