2.A Case of Prader-Willi Syndrome with Diabetes Mellitus.

Prader-Willi syndrome is characterized by infantile hypotonia, mental retardation, hyperhagia, hypogonadism and obesity. Approximately 60% of all PLW syndrome show an interstitial deletion of chromosome 15, 37% have apparently normal chromosome, and 3.6% have a variety of other abnormalities involving chromosome 15. Diabetes mellitus has been considered a component of PLW syndrome and the incidence is about 7%. We experienced a 17-year-old female who revealed mental retardation, hypogonadism, obesity, and non-insulin dependent type DM, compatible with Prader-Willi syndrome.

3.The Effect of Melatonin on Mouse Jejunal Crypt Cell Survival and Apoptosis.

PURPOSE: To evaluate protective mechanism of melatonin against radiation damage and its relationship with apoptosis in mouse jejunum. MATERIALS AND METHODS:' 168 mice were divided into 28 groups according to radiation dose and melatonin treatment. To analysis crypt survival, microcolony survival assay was done according to Withers an (l Elkind's method. To analysis apoptosis, TUNEL assay was done according to Labet-Moleur's method. RESULTS: Radiation protection effect of melatonin was demonstrated by crypt survival assay and its effect was stronger in high radiation dose area. Apoptosis index with 8 Gy irradiation was 18.4% in control group and 16.5% in melatonin treated group. After 18 Gy, apoptosis index was 17.2% in control group and 15.4% in melatonin treated group. Apoptosis index did not show statistically significant difference between melatonin treated group and control group. CONCLUSION: Melatonin shows clear protective effect in mouse jejunum against radiation damage but it.', protective effect seems not to be related with apoptosis protection effect.

4.A prospective study of totally implanted venous access system in 19 children with cancer.

A totally implanted venous access system was inserted in 19 children with cancer. The devices were utilized for the administration of antineoplastic drugs, parenteral fluids, antibiotics, and blood products. Total duration of implantation was 4,046 days for 23 implanted system (range 7-445 days). Complications included cather infection (0.247/100 catheter days), occlusion (0.692/100 catheter days), and dislodgement of needle (0.643/100 catheter days). There were major complications that necessitated removal of catheters, including systemic infections (0.09/100 catheter days) and complete occlusions (0.09/100 catheter days). The system was thought to be safe and convenient in chemotherapy, and permitted full physical activity.

Despite the modern advance in effective chemotherapy, gastrointestinal tuberculosis is considered to be relatively frequent in developing countries. The ileocecal region is the most common site of intestinal tuberculosis and duodenal involvement is rare. The isolated duodenal tuberculosis are reported 9 cases in Korea. The symptoms and signs of gastrointestinal tuberculosis are nonspecific and vague. In the absence of pulmonary tuberculosis, the diagnosis may be difficult. Pain and vomiting are common symptoms of duodenal tuberculosis. Patients may present with upper gastrointestinal bleeding. Therefore, tuberculosis should be considered in the differential diagnosis of gastrointestinal bleeding. We herein report a case of duodenal tuberculosis presenting as hematemesis and necessitating hospitalization. After anti-tuberculosis therapy, we have confirmed the healing of the lesion by the follow-up endoscopy, and review the current literature.

6.Two cases of conservative management of 10 and 8 week cervical pregnancies.

Cervical pregnancy is a rare and dangerous form of ectopic pregnancy in which the blastocyst implants within the cervical canal below the internal os of the uterine cervix. The characteristic clinical sign is a severe asymptomatic uterine bleeding in early pregnancy or during curettage. Most cervical pregnancies result in early spontaneous abortion, complicated by severe hemorrhage from the eroded blood vessels within the cervical tissue. Because of uncontrolled profuse vaginal bleeding, total hysterectomy has been the mostly necessitated to control life-threatening bleeding. Transvaginal sonography allows early diagnosis, and conservative treatment (two-time treatment : first treatement with systemic or intraamnionic methotrexate, secondly with curettage or ligature of the uterine arteries) improves the patient's fertility. Successful treatment is defined as elimination of the cervical pregnancy with preservation of the uterus. We report two cases of cervical pregnancy managed, successfully with methotrexate, leucovorin and curettage.

7.A Case of Respiratory Syncytial Virus Infection Presented with Apnea.

Respiratory syncytial virus(RSV) is currently implicated as the most common and otentially serious viral respiratory pathogen of young infants. Apnea in association with RSV infection occurred in about 18% of the infants. Premature birth and a young postnatal age were risk factors for development of apnea with RSV disease. We experienced a case of RSV infection in a 37 day-old premature girl. The presenting complaint was apnea, with respiratory signs and symptoms developing later. She required mechanical ventilation.

8.Preoperative Evaluation of Living Renal Transplant Donors: Comparison of Contrast Enhanced Magnetic Resonance Angiography and Conventional Renal Angiography.

PURPOSE: This study was designed to compare conventional renal and contrast enhanced magnetic resonance angiography (CRA & CE-MRA, respectively) for the preoperative evaluation of living renal transplant donors. MATERIALS AND METHODS: Between September 2002 and November 2004, forty-five consecutive live renal transplant donors underwent preoperative evaluations of the renal vessels, using both CRA and CE-MRA before the donor nephrectomy. The intraoperative findings of the number of renal vessels were compared with those of CRA and CE-MRA, respectively. RESULTS: Both angiographic modalities were able to detect multiple renal arteries and veins, with overall agreement between CRA and CE-MRA of 97.8 and 97.8%, respectively. The overall accuracies for predicting the number of renal arteries and veins, relative to the intraoperative findings, were 91.1 and 91.1% for CRA and 88.9 and 95.6% for CE-MRA, respectively. The CE-MRA missed 5 accessory renal arteries, whereas CRA missed 4 cases. Despite the undetected cases, there was no significant intraoperative morbidity. Additionally, renal cysts seen on CE-MRA were not detected by CRA. CONCLUSIONS: Our findings suggest that noninvasive CE-MRA is a promising substitute for CRA to evaluate the renal vessels of live renal transplant donors.

9.Surgical Decompression for the Impingement Syndrome of the Shoulder

The impingement syndrome has an anatomical character that occur against the anterior edge and the undersurface of anterior third of the acromion, coracoacromial ligament and acromioclavicular joint. The treatment consists of conservative and surgical things. We consider the surgical intervention only in cases fail to control pain by conservative treatment over 3 months(from Feb. 1990 to Jan. 1994). The author experienced 15 cases (14 pts) in positive impingement sign & test and evaluated by shoulder arthrography in all cases. 1. The mean age was 60 years ranged from 48 to 68 years and the average symptom duration was 23 months. 2. There were all positive impingement sign & test clinically. 3. There were partial tear of rotator cuff in 3 cases and complete tear in 12 cases radiologically. 4. The anterior acromioplasty with division of coracoacromial ligament was performed in all cases, additional rotator cuff repair in 3 cases and bicipital tenodesis in 2 cases. 5. At the follow up based on UCLA shoulder rating scale, results were excellent & good in 13(86%) and unsatisfactory in 2(14%) cases.
