PURPOSE: The purpose of the study was to find any difference of biometric features between acute angle closure glaucoma and chronic angle closure glaucoma. METHODS: A survey was conducted on glaucoma patients who visited the outpatient department between Jan 1, 1995 and May 31, 2001. We performed biometric measurements such as axial length, lens thickness, anterior chamber depth, and lens thickness/axial length in 48 patients with primary angle closure glaucoma who were previously done with laser iridotomy or trabeculectomy on both eyes. They are consisted of 24 patients of acute angle closure glaucoma, 24 patients of chronic angle closure glaucoma and 24 normal controls. RESULTS: This study attempted to determine morphological factors which may cause acute attack of primary angle closure glaucoma by comparing biometric features between chronic angle closure glaucoma and acute angle closure glaucoma, but the authors could not find any differences in the biometric features (lens thickness, axial length, anterior chamber depth) which may give rise to the acute attack. CONCLUSIONS: In the acute attack group and chronic angle-closure glaucoma group, there was no significant difference in biometric factors such as lens thickness, axial length, anterior chamber depth and ratio of lens thickness to axial length. Such result leads to the assumption that complex of biometric factors may give rise to acute attack. There's the need for further study with regard to morphological factor which gives rise to the acute attack.

BACKGROUND: The aim of this study was to determine which fall-risk tool is most accurate for detecting and predicting adults in the hospital setting. METHODS: A literature search was performed to identify all studies published between 1946 and 2014 from periodicals indexed in Ovid Medline, Embase, CINAHL, KoreaMed, NDSL and other databases, using the following keywords: 'fall', 'fall risk assessment', 'fall screening', 'mobility scale', and 'risk assessment tool'. The QUADAS-2 was applied to assess the internal validity of the diagnostic studies. Fourteen studies were analyzed using meta-analysis with MetaDisc 1.4. RESULTS: The result of comparing twelve tools was that the Morse Fall Scale (MFS) is the best tool for predicting falls for acute hospitalized adult patients. Six prospective validation studies using MFS with high methodological quality, involving 9,255 patients, were included. Meta-analysis finding of MFS was as follows; pooled sensitivity 0.73 (95% confidence interval [CI]: 0.68-0.78), pooled specificity 0.75 (95% CI: 0.74-0.76), area under the curve (AUC) of summary receiver operating characteristics (sROC) curve 0.79 (standard error [SE] = 0.02), and value of index Q* 0.72 (SE = 0.01) respectively. CONCLUSIONS: Falls in hospitalized adult patients can be effectively prevented using the MFS. These findings provide scientific evidence for using appropriate tool to prevent accidental falls and improve the safety of patients.

PURPOSE: Paresis of the inferior oblique is the least likely to result in paralysis. We report a patient without a history of trauma successfully treated using contralateral IO recession and SR recession. CASE SUMMARY: A 25-year-old male patient presented to us with an extended history of abnormal head posture, manifested by a marked habitual left head tilt with a face turn to the right. A cover test in the primary position demonstrated 15 prism diopter right hypertropia, which increased to 25 prism diopter right hypertropia in right gaze and 20 prism diopter right hypertropia in right head tilt. The patient was diagnosed with IO palsy, and a right IO recession was performed. RESULTS: Following the IO recession, head tilt was completely resolved and face turn to the right was slightly resolved. Cover test in the primary position demonstrated 12 prism diopter right hypertropia, which increased to 20 prism diopter right hypertropia in right gaze. A head tilt test demonstrated a symmetrical 12 prism diopter right hypertropia. We performed a right SR recession to decrease face turn and hypertropia in the primary position. CONCLUSIONS: We report a patient manifesting abnormal head posture diagnosed with IO palsy, which was successfully treated using contralateral IO recession and SR recession.

PURPOSE: This phenomenological study was done to identify comprehensively and in depth the experience of overcoming internalized stigma in persons with psychiatric disabilities. METHODS: Data were collected from December 2015 to June 2016 with 10 persons who had psychiatric disabilities. The participants took part in interviews on their experiences of overcoming stigma. The collected data were analyzed using Colaizzi method. RESULTS: From the raw data obtained from the study participants, 12 themes and 5 theme clusters were identified. The 5 theme clusters were found to be ‘being locked up in one's own stigma’, ‘trying to escape from the self in constraints’, ‘overcoming oneself by oneself’, ‘getting to see the true self’, and ‘living the life desired’. CONCLUSION: Study findings indicate the necessity of developing a mediation program to reduce and overcome the internalized stigma in the persons with psychiatric disabilities. A national and social effort is necessary for persons with psychiatric disabilities to help them overcome the internalized stigma.

PURPOSE: The study was done to identify factors influencing the quality of life of people with mental disabilities who use mental health centers. METHODS: Data were collected by questionnaires from 288 people with mental disabilities in Busan and Kyungnam Provience. Data were analyzed using t-test, ANOVA, Pearson correlation coefficients, multiple linear regression with the SPSS/WIN 14.0 program. RESULTS: The mean score for quality of life was 11.3+/-1.70, and the mean empowerment score was 59.7+/-8.44. The significant factors influencing quality of life were gender, age, residential status, number of support people, side-effects of drugs, service type, and empowerment. These factors explained 43.9% of the variance. CONCLUSION: The results suggest that in development and practice of program to increase the quality of life of people with mental disabilities a variety of factors should be considered.

Pigmented extraadrenal paraganglioma is an unusual neoplasm that has rarely been reported in the literature. Based on histochemical staining or electron microscopy, pigment has been classified as lipofuscin, neuromelanin or true melanin. We report a case of pigmented extraadrenal paraganglioma in the posterior mediastinum of a 70-year-old woman. Histologically, the tumor had a characteristic organoid architecture of "zellballen" pattern with rich delicate microvasculature. Tumor cells contained numerous coarse brown-black pigment granules. Ultrastructurally, the tumor showed abundant large electron-dense pigment granules that vary in size and shape and smaller membrane-bound neurosecretory granules. The larger granules were consistent with neuromelanin or lipofuscin. Histochemically, the pigment is most likely neuromelanin, which is a waste product of catecholamine metabolism.

Gestational trophoblastic tumors including choriocarcinoma bave become one of the most curable human malignancies with an overall cure rate exceeding 90%. Although systemic chemotherapy is the initial treatment for chorio- carcinoma, some patients with chemotherapy-resistant choriocarcinorna can be treated by integration of cbemotherapy, surgery and radio- therapy. We report two cases of persistent localized choriocarcinoma which was treated by surgical intervention.
