1.Unilateral Chronic Organizing Hematoma after Breast Explantation Mimicking Chest Wall Tumor:a Case Report with Imaging Features

The number of women undergoing breast augmentation surgery with a prosthesis for cosmetic purposes or reconstruction after a mastectomy is steadily increasing.Hematoma is one of complications associated with breast augmentation surgery.It usually occurs early in the postoperative period. It rarely occurs late (after six months). However, chronic hematomas after prosthesis removal have not yet been reported in the radiological literature. We present a case of unilateral chronic organizing hematoma that developed late and grew persistently over long period after breast explantation, mimicking a soft tissue tumor of the chest wall clinically. Meanwhile, characteristic magnetic resonance imaging features of heterogeneous signal intensities on T1-weighted and T2-weighted images and dark signal intensity with a persistent enhancement of the peripheral wall of the lesion were found. These can be used for a differential diagnosis.

Dual left anterior descending artery (LAD) is a rare congenital coronary artery anomaly with a prevalence of approximately 1% in the general population. To date, 10 types of dual LAD artery anomalies have been reported. Among these, type 4 is one of the rarest. Knowledge and recognition of the dual LAD artery are important for correct diagnosis and planning of coronary bypass surgery and percutaneous coronary intervention. We report a case of a 59-year-old male with type 4 dual LAD artery who presented with dyspepsia and sweating for several months and had approximately 50%–70% stenosis in a major diagonal branch off the short LAD artery.

OBJECTIVES: This study aimed to investigate the prospective associations of premenstrual dysphoric disorder (PMDD) with quality of life, socio-occupational function and disability. METHODS: 170 nurses were recruited from two general hospitals. Interviews were made at baseline and at four follow-up points (two mid follicular phases and two late luteal phases of the two consecutive menstrual cycles). The baseline evaluation consisted of sociodemographic characteristics and menstrual history. PMDD was diagnosed by DSM-IV criteria after observation of the two menstrual cycles. The follow-up evaluations were consisted of the World Health Organization Quality of Life-Brief form, the Social and Occupational Functioning Assessment Scale, and the World Health Organization Disability Assessment Scale-II. RESULTS: PMDD was detected 15 (8.8%) of 170 participants. The PMDD group showed significantly worse quality of life and socio-occupational functioning than the none-PMDD group particularly at the late luteal phases. No significant differences were found in sociodemographic characteristics and menstrual history between those with and without PMDD. CONCLUSION: PMDD was prospectively associated with worse quality of life and socio-occupational functioning in this study group.

Age-related body composition changes such as sarcopenia and obesity affect functional decline in the elderly. We investigated the relationship between body composition parameters and functional limitation in older Korean adults. We enrolled 242 men and 231 women aged > or = 65 yr from the Korean elderly cohort. We used appendicular skeletal muscle mass (ASM) divided by height2 (ASM/Ht2) and ASM divided by weight (ASM/Wt). The isokinetic strength of knee extensor muscles were measured using an isokinetic device. Functional limitations were assessed using the Short Physical Performance Battery (SPPB) score less than nine. Men within the bottom tertile of ASM/Ht2 confer an increased risk for functional limitation compared with those within the top tertile (OR, 6.24; 95% CI, 1.78-22.0). However, in women, subjects within the lowest ASM/Wt tertile had a higher risk compared with those within the highest tertile instead of ASM/Ht2 (OR, 7.60; 95% CI, 2.25-25.7). Leg muscle strength remained the strong measure even after controlling for muscle mass only in women. Only large waist circumference was positively associated with functional limitation only in women. We might consider a different muscle index to assess functional limitation according to the gender.

9.Requests for Child Abuse Education in Medical School Curricula.

OBJECTIVES: This study aimed to examine current educational experiences, knowledge, intention to report, and requests for child abuse education in medical interns. METHODS: A descriptive cross-sectional study was conducted in 2006 and 2007. The study sample consisted of 193 medical interns who served their internships at the university hospital. They answered 11 self-administered questionnaires related to child abuse. RESULTS: Although respondents indicated a strong will to assist in eradicating abuse of children, about 90% had no educational experiences and knowledge of child abuse. Ignorance was a major factor for low reported cases of abused children. The preferred nominated reporting agency for child abuse was the National Child Protection Agency in 47.9% of female respondents, while 48.3% of males nominated Police Stations as their preferred option. In relation to sexual abuse, Police Stations were the preferred reporting agency by 49.2% of males and 37.0% of females. Medical school curricula were chosen by the majority of interns as the most appropriate stage where child abuse education should be introduced. CONCLUSION: This study found that medical graduates had limited experience and knowledge related to child abuse. The medical school curricula for child abuse needs to be further developed, implemented, and evaluated when appropriate.

10.Retroperitoneal Giant Liposarcoma.

Retroperitoneal liposarcoma is an infrequent, locally aggressive malignancy. We report two cases of huge retroperitoneal liposarcomas. The presence of a palpable abdominal mass was a common symptom of the two patients. Preoperative imaging study showed huge retroperitoneal tumors. Both patients underwent complete surgical resections, and a negative microscopic margin was achieved in both cases. The histopathologic diagnosis was a well-differentiated retroperitoneal liposarcoma. Neither of the two patients developed a recurring tumor during the 1.5 years of follow-up.
