2.Epidemiological Study of Contact Dermatitis.

Studies on epidemiological aspects of contact dermatitis have been published in recent years since the introduction of standardized method of patch test.However, most previous epidemiologic reports studied patients with allergic contact dermatitis in hospitals with patcb testing. Such data are not directly applicable to the population at large. Very recentiy there are a few reports ahout rates of exposure and patch test reactivity to certain antigens in a general population. which is difficult to be applicable in a developing country at present. With regard to this, we have tried to analyze the contact dermatitis questionaire containing various items of common causative agents producting contact dermatitis (for example, metals, cosmetics, plants, medicaments and rubber etc.) for the detection of past contact dermatitis histories in a total new dermatologic outpatients. A11 2258 dermatologic outpatients entered this study including 303 contact dermatitis patients (allergic contact dermatitis, primary irritant dermatitis and housewife eczema). We also analyzed the patch test results of 107 patients with suggesting contact dermatitis who visited our hospital at the same period of collecting questionaires. The study results were as follows. 1. Positive rate of each item of the questionaire (No. of said Yes/ No. of responders) was metals, 10.4% cosmetics, 31.8%, plants, 18.2%; medicaments, 12.4%; rubber, 5.8%; and others, 11.7%. 2. The most common all regens of the patch test positive reactions were nickel sulfate (9.3%), fragrance mix(9.3%), formaldehyde(6.5%), potassium dichromate (5.6%), neomycin sulfate (5.6%), balsam of Peru (4.7%), cobalt chloride(3.7%), wood tar(3.7%) and paraphenylene diamine (2.8%) in order of frequency. And the patch test positive rate of commercial cosmetic products was 10.3%. In conclusion, cosmetics, medicaments, metals, plants and rarely rubber are found the most common etiologic agents of contact dermatitis in this study. The reasons and back-ground of the results were also discussed in various aspects.

4.A Case of Respiratory Syncytial Virus Infection Presented with Apnea.

Respiratory syncytial virus(RSV) is currently implicated as the most common and otentially serious viral respiratory pathogen of young infants. Apnea in association with RSV infection occurred in about 18% of the infants. Premature birth and a young postnatal age were risk factors for development of apnea with RSV disease. We experienced a case of RSV infection in a 37 day-old premature girl. The presenting complaint was apnea, with respiratory signs and symptoms developing later. She required mechanical ventilation.

The origin of osteoclast-like giant cell tumor (OGCT) of the salivary gland has been debated because the prototypic cells of osteoclast-like cells and mononuclear stromal cells are largely unexplained in this gland. Bone marrow-derived CD14+ and CD45+ monocyte-derived multipotential cells (CD14+/CD45+ MOMC) may be one of the possible origins of OGCTs of salivary glands, which have never been explored in salivary OGCTs. We present a case of OGCT accompanied with carcinoma ex pleomorphic adenoma in the parotid gland of a 67-year-old Korean female. The tumor presented as a rapidly growing cervical mass comprising a central area of carcinoma ex pleomorphic adenoma and a peripheral circumferential area of OGCT. The immunohistochemical staining pattern was phenotypically consistent with bone marrow-derived CD14+/CD45+ MOMC. This case is the first report of a salivary OGCT in Korea.

7.Characteristics and Surgical Outcomes of Intertrochanteric or Subtrochanteric Fractures Associated with Ipsilateral Femoral Shaft Fractures Treated with Closed Intramedullary Nailing: A Review of 31 Consecutive Cases over Four Years at a Single Institution

PURPOSE: To evaluate the clinical characteristics of intertrochanteric or subtrochanteric fractures associated with ipsilateral femoral shaft fractures and assess the surgical outcomes of a novel, closed intramedullary nailing surgical approach designed to minimize fixation failure. MATERIALS AND METHODS: Between May 2013 and April 2017, 31 patients with intertrochanteric or subtrochanteric fractures associated with ipsilateral femoral shaft fractures treated with closed intramedullary nailing or long proximal femoral nail antirotation (PFNA) were enrolled in this study. Preoperative data included age, sex, injury severity score, body mass index, location of shaft fracture, injury mechanism, accompanying traumatic injury, walking ability before injury, and surgical timing. Perioperative outcomes, including follow-up period, types of intramedullary nails, number of blocking screws used, operation time, and blood loss were assessed. Radiologic outcomes, including union rate, time from surgery to union, and femoral shortening, and clinical outcomes, including hip flexion, walking ability, and Harris hip score were also evaluated. RESULTS: A total of 29 unions (93.5%) were achieved. The time to union was 16.8 months (range, 11–25 months) for hip fractures (15.7 weeks for intertrochanteric fractures and 21.7 weeks for subtrochanteric fractures) and 22.8 months for femoral shaft fractures. There were no significant differences in surgical outcomes between the two groups except for type of intramedullary nail. CONCLUSION: Closed intramedullary nailing in the treatment of intertrochanteric or subtrochanteric fractures associated with ipsilateral femoral shaft fractures may be a good surgical option. However, fixation of femoral shaft fractures might not be sufficient depending on the implant design.

8.Lipoma Arborescens in the Knee Joint: A Case Report.

Lipoma arborescens is a rare intra articular lesion consisting of a villous lipomatous proliferation of the synovial lining and occurs mainly in the knee joint. It is mostly found in older patients with joint trauma, meniscal lesions or chronic synovitis. The authors describe one case of a 29-year-old male with multiple lipomas in the right knee joint who has a history of joint trauma. He had joint effusion, pain, giving way and restricted terminal extension of the right knee. Histology and gross findings are consistent with the lipoma arborescens. After arthroscopic excision of the masses, symptom was relieved completely and the patient had gained full range of motion of the right knee joint.

9.Clinical Features and Prognosis of Lung Cancer with Brain Metastasis.

PURPOSE: Brain metastasis is estimated to occur in 20~40% of solid tumor patients and the most common primary tumor is lung cancer. Even though the prognosis of brain metastasis is grave and the 1-year survival rate is only 15%, symptom palliations are made with whole brain radiation therapy. We retrospectively evaluated the clinical features and prognostic factors of lung cancer with brain metastasis. MATERIALS AND METHODS: From January 1987 to October 1999, 50 lung cancer patients with brain metastasis underwent whole brain radiation therapy. We reviewed the improvement in neurologic symptoms and survival according to the following parameters; performance status, histological type, presence of brain metastasis at the initial diagnosis of lung cancer, presence of extracranial metastasis, multiplicity of brain lesion, presence of primary lung symptom and treatment modalities. RESULTS: The most frequent symptom with brain metastasis was a headache (50%). Palliation of the headache and other symptoms was achieved in 81% of the patients. Median overall survival after brain metastasis was 21 weeks and the 1 year survival rate was 15%. Patients without extracranial metastasis had a longer median survival than those with, 38 weeks versus 15 weeks, respectively (p=0.01). CONCLUSION: In lung cancer with brain metastasis, neurologic symptoms can be palliated with whole brain radiation therapy, and in this study among such patients, absence of extracranial metastasis can be a good prognostic factor.

10.Tuberculous Uveitis in a Hemodialysis Patient.

Tuberculous uveitis is a rare manifestation of tuberculosis infection. Although early diagnosis is important to saving the sight of patients, it is difficult. In dialysis patient, the diagnosis of tuberculosis is often delayed because of non-specific symptoms and extrapulmonary involvements. We report a case involving a 37-year-old man receiving hemodialysis who was presented with persistent fever and left- sided blurred vision. At ophthalmic examination, active papillitis, vitritis, and peripapillary retinal detachment were observed. Neck computed tomography showed multiple necrotic lymphadenopathies (LAPs) in right lateral neck, which was proven as tuberculous lymphadenitis on histological examination. After anti-tuberculosis therapy, fever was subsided and LAPs were improved. Visual acuity was also improved.
