3.Psychosis in Parkinson.

Current treatment strategies for levodopa-induced psychosis in advanced Parkinson's disease have had limited success. Reduction or discontinuation of levodopa and coadministration with dopamine-blocking neuroleptics may attenuate the psychotic symptoms, but these strategies are associated with worsening of parkinsonian symptoms. Administration of 5-HT3 receptor antagonist ; ondansetron, a newer strategy to attenuate psychosis of Parkinson' disease without motor deterioration was introduced. A 41-year-old young-onset male, who was diagnosed as Parkinson's disease 7 years ago, was treated with levodopa therapy, and had levodopa-induced psychosis(delusion, hallucination, paranoid, insomnia). After trial of ondansetron, he showed improvement in the Brief Psychiatric Rating Scale(from 21 points to 9 points) in spite of increasing the dosage of levodopa. With ondansetron, we could increase the dosage of levodopa without psychotic complications(esp, hallucination), and he showed improvement in the motor fluctuation.

4.A case of Ovarian Metastasis from Carcinoma of the Gallbladder: a rare Krukenberg Tumor.

A wide variety of cancers metastasize to the ovaries. In a majority of instances the primary site is the gastrointestinal tract, breast, or other gynecologic organs. The best known tumor of this type is signet-ring cell adenocarcinoma. The gallbladder and bile duct are rare sources of these metastases. The authors have had an experience of a case that was presented of Krukenberg tumor metastatic from the gallbladder and report the case with brief review of literature.

5.Operative Treatment in the Radial Head Fractures.

Sixteen patients who had radial head fractures were treated with open reduction and internal fixation using Herbert screws (10 patients), and with silastic prosthetic replacement (6 patients) from March, 1992 to December, 1994 in Chungnam National University hospital. The average duration of follow up was 19.5 months. The purpose of this study is to examine the role of Herbert screw fixation and silicone replacement arthroplasty and to asscess the clinical results of two groups functionally, radiologically in raidial head fractures. By functional rating index (modified After B.F. Morrey et al), the results were classified as excellent (9 patients), good (6 patients), fair (No patient) and poor (1 patient). Nine of the ten patients treated by open reduction and internal fixation using Herbert screws had resulted in excellent or good. All patients were inserted silastic implants were showed excellent or good results. Especially, in Mason Type III radial head fractures, three of four patients treated by Herbert screws and all 5 patients treated by silastic prosthesis were considered to excellent or good results. We concluded that Herbert screw fixation or silastic prosthetic replacement had satisfactorily appeared to be the alternative treatment option for Mason Type III radial head fractures.

6.Clinical Study of ankylosing Spondylitis

Ankylosing spondylitis is an inflammatory disorder of unknown etiology that primarily affect, the spine, the axial skeleton, and the large proximal joints of the body with striking tendancy toward fibrosis with secondary ossification and ankylosis of involved joints. This study was carried out to analyse clinical manifestation, evaluate disease activity indicator and set up diagnostic work-up. 38 cases of ankylosing spondylitis were studied at Department of Orthopedic Surgery, Seoul National University from Oct. 1987 to Mar. 1989. Following results were obtained. l. Average age was 28.5 years and all patients were male. 2. Low back pain was the most frequent symptom(63%) and back stiffness was next(13%). 3. S-I joint change on x-ray was the most frequently found abnormalities(100%) and HLA B27 (+) was next(97%). 4. SI/S ratio was inversely correlated with duration of symptom and seems to be significant durng the early phase of disease. 5. As a disease activity indicator, CRP was more reliable than ESR during follow up.

7.Surgical Treatment of Chondrosarcoma

Chondrosarocoma is a malignant, relatively slow-growing cartilaginous tumor that generally has a long natural history. From Feb. 1986 to Jan. 1993, 29 cases of chondrosarcoma were registered in the Department of Orthopedic Suergery of Korea Cancer Center Hospital. We reviewed 23 cases of chondrosarcoma which were treated surgically, aimed at curative procedure. Fourteen patients were male and 9 female. The average age at the time of diagnosis was 31.9(17-59) years. The pelvis was the most frequent site(7 patients) followed by femur(6), scapula(4), tibia(2), and each one case of spine, rib, humerus, and finger. The Enneking s surgical stage was II B (18 cases), II A (1), I B (3) or I A (1). The histological grades were as follows : 4 cases of grade 1; 3 grade 2 and 16 grade 3. Local resection with or without reconstruction were done for 20 cases and amputation or disarticulation 3. Chemotherapy was done in four cases and was not effective. The average follow-up period was 34.2(9-81) months. As complications, local recurrence occurred in 4(17.3%) cases, infection 2(8.7%), fracture 1(4.3%), lymphedema 1 and rediation necrosis of soft tissue 1, Distant metastasis occurred in 7(30.0%) cases: 6 cases to lung and 1 to brain. At final follow up 15 cases were continuous disease free, 3 alive with disease, 4 died of disease and 1 no evidence of disease. The Kaplan-Meier's estimated 5-year actuarial survival rate of total 23 cases was 66. 9%. Seven cases with histopathologic grade less than 2 showed 100% of 5 year actuarial survival rate, even one case is alive with disease at final follow up of 62 months. Fifteen cases of surgical stage II B with histopathologic grade 3 had 55.4% of 5 year actuarial survival rate: ten cases with wide surgical margin 87.5%o five cases with intralesional or marginal surgical margin 25%. Early surgical excision of chondrosarcoma with enough wide surgical margin can prevent local recurrence and metastasis.

8.Cerebral Arterial Embolism Treated by Intra-Arterial Infusion of Urokinase Which Was Occurred during Percutaneous Balloon Mitral Valvuloplasty.

Cerebral arterial embolism which resulted in total occlusion of right proximal middle cerebral artery was occured in 33 years old female patients with severe mitral stenosis and atrial fibrillation during percutaneous mitral balloon valvuloplasty(PMV). The occluded artery could be detected by transfemoral carotid angiography and recanalized immediately by intra-arterial infusion of urokinase. There were no cerebral hemorrhage by CT scan after intra-arterial urokinase infusion and the patients discharged without any neurologic sequale 7 days after thrombolysis. We report a case of successful recanalization with intraarterial infusion of urokinase in patient with cerebral arterial embolism occurred during PMV.

9.A Case of Becker's Nevus Associated with Smooth Muscle Hamartoma.

We report a case of Becker's nevus associated with smooth muscle hamartoma, in a 21-year-old male patient, which shows clinically match-head sized, flat topped, round to oval, grouped papules with hairs on the outer surface of the right arm, and microscopically reveals numerous bundles of smooth muscle fiber in the dermis.

10.Clinical Characteristics of the Poststroke Depression Patient.

BACKGROUND & OBJECTIVES: Depression is one of the most common neuropsychiatric sequelae of stroke, and depression is a serious disorder that can compromise the quality of life and overall rehabilitating process. The aim of this study was to investigate the frequency of depression in acute stroke patient and to compare with nondepressive patients in terms of demographic data, lesion site, pathology and the days of hospitalization. METHOD: Total 200 stroke patients admitted in Korea university hospital from Jan. 1993 to Dec. 1994 were included in this study. Data was collected from the medical records retrospectively and analyzed by means of statistics. RESULT: The frequency of depressive episode was 17% (34/200). The pathologic lesion was more frequently located in the right hemisphere (no statistical significance) and patients with depressive episode showed a higher frequency of lesions in anterior areas of the left hemisphere and posterior areas of the right hemisphere. The hospitalization days of depressive group was longer than that of non-depressive group (student t-test, p<.05). The age of depressive group was younger than that of non-depressive group (student t-test, p<.05). There was a positive correlation between the hospitalization days and the severity of depression(r=0.41). There also was a positive correlation between NIH stroke scale and the severity of depression (r=0.4988). CONCLUSION: In patients with depressive episode after acute stroke, it was more frequently developed in right posterior hemispheric lesion and their hospitalization days was longer in the depressive group than in nondepressive patients.
