1.Estrogen and Progesterone Receptor Expressions in Benign Prostatic Hypertrophy and Prostatic Adenocarcinoma.

The effect of androgen in the development of the normal prostate and the evolution of benign prostatic hypertrophy (BPH), and prostatic adenocarcinoma has been proven. In addition to androgen, estrogen and progesterone are also thought to play a role in the pathogenesis of BPH and carcinoma. However, their exact roles are not yet known because there is no conclusive evidence. Thirty cases of prostatic adenocarcinoma and 16 cases of BPH were studied. Immunohistochemical staining for estrogen receptor (ER) and progesterone receptor (PR) in epithelial and stromal cells, respectively was performed and the results were assessed semiquantitatively based on the number of positive cells per 100 total cells. Slides were scored as negative; less than 5% of cells, 1 ; 6~15% of cells, 2 ; 16~25% of cells, and 3 ; more than 26% of cells. The relationship between ER and PR expression and the patient's age, histologic grade, and clinical stage was evaluated in prostatic adenocarcinomas. ER was negative in epithelial and in stromal cells for all prostatic adenocarcinomas and BPH cases. The PR expression in epithelial cells and in stromal cells of BPH was noted in 15 (93.8%) and 16 (100.0%) out of 16, respectively. The PR expression of carcinoma cells and stromal cells in prostatic adenocarcinoma was found in 28 (93.3%) and 23 out of 30 (76.7%), respectively. The PR immunoreactivities of stromal cells around carcinoma were 3 in 18 cases, 2 in one case, and 1 in 4 cases, but those of epithelial and stromal cells of BPH and carcinoma cells of prostatic carcinoma were similar to each other with a value of 3 in most cases. The PR expression rate of stromal cells around carcinoma was significantly correlated with the patient's age (p=0.044), but not with histologic grade and clinical stage. In summary, estrogen does not have a direct effect on the biological behavior of BPH and prostatic adenocarcinoma, but progesterone appears to play a role in the pathogenesis of BPH and prostatic adenocarcinoma. Further studies should clarify the biological role of progesterone in the human prostate.

3.Mucous Gland Adenoma of the Bronchus; Light Microscopic and Ultrastructural Features.

We present a case of mucous gland adenoma arising from the main bronchus of the lower lobe of the left lung in terms of clinicopathologic, immunohistochemical, and ultrastructural aspects as well as review of related literatures. The patient, a 31-year-old female, was admitted to Catholic University Medical College Hospital with complaints of coughing and purulent sputum for about seven years. The chest CT showed a severely calcified tumor in the left lower lobe of the lung. Grossly, the calcified tumor arising from the main bronchus protruded into the lumen and showed bronchiectasis of the lower lobe and atelectasis of the upper lobe of the lung. The tumor was pale brown-gray and sharply circumscribed and showed some small cystic spaces filled with mucoid material. Microscopically, most of the tumor showed dystrophic calcification. The growth pattern of the tumor is composed of cysts, tubules, and glands lined by cytologically bland columnar, cuboidal, or flattened mucus secreting cells. Electron micrograph of tumor cells showed some round or oval mucous granules measuring 0.5-1.8 micrometer.

4.Malignant Fibrous Histiocytoma of the Heart: A case report.

A case of a 58-year-old man with malignant fibrous histiocytoma (MFH) of the right ventricle is reported light and electron microscopically. This is the first case of MFH of the heart in the Korean literature. A tendency for malignant fibrous histiocytoma of the heart to occur in the left atrium of young women is suggested; this sarcoma's usual location is in the soft tissue of elderly men. The tumor consisted of spindle cells arranged in a focal storiform patterns, clusters or sheets of histocyte-like cells, benign and malignant giant cells, inflammatory cells, scattered mitotic figures and anaplasia of stromal cells. In ultrastructure the constituent of cells of the tumor are primitive mesenchymal cells, histiocytoid cells and fibroblast like cells including giant cells.

5.A Clinical Alnalysis of Arthorodesis of the Knee Joint

In the past, arthrodesjs was a widely used procedure but in recent times, with the development of replacement arthroplasty, decreased in tuberculosis and polio. etc, it is not commonly used by orthopedic surgeons. However, for those in the young age group, people employed in hard labor and patients with acute inflammatory disease, arthrodesis is through to be more appropriate than replacement arthroplasty. From January 1980 to December 1985, 20 cases of arthrodesis of the knee joint were carried out. The results were follows.; 1. Methods of arthrodesis used were Charnley compression arthrodesis in 10 cases, Hoffman compression arthrodesis in 2 cases, cross pinning in 3 cases and and cross pinning with patella graft in 5 cases. 2. Bone union was seen in all 12 cases of compression arthrodesis at an average of 21.3 weeks while bone union was seen in 7 out of 8 cases of non-compression arthrodesis at an average 25 weeks. 3. Among the 20 cases, complication were pseudoarthrthrosis in one case, pin tract infection in 2 cases and skin necrosis in 5 cases. 4. Inconclusion of the above results, bone union was carried out in 19 cases out of total 20 cases, and compression arthrodesis showed faster bone union at an average of 3.7 weeks earlier than non-compression arthrodesis.

6.Successful Application of Extracorporeal Membrane Oxygenation for a Patient with Clinical Amniotic Fluid Embolism.

Amniotic fluid embolism (AFE) is a rare but potentially fatal complication that occurs acutely during pregnancy or within 12 h of delivery. The management of AFE focuses initially on supportive measures for cardiopulmonary stabilization. Extracorporeal membrane oxygenation should be considered in patients who are unresponsive to medical treatment in order to prevent additional hypoxia and subsequent organ failure. We present a 41-year-old woman with clinical AFE who developed acute respiratory distress syndrome and was treated successfully with extracorporeal membrane oxygenation.

8.A Clinical Observation on Modified Denis-Browne Urethroplasty.

A clinical observation was made on 10 cases of modified Denis-Browne urethroplasty experienced at Department of Urology, Jeonbug National University, from January 1978 to July 1980 and the following results were obtained: 1. The age was distributed from 2 years to 44 years and most of cases was found in the age group which was less than 7 years. 2. Among 10 cases of modified Denis-Browne urethroplasty, 4 cases were penile hypospadias, 4 cases were penoscrotal hypospadias and 2 cases were perineal hypospadias. 2 cases of penile hypospadias were not combined chordee. 3. Cryptorchism and meatal stenosis were the most common associated anomalies. 4. Interval from chordectomy to modified Denis-Browne urethroplasty took 6 months to 1 year in 6 cases (75%), with the average of 10 months 5. We performed trocar cystostomy as urinary diversion in all cases. 6. 3-layer longitudinal monofilament subcuticular suture was used in all cases. 7. Among 10 cases, complications developed in 2 cases (20%), one case was urethral fistula, the other was urethral stricture 8. Success rate of modified Denis-Browne urethroplasty in our cases was 80%.

9.The Effects of Alcohol On the Serum Lipid Level of Healthy Adutss.

BACKGROUND: It has been reported that hypercholesterolemia is associated with coronary heart disease and that alcohol intake has influence on the lipid metabolism. This study was conducted to evaluate the relations between alcohol intake and serum lipid level. METHODS: We reviewed 906 subjects who had visited for routine checkup at family practice of Hanil hospital and then excluded 130 subjects with DM, renal disease, thyroid disease, hepatic disease or drug administration which might have influence on serum lipids. From the remaining 776 subjects, we obtained the amount and the frequency of alcohol drinking during the previous 2 weeks and the duration of alcohol intake, using a formed questionnaire. The alcohol dependence score was recorded by the National Alcoholism Screening Test. The serum lipids such as total cholesterol, triglyceride, and HDL-cholesterol were tested by enzyme method and the obesity index was obtained by Broca's method. These data were analyzed by SAS/PC program RESULTS: The total number of subjects was 776. Of those, 67.7%(n=525) were male and 32.3%(n= 251) females. The mean age of subjects was 50.5 years(range 15 to 77 years). The distribution according to the obesity index was as follows ; underweight group 7.1%, normal-weight group 38.1%, overweight group 31.6%, obesity group 23.2%. The number of the drinkers was 382(49.2%) in males and 7 (0.9%) in females. The mean serum lipid level were compared and analyzed among the groups. The results were as follows ; the total cholesterol and triglyceride levels were higher in males and the HDL-cholesterol levels was higher in female. The total cholesterol and triglyceride levels increased with age and obesity index, the HDL-cholesterol level decreased with obesity index. In the relationship between alcohol intake and serum lipid levels in males, the geometric mean of triglyceride level was higher among all the variables of alcohol intake, and that of HDL-cholesterol level increased with the amount and the frequency of alcohol intake. The results of multiple regression analysis associated with age, obesity index, the amount and the frequency of alcohol intake in males were as follows ; the triglyceride level was positively associated with the amount of alcohol intake, and the HDL-cholesterol level was positively associated with the frequency of akohol intake. CONCLUSIONS: The effects of age, sex and the obesity index on the serum lipid level are remarkable, and in males, alcohol intake increases triglycaride and HDL-cholesterol levels. Further study is needed to find out about whether alcohol intake have influences on coronary heart disease according to serum lipid levels.

10.A Clinical Observation on Tumors of the Genitourinary Tract.

A clinical observation was made on 282 patients with tumors of the genitourinary tract during the period from Jan. 1978 to Mar. 1982. The results were as follows: 1. 282 of the 1457 inpatients (19.35%) were the patients with tumor of the genitourinary tract. Men with thetumor were predominated (male to female: 17.8:1). 2. Prostate was the most commonly involved organ (66.8%) and the bladder. kidney, ureter, penis were followed. 3. Age was ranged from 8 months to 84 years, showing the highest incidence in the 8th decade (39.3%) andover 50 years old were 89.3%. 4. Urinary retention was the most common symptom (34.7%) and dysuria, gross hematuria, palpable mass were followed. 5. On histologic examination, begin prostatic hyperplasia was the most common (66.9%) and transitional cell carcinoma of the bladder, adenocarcinoma of the prostate, renal cell carcinoma. squamous cell carcinoma ofthe penis were followed. 6. 223 of the 282 patients with tumor (79.1%) were performed various type of operation. Transurethral surgerywas the most commonly performed (46.2%).
