OBJECTIVES: To evaluate the effect of parity & delivery method on female fecal incontinence. METHOD: Based on 213 cases of 4 different delivery method and 40 cases of one non-delivery group, a retrospective analysis of the prevalence of female fecal incontinence, between January 1996 and December 1998, was done. With delivery mode, 4 different groups were: first vaginal delivery group , two or more vaginal deliveries group, first cesarean section group, and first vaginal delivery followed by cesarean section group. All subjects were examined by questionaire or phone about the experience of fecal incontinence after delivery. In cases of fecal incontinence , we reviewed the obstetric complications with medical records. RESULT: There was no fecal incontinence in the non-delivery group. The incidence of fecal incontinence in the two or more vaginal delivery group(11.7%, 9/77) and the first vaginal delivery followed by cesarean section group(18.1%, 8/44) was higher than the first vaginal delivery group(6.8%, 3/44), and the first cesarean section group(2.1%, 1/48)(P<0.05). The obstetric complications in fecal incontinence cases were: a prolonged second stage, anal sphincter laceration, vacuum extraction, a large baby etc. CONCLUSION: Fecal incontinence was significantly correlated with the number of vaginal deliveries and obstetric complications during delivery. Fecal incontinence after the first vaginal delivery or vaginal delivery followed by cesarean section are very important factors in choosing the next delivery method.

BACKGROUND: During the past eight years, we have been performing partial autonomic nerve preservation surgery (PANP) for the treaolent of advanced rectal cancer in order to reduce the postoperative male sexual dysfunctions. METHODS: This is a prospective study of 25 rectal cancer patients who underwent PANP from 1990 to 1997. All patient were in Duke C stage and their ages were between 30 and 55. Patients were divided into four types: 1) Partial sacrifice of inferior mesenteric plexus (PSIP). 2) Partial sacrifice of preaortic nerve (PSPN). 3) Complete sacrifice of preaortic nerve with complete pelvic nerve preservation (CSPN). 4) Unilateral sacrifice of pelvic autonomic nerve (USPN). RESULTS: The percentage of ejaculation and erection difficulties at each surgery was as follows: PSIP 0/5 (0%) for ejaculation vs. 0/5 (0%) for erection, respectively, PSPN 5/7 (71.4%) vs. 2/7 (28.6%), CSPN 6/7 (85.7%) vs. 0/7 (0%) and USPN 6/6 (100%) vs. 5/6 (83.3%). Summary: PANP is possible for the treatment of advanced rectal cancer.

Authors observed one case of subcutaneous dermoid cyst which did not seem to have been described in the Korean literature. A 11 year old female patient complained of a asymptomatic mass near the left sternoclavicular joint in January 1978. The mass was discovered at the age of 5 when that was pea-sized and had been slowly growing. In January 1978, the mass was palpable as a peanut-sized subcutaneous solitary round mass, and overlying skin was freely movable and slightly elevated. There was no trauma history and family history was negative except her elder sister had a, axillary accessory breast. Histopathologic studies revealed a keratinizing stratified squamous epithelium lining the cystic wall, and sebaceous glands, hair follicle and eccrine glands in the surrounding tissues, and keratinous materials in the cyst. There was no evidence of malignancy. Diagnosis was confirmed by clinical features and histopathological findings, and surgical excision of tumor mass was performed.

A survey was conducted to study perception and attitudes of examinees of, health screening center towards items, cost and procedures of health examination and degree of satisfaction with health examination, from March 2 to May 31, 1995. The study population was 468 examinees received health examination at Health Screening & Diagnosis Center of Yeungnam University Hospital. A questionnaire method was used to collect data. The followings are summaries of findings Examinees were predominantly male(77.4%) ; had college education(68.6%), and held white-collar workers (57.8%). The effects of testosterone on the pituitary growth hormone (GH) response directly and to hypothalamic growth hormone-releasing hormone (GHRH) were evaluated in vitro using a male pituitary cell monolayer culture system. Wistar male rats were gonadectomized at 22 days of age, and 21 days later their anterior pituitaries were removed and trypsinized for cell dispersion. Testosterone 0, 0.1, 1.0, 10.0 nM was added to the medium for 1 day and GH amounts in media were measured. In another experiment, testosterone 1, 0.1, 1.0, 5.0, 10,0 nM was added to the medioum for 3 days, and subsequently 5 nM GHRH was added for 1 day, thereafter GH amounts in media were measured. The results were as follows: 1) The increase of GH response after testosterone administration to the cultured rat pituitary cell was not significant. 2) The rat pituitary cell response to GHRH was augmented after pretreatment with testosterone. These results are suggested that testosterone has no direct effect on GH secretion, but by increasing the pituitary cell response to GHRH, contributes to the regulation of GH secretion in vitro.

Serologic tests for syphilis, including the qualitative and quantitative VDRL test and TPHA test, were carried out on 18, 151 healthy young men, who took a medical check-up for the overseas employment at Kyunghee University Hospital from April, 1978 to February, 1979. The syphilitics who revealed TPHA reactive, were given a questionare that dealt with three items. The results are summarized as follows. 1) The reactive rate of VDRL test was 3. 1% among 18, 151 healthy young men, 2) The biologic false positive rate of VDRL test was 7. 7% among the 520 men in whom TPHA test was carried out, using TPHA as standard. 3) The reactive rate of TPHA test was 2. 7% among 18, 109 men. On 42 men the TPHA test was not carried out. 4) With regard to the VDRL quantitative test, the reactive rate of the group with a titration of 1: 4 or lower was 85,2% out of 480 syphilitics. 5) In 480 syphilitics, 45. 8% (220) had a history of venereal 72.3%(347) had no general knowledge of syphilis, and 86. 2% (414) was unaware of his syphilitic infection.

Uterine didelphys with unilateral hematometra, hemicolpos, and ipsilateral renal agenesis is a rare m01lerian duct malformation. Uterine didelphys with an obstructed hemivagina is frequently associated with renal agenesis, mostly ipsilateral to the blind vaginal pouch. Accurate early diagnosis and septal resection elevate pregnancy rate and birth rate but it is difficult to find these malfomations until the complication associated with pregnancy was developed. We report a case of successful gestational outcome in the presence of preterm labor due to hematometrocolpora caused by uterine didelphys with unilateral imperforated vagina and ipsilateral renal agenesis with brief review of the literatures.

We studied the expressions of E-cadherin, MMP-7 and CD44V in colorectal cancer and their corresponding normal mucosa using RT-PCR. From the 26 patients whom a com parative study of clinical and histopathological data is available, MMP-7 and CD44V were significantly enhanced in cancer and their metastatic tissues, compared with their normal mucosa. E-cadherin did not reveal any difference between cancer and normal mucosa. The relashionship between these genes and colorectal cancer development can not be confirmed by this study, however CD44v and MMP-7 may be associated with metastasis of colorectal cancer.
