2.A Case of Perforating Granuloma Annulare.

We present a case of perforating granuloma annulare which appeared as multiple small papular lesions with central umbilication on the dorsum of both foot and sides of foot, The patient was a 12 year-old female child who had slightly tender lesions of one month's duration. Histologic study showed perforation of the epiderrnis and release of the abnormal necrobiotic material transepiderrnally. The epidermal perforation communicated with area of granuloma annulare in the dermis that consist of necrobiotic collagen surrounded by palisading mononucler phagocytes in the dermis. All the lesions resolved spontaneously with a slight hyperpigmentation in 4 months. She also had erythematous tender nodules, which diagnosed as erythema nodosum, on both shins. The lesions responded to potassium iodide 1.0g for 2 weeks.

3.Immunohistochemical Staining in Metastatic Skin Cancer.

Immunohistochemial staining was performed in metastatic skin cancer using a variety of antistera. Both carcinoembrytonic antigen(CEA) and epithelial membrane antigen(EMA) were demonstrated in large cell carcinoma of the lung, adenocarcinoma of the stomach and skin adnexal cancer with eccrine differentiation. Although papillary carcinoma of the thyroid, squamous cell carcinoma of the skin and squamous cell carcinoma of the lung showed positive staining for CEA and EMA, these were weak or focal reactions in the keratinizing area. In rectal adenocarcinoma, staining for CEA was significantly positive, but staining for EMA was negative. Epiderrnal cytokeratin was observed in squamous cell carcinoma of the lung, squamous cell carcinoma of the skin and skin adnexal cancer with eccrine differentiation. Epidermal cytokeratin was also demonstrated in well differentiated adenocarcinoma of the gastrointestinal tract. By using a variety of tumor markers, the authors tried to locate possible primary sites in six metastatic skin cancers of unknown origin.

4.Comparison of Immunological Methods for Diagnosis of Invasive Candidiasis.

BACKGROUND: The early diagnosis of invasive candidiasis is the most important for reducing of morbidity and mortality rates in the immunocompromised patients. The study of antigen detection was performed by Cand-Tec kit, but antibody detection by Western blot, had not been reported. OBJECTIVE: We reviewed 62 cases of suspected invasive candidiasis(10), immunocompromised patients with leukemia, leukopenia, pneumonia, et al(44) and normal colonization control(8) in the urine and sputum for the immunological diagnosis of invasive candidiasis by Cand-Tec and Western blot assays. METHODS: The antigen and antibody detection were done by both Cand-Tec kit and Western blot assay, in the sera of the patients collected from several hospitals. RESULTS: The sera from 4(40%) and 8(80%) of 10 suspected invasive candidiasis had a threshold positive titers of > or = 1:4 by latex agglutination (Cand-Tec) and the results of positive antibody to the immunodominant antigen (47KDa) of Candida spp. by Western blot assay, each other. Both antigen and antibody we.e detected from 21(47%) of 44 patients in the immunocompromised group and 1(12%) of 8 cases with normal flora, separately, but the antibodies from 3(18%) of 16 healthy controls were detected by Western blot analysis, only. CONCLUSION: This study confirm that the Western blot method of detecting antibody against immunodominant antigen(47KDa) of Candida spp. was more effective than antigen detection by Cand-Tec method for immunodiagnosis of invasive candidiasis.

5.Longitudinal Change in Trabecular Bone Score during and after Treatment of Osteoporosis in Postmenopausal Korean Women.

BACKGROUND: The aim of this study was to evaluate the longitudinal changes of trabecular bone score (TBS) during and after bisphosphonate (BP) treatment in postmenopausal Korean women with osteoporosis. METHODS: We analyzed 191 patients who took BP and underwent bone mineral density (BMD) test for the period from January 2010 to December 2015. The mean follow up period during treatment and after treatment was 22.8 months and 18 months, respectively. The TBS and BMD values were evaluated by the percent changes relative to the baseline. RESULTS: In 191 patients, who treated with BPs, L-spine BMD increased 3.65±0.5% and TBS increased 0.26±0.4% from baseline during first 1 year. At 2 to 4 years, the changes of BMD and TBS from baseline gradually increased up to 9.3±3.25% and 2.69±0.98% and both results showed statistically significant correlation. In 86 patients who stopped BPs, L-spine BMD decreased -0.54±1.07% and TBS increased 0.33±1.96% from baseline during 3 years follow up period. CONCLUSIONS: Lumbar spine TBS increase over time with BPs treatment although the changes were less than that of BMD. Also, it preserve for years after stopping treatment, as the changes of lumbar spine BMD. The results of BMD and TBS showed significant correlation during treatment but not during drug withdrawal.

6.Transverse uterine incision closure: One versus Two layers.

OBJECTIVE: Closure of a low transverse cesarean incision with one layer and two layer, we compared operative time, hemostasis, blood loss and postoperative sonohysteroperitoneographic findings. Study design: At our hospital 103 women were randomized to closure of a low transverse cesarean incision with either one continuous layer of a locking No. 1 chromic suture or two continuous of No. 1 chromic suture with the first layer locked between Mar. 1, 1998 and Dec. 31, 1998. After three months later, sonhysteroperitoneography was taken and then we evaluated uterus indirectly. RESULTS: A one layer closure required less operative time, 16 versus 20.8 minutes (p<0.01), less hemoglobin change, 0.44 versus 1.3 (p<0.01) and less suture materials (p<0.01). But postoperative follow up sonohysteroperitoneography, a one layer closure shows slightly thinning of isthmic layer of myometrium, two layer closure shows more adhesional band and both are similar other findings. CONCLUSION: A one layer does not significantly affect the clinical course than traditional two layer closure. So we recommended a one layer closure when its use is anatomically feasible.

7.A Case of Urinary Incontinence Treated with AMS 800 Artificial Sphincter.

A 31 year old male patient presenting urinary incontinence 2 years after the traumatic injury of the posterior urethra and bladder neck, fails to respond to the alpha-adrenaergic agents and other ordinary methods. However, using the artificial sphincter(Model AMS 800) implantation, he successfully voided freely without urinary incontinence and the residual urine. The urethral closing pressure was significantly increased. The radiographic findings of inflation and deflation of the sphincter revealed satisfactory functions.

9.Self-Reported Versus Caregiver-Reported Quality of Life in Community-Dwelling Elderly with Cognitive Impairment

Objectives:
:The purpose of this study was to investigate the differences between elderly with cognitive impairment and caregiver ratings on quality of life in the participants. Furthermore, we assessed the relationship between elderly’s quality of life and cognitive function, activities of daily living, and depression.
Methods:
:Seventy community-dwelling elderly with cognitive impairment and their caregivers participated. Each participants assessed elderly’s quality of life using the Korean version of Quality of Life-Alzheimer’s Disease (KQOL-AD) scale. The Korean Mini Mental Status Exam in the Korean version of the Consortium to Establish a Registry for Alzheimer’s Disease (CERAD) assessment packet (MMSE-KC), Seoul-instrumental activities of daily living (S-IADL) and Short form of the Geriatric Depression Scale (SGDS) were also evaluated.
Results:
:There were significant correlations between elderly and caregiver reports on all quality of life items and total KQOL-AD scores. There were no significant differences between them on all QOL items, except marriage.Total KQOL-AD Scores of self-reported quality of life were significantly correlated with SGDS scores (r=-0.623). Total KQOLAD scores of caregiver-reported quality of life were significantly correlated with MMSE-KC, S-IADL, and SGDS scores (r=0.239, r=-0.539, r=-0.384, respectively).
Conclusions
:Both self-reported quality of life and caregiver-reported quality of life in elderly with cognitive impairment were negatively associated with depression. Therefore, appropriated psychiatric intervention of depression in the elderly is essential to increase quality of life.
10.Finite Element Analysis of Stress Distribution and Range of Motion in Discogenic Back Pain

Objective:
Precise knowledge regarding the mechanical stress applied to the intervertebral disc following each individual spine motion enables physicians and patients to understand how people with discogenic back pain should be guided in their exercises and which spine motions to specifically avoid. We created an intervertebral disc degeneration model and conducted a finite element (FE) analysis of loaded stresses following each spinal posture or motion.
Methods:
A 3-dimensional FE model of intervertebral disc degeneration at L4–5 was constructed. The intervertebral disc degeneration model was created according to the modified Dallas discogram scale. The von Mises stress and range of motion (ROM) regarding the intervertebral discs and the endplates were analyzed.
Results:
We observed that mechanical stresses loaded onto the intervertebral discs were similar during flexion, extension, and lateral bending, which were greater than those occurring during torsion. Based on the comparison among the grades divided by the modified Dallas discogram scale, the mechanical stress during extension was greater in grades 3–5 than it was during the others. During extension, the mechanical stress loaded onto the intervertebral disc and endplate was greatest in the posterior portion. Mechanical stresses loaded onto the intervertebral disc were greater in grades 3–5 compared to those in grades 0–2.
Conclusion
Our findings suggest that it might be beneficial for patients experiencing discogenic back pain to maintain a neutral posture in their lumbar spine when engaging in daily activities and exercises, especially those suffering from significant intravertebral disc degeneration.
