3.Pityriasis Rubra Pilaris in Down Syndrome.

A 23-year-old woman with Down syndrome presented erythematous keratotic plaques with whitish scales on the elbows, forearm, knee and leg. Histopathological findings revealed acanthosis with broad and short rete ridges, alternating orthokeratosis and parakeratosis oriented in both vertical and horizontal directions, and dermal superficial perivascular lymphocytic infiltration, compatible with pityriasis rubra pilaris.

4.Generalized Erythema Multiforme-like Eruption is the Novel Cutaneous Manifestation of Vibrio Vulnificus Septicemia.

Cutaneous manifestations of Vibrio vulnificus septicemia are bullae, vesicles, necrotic ulcers, localized swelling, cyanosis, and gangrene, which begin to occur mostly on the legs. The characteristic cutaneous lesions associated with pain often give an informative clue for the suspicion of Vibrio vulnificus infection in endemic area. We report a case of Vibrio vulnificus septicemia with atypical cutaneous manifestation of generalized erythema multiforme-like eruption in a Korean man, who died within 9 hours following hospitalization in spite of the intensive therapy. We suggest EM-like eruption of V. vulnificus septicemia is the novel cutaneous manifestation and may indicate poor prognosis.

6.Factors Affecting the Period of Bone Union When Treating Femoral Fractures with a Retrograde Intramedullary Nail

Purpose:
A retrograde intramedullary nail is commonly used to treat femoral shaft and distal femur fractures. The authors analyzed the treatment results and the factors affecting the period of bone union for thirty-five patients treated with a retrograde intramedullary nail.
Materials and Methods:
Thirty-five patients who could be followed up for more than twelve months after the treatment with retrograde intramedullary nailing were analyzed retrospectively. The treatment results and the relationship between the period of bone union and the patient’s age, sex, comminution of fracture, presence of open fracture, location of the fracture, and accompanying fractures were evaluated.
Results:
The average bone union time was 4.50 months. The period of bone union was unaffected by the age, sex, location, and presence of open fractures but was affected by the presence of comminution and accompanying fractures.
Conclusion
Retrograde intramedullary nailing is effective for distal and shaft of fractures of the femur. The period of bone union is affected by the presence of comminution and accompanying fractures.

8.Central Retinal Artery Occlusion After Carotid Artery Angioplasty and Stenting in an Elderly Patient: A Case Report.

Carotid artery angioplasty and stenting (CAS) has become increasingly accepted as an alternative therapy to carotid endarterectomy for treatment of carotid artery stenosis. Central retinal artery occlusion (CRAO) is one of the diseases presented due to carotid artery stenosis. But CRAO without cerebral ischemia after CAS is uncommon. An 80-year-old man was admitted to the hospital with the right centrum ovale ischemic stroke and right proximal carotid artery stenosis. We performed CAS with a distal protection device after pre-ballooning 3 times, without post-ballooning. Then, 12 hours after the CAS, the patient complained of blindness in the right eye and was diagnosed with CRAO. However, Diffusion weighted magnetic resonance imaging (DW-MRI) showed no significant findings in the brain. CRAO after CAS without intracranial infarction is a rare complication.

To evaluate the differences and factors to be considered when measuring intraocular pressure after photorefractive keratectomy[PRK]and laser in situ keratomileusis[LASIK], we prospectively reviewed the medical records of myopic eyes between -4~-7 diopters. Among them, 14 patients, 22 eyes underwent PRK and 10 patients, 18 eyes recieived LASIK. We measured intraocular pressures by Goldmann applanation tonometry preoperatively, 1 month, 3 months and 6 months postoperatively in each group. Corneal curvatures, central corneal thicknesses were measured and compared when measuring intraocular pressure. The postoperative intraocular pressure was lower than the preoperative value in both groups[p<0.01], and the decreased amount of IOP were not statistically different in both groups[p=0.29]. Targeted ablation depth[p=0.19]and ablation diameter[p=0.16]did not show statistically significant correlation to postoperative IOP decrease. In measuring intraocular pressure after PRK or LASIK, attention should be given to the pressure and its interpretation.
