4.Pancreatic islet-cell adenoma.

Pancreatic islet cell adenoma is a benign tumor of pancreatic beta-cell and a rare cause of hyperinsulinemic hypoglycemia in children. The authors experienced a case of pancreatic islet cell adenoma(Insulinoma)in a 11 year and 8 month old male who had frequent loss of consciousness and seizure. Enucleation was done after localization of tumor by selective celiac artery angiography and abdominal computed tomography. Diagnosis was confirmed by histologic findings as pancreatic-adenoma, gyriform growth pattern. A brief review of related literature was made.

5.Solitary Nevus Lipomatosus Cutaneous Superficialis on the Forehead Presenting as a Soft Edematous Plaque.

Nevus lipomatosus cutaneous superficialis(NLCS) is a rare nevoid anomaly characterized by ectopic adipose tissue in the dermis. In this disease, two clinical types are distinguished: the classical type consisting of grouped papules and nodules, which are distributed linearly over the buttocks or upper thighs, and solitary type consisting of solitary pedunculated or dome-shape nodule without predilection. The solitary type is much rarer than the classical type. Unlike the classical type, the solitary type, especially on the forehead, is often mistaken for another type of benign tumor such as a lipoma. The author experienced a case of solitary NLCS, a 56-year-old female who had flesh, soft and edematous plaque on the forehead with soft surface during 13 months. Histological examination showed ectopic mature adipocytes in the dermis. The lesion was treated by surgical excision.

6.A Case of Apocrine Poroma with Follicular and Sebaceous Differentiation.

A poroma is usually described as a benign neoplasm arising from ductal epithelium of eccrine gland. However, in recent years there have been several reports on poromas featuring with combined sebaceous, follicular, and ductal differentiation, and thus establishment of this separate disease entity, "apocrine poroma" may well be justified considering the common embryologic origin of folliculosebaceous-apocrine unit. In our case, the tumor presented as a dark red nodule on the pubic area, being revealed as poroid neoplasm with folliculosebaceous components embedded in the lobules. Immunohistochemically, poroid cells stained with lysozyme. Taking the immunohistochemical staining results as well as the histopathologic findings of folliculosebaceous differentiation into consideration, we have concluded that this tumor is a case of an "apocrine poroma".

7.Appendiceal Mucocele with Lower Gastrointestinal Bleeding.

The appendiceal mucocele is very rare disease of 0.2% incidence. About 24% of patients are asymptomatic and symptomatic patients present with pain in the right lower quadrant of abdomen in 64%, plapable maas in the right lower quadrant of abdomen in 50%, and rarely, melena, hematochezia, anemia, diarrhea, malaise, and abdominal distension. The gastrointestinal bleeding may be presented in the patient with intussusception, but the massive bleeding is generally absent. We report a case of appendiceal mucocele accompanying with gastrointestinal bleeding and review of literature.

8.A Case of Cutaneous Metastasis from Pancreatic Adenocarcinoma.

Cutaneous metastases from internal carcinomas are relatively rare, especially from pancreatic carcinoma. Pancreatic carcinomas are usually adenocarcinomas which arise in the head of the gland, and are known to rapidly metastasize to the lymphatic system by permeation and embolization. We report a case of cutaneous metastasis from pancreatic adenocarcinoma. in 74-year-old male patient who have two pea sized, slight erythematous nodules on the lower abdomen and posterior side of the neck.

10.Effects of Brain Death on the Myocardium in Canine Brain Death Models.

BACKGROUND: Clinical and experimental studies have suggested that brain death may cause hemodynamic, electrocardiographic, functional or histopathologic changes of the heart. METHODS: Brain death was induced by increasing intracranial pressure(ICP) abruptly by intermittent bolus injection of saline(model ) or gradually by continuous infusion of saline(model ) to the epidural catheter in 5 mongrel dogs, respectively. Hemodynamic and biochemical changes during the process of brain death and histopathologic changes of the myocardium were analyzed and compared in two brain death models, and the association of apoptosis was also evaluated. RESULTS: 1) Two predominant subsets of acute contraction band lesion were produced in both brain death models : paradiscal and holocystic contraction band lesions. Both contraction band lesions were more prevalent in brain death model . 2) The frequency of both contraction band lesions was lowest in the epicardial layer and highest in the endocardial layer in both models, but no correlation was observed between the degree of contraction band lesions and ICP, LV maximum +dp/dt or catecholamine levels. There was no statistical difference between any of the LV circumferential blocks and either type of contraction band lesion, and transaxial distribution was not also different in both models. 3) There was no remarkable histopathologic changes in the analysis of major epicardial coronary arteries. Apoptotic cells were suggested in the scattered myocytes in the light microscopy and apoptosis was detected by in situ nick end labeling method. Electron microscopy revealed a condensation of nuclear chromatin and convolution of nuclear membrane in those myocytes. CONCLUSIONS: Myocardial changes due to brain were observed frequently, and few apoptotic cells were found in the brain death heart. Studies on the treatment strategy to minimize damages of myocardial structure and function caused by brain death should be followed in the near future.
