1.A Case of Senils Sebaceous Hyperplasia.

Senile sebaceous hyperplasia refers to a benign enlargement of a normal sebaceous unit which is not uncommon in the old age group, and is characterized clinically by single or multiple small cream to yellowish round papules developed on the face, chiefly on the forehead and cheeks. A case of florid senile sebaceous hyperplasia developed in a 81-year-old man was presented. Because this disorder is one of the geriatric dermatoses and the average life span of our people is remarkably lengthened in recent years, we believe dermatologists should pay more attention to this kind of dermatosis.

2.Lectins Binding in Squamous Cell Carcinoma of the Uterine Cervix as a Diagnostic and Prognostic Marker.

OBJECTIVE: The aim of the study was to evaluate the corelation between an expression of lectins and survival of patients with squamous cell carcinoma of uterine cervix. METHODS: The cell surface carbohydrate profile of formalin-fixed paraffin embedded tissue section of squamous cell carcinoma of the uterine cervix was evaluated using ulex europaeus agglutinin(UEA-1), peanut agglutinin(PNA), dolichos biflorus agglutinin(DBA), soybean agglutinin(SBA) and lotus tetragonobus lectin(LTL) by the avidin-biotin complex method. Fifty-one cases of squamous cell carcinoma and 10 cases of normal squamous epithelium of the uterine cervix were selected from the file which were treated during Jan. 89 to 31 Dec. 1992 in Department of Obstetrics and Gynecology, Korea University Hospital. RESULTS: UEA-1 and LTL were negative in normal cervix while positive in 76.5%, 47.1% of squamous cell carcinoma respectively, and useful markers for differential diagnosis between normal and squamous cell carcinoma. SBA and LTL were useful for differential diagnosis of keratinizing and non-keratinizing squamous cell carcinoma. UEA-1 may play an important role in lymphovascular invasion of squamous cell carcinoma of the uterine cervix. CONCLUSIONS: There was no correlation among clinical staging, patient's survival and lectins binding in squamous cell carcinoma.

3.Pemphigus Vulgaris Developed During Pregnancy.

We present a case of pemphigus vulgaris developed during pregnancy. The clinical features of this patient were similar to those of hetpes gestationis. By immunofluorescence studies we found that the patient's serum contained autoantibodies to intercellular substance of epidermis. Direct immunofluorescence microscopy of a perilesional skin also demontrated immunoglobulin deposition in the intercellular areas. Additionally we did in vitro C3 staining for further confirmation of the diagnosis but it was negative in basement membrane zone. This patient was initially treated with corticosteroid and azathioprine. Several months later gold compound was given instead of azathioprine and corticosteroid with good therapeutic response.

4.A Clinical comparison between One Plane Unilateral and One Plane bilateral Frame of External Fixation in the Treatment of Tibial Open Fractures

External fixation was improved in its material, design and techniques as a specific method of open fracture treatment. Seventy-three, tibial open fracture, patients were treated with external fixator in Korea Veterans Hospital from January 1983 to April 1988. Each fixation method was divided into two types;one-plane unilateral frame and one-plane bilateral frame. 1. The duration of external fixation was 14.3 weeks in averge, 15.4 weeks in one plane unilateral frame and 13.2 weeks in one plane bilateral frame. 2. Secondary operation for bony union was performed 19 cases in one plane unilateral frame and 8 cases in one plane bilateral frame. 3. The union time was 23.6 weeks in average, 26.8 weeks in one plane unilateral frame and 21.4 weeks in one plane bilateral frame. 4. The common complications were delayed union and nonunion (7 cases) and pin tract infection (5 cases) in one plane unilateral frame, and pin tract infection ( 9 cases), joint stiffness (4 cases) and delayed union and nonunion (3 cases) in one plane bilateral frame. 5. External fixation gave rigid fixation and easy wound access, but did not give excellent bony union. 6. One plane bilateral frame showed more or less better result than one plane unilateral frame except some complications.

5.Risk of chronic periodontitis in patients with obstructive sleep apnea in Korea: a nationwide retrospective cohort study

METHODS:
A nationwide, population-based, retrospective cohort study was conducted using patient records from the Korean National Health Insurance Service database. For the period 2004-2019, patient data were categorized into 2 groups: a diagnosis of OSA (747 subjects) and no diagnosis of OSA (1,494 subjects). Subsequently, 1:2 propensity score matching was performed to ensure the homogeneity of the 2 groups. To analyze the risk of incident chronic periodontitis, a Cox proportional-hazards model was used to calculate hazard ratios (HRs) with 95% confidence intervals (CIs).
RESULTS:
In the Kaplan-Meier curve, the disease-free probability was significantly lower in the OSA group than in the non-OSA group (p for log-rank test=0.001). The crude HR for the association between OSA and chronic periodontitis was 1.29 (95% CI, 1.16 to 1.43). The multivariable-adjusted HR was calculated at 1.28 (95% CI, 1.15 to 1.42).
CONCLUSIONS
This study confirmed a relationship between OSA and chronic periodontitis. Therefore, OSA patients require oral care to prevent the progression of chronic periodontitis from mild to severe.

7.Withaferin A-Caused Production of Intracellular Reactive Oxygen Species Modulates Apoptosis via PI3K/Akt and JNKinase in Rabbit Articular Chondrocytes.

Withaferin A (WFA) is known as a constituent of Ayurvedic medicinal plant, Withania somnifera, and has been used for thousands of years. Although WFA has been used for the treatment of osteoarthritis (OA) and has a wide range of biochemical and pharmacologic activities, there are no findings suggesting its properties on chondrocytes or cartilage. The aim of the present study is to investigate the effects of WFA on apoptosis with focus on generation of intracellular reactive oxygen species (ROS). Here we showed that WFA significantly increased the generation of intracellular ROS in a dose-dependent manner. We also determined that WFA markedly leads to apoptosis as evidenced by accumulation of p53 by Western blot analysis. N-Acetyl-L-Cystein (NAC), an antioxidant, prevented WFA-caused expression of p53 and inhibited apoptosis of chondrocytes. We also found that WFA causes the activation of PI3K/Akt and JNKinase. Inhibition of PI3K/Akt and JNKinase with LY294002 (LY)/triciribine (TB) or SP600125 (SP) in WFA-treated cells reduced accumulation of p53 and inhibited fragmented DNA. Our findings suggested that apoptosis caused by WFA-induced intracellular ROS generation is regulated through PI3K/Akt and JNKinase in rabbit articular chondrocytes.

8.Factors Affecting the Practice of Accident Prevention Behavior in Middle-school Students

PURPOSE: This study aimed to identify the degree of accident prevention behavior, self-esteem, self-regulation ability, and social support among middle-school students, and investigate factors influencing their practice of accident prevention behaviors.METHODS: The participants were 174 students from two middle schools located in J city. Data were collected from December 28, 2017 to February 9, 2018 using self-reported questionnaires.RESULTS: The mean score were 3.08±0.37 of 4 for practice of accident prevention behaviors, 3.07±0.57 of 4 for self-esteem, 3.69±0.48 of 5 for self-regulation ability, and 3.38±0.75 of 5 for social support. Positive correlations were found among self-regulation ability, self-esteem, social support, and practice of accident prevention behaviors. The factors influencing the practice of accident prevention behaviors were personality (being ‘haste’) (β=−.20, p=.003), self- regulation ability (β=.48, p<.001), and social support (β=.23, p<.001). These factors explained 39.0% of the variance in practice of accident prevention behaviors.CONCLUSION: It was found that the middle-school students engaged in prevention behavior to an extent that was insufficient to prevent accidents. Therefore, it is necessary to develop intervention strategies to improve the practice of accident prevention behavior among middle-school students, considering self-regulation ability, social support, and personality, which affect the practice of accident prevention behaviors.

9.p38 Kinase Regulates Nitric Oxide-induced Dedifferentiation and Cyclooxygenase-2 Expression of Articular Chondrocytes.

BACKGROUND: Caveolin, a family of integral membrane proteins are a principal component of caveolae membranes. In this study, we investigated the effect of p38 kinase on differentiation and on inflammatory responses in sodium nitroprusside (SNP)- treated chondrocytes. METHODS: Rabbit articular chondrocytes were prepared from cartilage slices of 2-week-old New Zealand white rabbits by enzymatic digestion. SNP was used as a nitric oxide (NO) donor. In this experiments measuring SNP dose response, primary chondrocytes were treated with various concentrations of SNP for 24 h. The time course of the SNP response was determined by incubating cells with 1 mM SNP for the indicated time period (0~24 h). The cyclooxygenase-2 (COX-2) and type II collagen expression levels were determined by immunoblot analysis, and prostaglandin E2 (PGE2) assay was used to measure the COX-2 activity. The tyrosine phosphorylation of caveolin-1 was determined by immunoblot analysis and immunostaining. RESULTS: SNP treatment stimulated tyrosine phosphorylation of caveolin-1 and activation of p38 kinase. SNP additionally caused dedifferentiation and inflammatory response. We showed previously that SNP treatment stimulated activation of p38 kinase and ERK-1/-2. Inhibition of p38 kinase with SB203580 reduced caveolin-1 tyrosine phosphorylation and COX-2 expression but enhanced dedifferentiation, whereas inhibition of ERK with PD98059 did not affect caveolin-1 tyrosine phosphorylation levels, suggesting that ERK at least is not related to dedifferentiation and COX-2 expression through caveolin-1 tyrosine phosphorylation. CONCLUSION: Our results indicate that SNP in articular chondrocytes stimulates dedifferentiation and inflammatory response via p38 kinase signaling in association with caveolin-1 phosphorylation.

10.Nocturnal Arterial Oxygen Saturation Monitoring in Patients with Respiratory Disease.

To find out the predictors of nocturnal arterial oxygen desaturation in patients with respiratory diseases, transcutaneous oxygen saturation(StcO2) monitoring studies using a pulse oximeter were performed during sleep in 20 patients. StcO2 was decreased more than 4% from the baseline value in 18 patients(90%) and more than 10%('Desaturator') in 8(40%). Five of the seven patients(71.4%) with awake PaO2<60mmHg and three of the thirteen patients(23.1%) with awake PaO2≥60mmHg were 'desaturators'. The awake PaO2/FIO2 and PaO2/PAO2 could distinguish 'desaturator' from 'nondesaturator, and PaO2, SaO2 or StcO2 could not. These results suggest that the nocturnal oxygen desaturation depends on the severity of the underlying disease rather than the baseline PaO2. Anthropomorphic and lung function factors could not separate between 'desaturator' and 'non-desaturator', and about a quarter of patients with a wake PaO2≥60mmHg developed significant desaturation. Therefore, it is necessary to monitor the nocturnal arterial oxygen saturation in patients with respiratory diseases regardless of their severity of airflow obstruction or awake PaO2.
