1.Radiological evaluation of cortical rim sign of the kidney

The rim sign in renovascular compromise may be defined as a thin nephrographic rim outlinning a kidney with another wise faint nephrogram. Renal infarction with gelform and blood clots was induced experimentally in 5 dogsand high dose IVP and CT were performed to evaluate renal cortical rim sign after renal infarction. One expireddue to anesthetic technical failure, and one was embolized partially. So we could be obtain the result on 3 dogs.The reults were as follows; 1. The high dose IVP films show cortical rim sign on 24 hrs and 72 hrs films in twodogs. The CT films show rim sign in 3 dogs. 2. The rim sign may be visualized, if the renal artery is occuludedenough to drop perfusion presssure below the authoregulatory range. 3. The high dose IVP and early nephrogramphase are necessary for the rim sign.

2.The Application of Doppler Ultrasound in the Assessment of Fetal Weight.

OBJECTIVE: The objective of this study was to determine the relationship between the fetal doppler flow velocimetry and birth weight in low risk pregnancy population. METHODS: From December 1995 to May 1996, We prospectively performed doppler study in 254 uncomplicated, term pregnant women, who visited Pohang Hospital, Dongguk University. Using pulsed color doppler, we measured umbilical artery RI, middle cerebral artery RI and middle cerebral-umbilical artery RI ratio within one week before delivery. RESULTS: The result was that low birth weight group (below 2500gm) had very significant lationship with umbilical artery RI(P<0.01), middle cerebral artery RI(P<0.05) and middle cerebral-umbilical artery RI ratio(P<0.05), but there was no significant relationship in these blood flow indices between normal birth weight group (2501gm- 3999gm) and macrosomia group (above 4000gm). CONCLUSIONS: We concluded that application of doppler ultrasonopaphy in the assessment of fetal weight is somewhat helpful for identification of low birth weight, not for macrosomia.

4.Report of A Case of Renal Actinomycosis.

Actinomycosis, in which the principal causative agent in man is known to Actinomyces israelii, is a chronic, suppurative diseases characterized by extensive fibrosis, multiple abscesses, and formation of sinus tracts that drain suppurative exudates. On the basis of the anatomical sites involved; it can be subclassified into the cervicofacial form, which is the most common form, pulmonary form and abdominal form. Kidneys are rarely affected. Clinically, radiologically, and at operation it is difficult to differentiate the renal actinomycosis from renal tuberculosis and renal carcinoma. The prognosis is excellent after nephrectomy followed by appropriate antibiotic therapy. We presented a case of renal actinomycosis with a brief review of the literatures.

5.A Case of Complication of Expandible Metallic Stent with Endobronchial Stenosis.

To maintain an adequate airway in a patient with tracheobronchial narrowing coming from various causes, prosthetic tracheobronchial stents provide palliative treatment for narrowed airways where surgical resection is inadvisable. After insertion, precious reported complications were granuloma formation, dysphagia, suction catheter entrapment and fatal massive hemoptysis. We report a case of complication associated with expandible metallic scent with endobronchial stenosis due to tuberculosis.

7.Vascular Risk Factors in Normal Tension Glaucoma.

To evaluate the implication of vascular factor in normal tension glaucoma, authors performed color doppler imaging[CDI] for ophthalmic artery, central retinal artery and posterior ciliary artery in 21 eyes of 12 patients in normal tension glaucoma and 16 eyes of 11 patients in normal control group. We compared peak-systolic velocity[PSV], end-diastolic velocity[EDV], mean velocity[MV], resistance index[RI] and pulsatile index[PI] between the two groups. We evaluate the difference in parameters according to age, sex, systolic and diastolic blood pressure, past history of diabetes and hypertension, cup-disc ratio and visual field change. In normal tension glaucoma group, PSV, EDV and MV of ophthalmic artery and central retinal artery were significantly decreased and pulsatile index was significantly increased compared to those of normal group. Resistance index was also increased in normal tension glaucoma group, but statistically not significant. All parameters of posterior ciliary artery had no significant difference between the two groups. Age was related positively to pulsatile index and resistance index of central retianl artery and diastolic blood pressure was related positively to resistance index of ophthalmic artery. Cup-disc ratio was related negatively to PSV and EDV of ophthalmic artery. Above results imply that blood flow velocity decreased by increasing resistance factor in normal tension glaucoma, therefore local vascular factor might be related to the development of the normal tension glaucoma and affected by age, blood pressure and cup-disc ratio change.

8.Two Cases of Mayer-Rokitansky-Kster-Hauser Syndrome Managed by McIndoe Operation.

Mayer-Rokitansky-Kster-Hauser Syndrome is characterized by the absence of the vagina and uterus, the presence of apparently normal tubes and ovaries, feminine appeara-nce, normal female secondary sexual characteristics, a normal 46,XX karyotype, and a fem-inine psychosexual orientation. Various methods of surgical treatment have been introduced, but the ideal method is still not found. The two cases reviewed in this paper were all treated with McIndoe operation using full thickness skin graft. Both women were satisfied with vaginal depth and excellent result was achieved. We report two vaginoplasties in patients of Mayer-Rokitansky-Kster-Hauser Syndr- ome including a brief review of literature.

9.Comparision of Ocular pain Between Topical and Retrobulbar Anesthesia for Cataract Surgery.

We analysed a perceiving pain at each step (8 step: traction suture, wound incision, anterior capsulotomy, phacoemulsification, irrigation and aspiration, intraocular lens (IOL) implantation, wound suture, subtenon's injection) of cataract surgery by using visual analogue scale (0-10), prospectively. One hundred cases of cataract surgery were performed from November 1995 to March 1996 at Chonnam national university hospital and topical or retrobulbar anesthesia prior to cataract surgery was applied to each 50 patients. There was no difference of age, sex, running time for surgery, and incision method between topical and retrobulbar anesthesia. Pain scores at phacoemulsi fication. irrigation and aspiration, and subtenon's injection were higher than those at other steps in both types of anesthesia. However, a major range of pain scores at each step except of subtenon's injection was mild (1-3). In topical anesthesia, pain score was higher than retrobulbar anesthesia, but there was no statistically significant difference except a step of subtenon's injection. In conclusion, topical anesthesia might be an adequate anesthetic method for cataract surgery.

10.Ki - 67 Expression in Cutaneous Lymphoid Infiltrates.

BACKGROUND: A number of reports have stressed the usefulness of cell kinetic estimations as an aid in the prognostic and diagnostic classification of malignant lymphoma. But very limited information is available concerning cell cycle distirbutions in cutaneous lymphoid infiltrate, although a number of these conditions present tremendous problems in diagnosis and management. OBJECTIVE: The purpose of this study is to evaluate the proliferative activities in various cutaneous lymphoid infiltrates and its usefulness in diagnosis or foretelling a prognosis. METHODS: Fifty two arehival biopsy specimens were studies. These included cases of peripheral T-cell lymphoma, lichen planus, lupus erythematosus, polymorphous light eruptions, and pseudolymphomas such as insect bite, lymphomatoid papulosis, and other lymphocytoma cutis. Ki-67 was identified by immunoperoxidase technique using MIB1, a monoclonal antibody that recognizes a fixation and embedding resistant epitope on the Ki-67 protein. RESULTS: Higher Ki-67 positive rates were observed in high grade angiocentric T-cell lymphomas, which was helpful in differentiating from benign disease. In two cases of lymphomas which Ki-67 positive rate were more than 80 percent despite of minimal lymphoid infiltrate in skin biopsy specimen, evaluation of Ki-67 expression was profoundly helpful in diagnosing them as malignancies. CONCLUSION: Use of this technique was useful in evaluation of proliferative activity of cutaneous lymphoid infiltrate. In some cases, use of this technique may aid us in differential diagnosis of malignant lymphoma from benign chronic inflammatory disease of the skin.
