1.Evaluation of sonographic and radiographic findings of pelvic masses

Ultrasonographic and radiographic findings in 72 cases of pelvic masses which have been provedhistopathologically were analyzed. The results were as follows; 1. Among 72 cases, 28 cases were uterine massesand 44 cases were adnexal masses. 2. Pathologic accuracies of ultrasonographic diagnosis were 64.3% in uterinemasses, 70.5% in adnexal masses, and 68.1% in all pelvic masses. 3. Plain or IVP findings were not specific indiagnosis and showed secondary mass effects or functional disturbances. 4. Because of its high accuracy ofpathologic diagnosis and safety, ultrasonography can be regared as the most valuable and preferential study. Butin cases of cystic myomas, endometrioses, cystic missed abortions and huge masses, the accurate diagnoses weredifficult.

3.A Case of the Torsion of the Term Pregnant Uterus with a Transverse Lie of the Fetus.

Uterine torsion is defined as the mtation of more than 45 degrees around the long axis of the uterus or a rotation which is severe enough to produce symptoms. Torsion of the human pregnant uterus is a very rare complication. The diagnosis is problematic and is often made during a cesrean section due to inhibited labor. Torsion of the human pregnant uterus can be caused by myoma or other uterine abnormalities, ovarian cysts, adhesions, abnormal fetal presentations, or fetal abnormalities ( though no cause can be found in about 20% of cases ). Uterine torsion produces symptoms of varying severity depending on the degree of rotation, and duration of the pregnancy. High mortality rates for both mother and infant have been reported. We experienced one case of the torsion of term pregnant uterus and report this case with a brief review of the concerned literature.

4.Transient Effects on the Risk of Occupational Injuries as an Acute Events: a Case-crossover Study.

OBJECTIVES: To elucidate the transient effects on the risk of occupational injuries as acute events and establish an alternative proposal. METHODS: The study population comprised a total of 302 workers randomly selected from applications for occupational injury compensation reported to the Inchon local labor office from January 1, 1999 to December 31. A case-crossover design, where each case serves its own control, was applied to this study. Through a telephone interview, workers provided useful data concerning five job related stressful events such as company transfer, work load change, overtime work, exchange duty, and work-part transfer. They were asked whether there were stressful events within a week of the occurrence of injury and the degree of stress. Exposure status from one year prior was used as control information. In the end, the data provided by 158 of selected persons was used for the analysis based on the quality of the data provided by the participants. A conditional logistic regression was used to discover the transient effects on the risk of occupational injuries as acute events. RESULTS: The effect of a company transfer and work load change on occupational injury was statistically significant on the risk of occupational injuries as an acute event(RR=5.5, 95% CI=2.501-12.428; RR=3.1, 95% CI=1.963-5.017, respectively). Other stressful events were found to elevate the risk factor for the occurrence of occupational injury, but were not significant. CONCLUSIONS: Our results suggested that transient stressful events elevated the risk factor for the occurrence of occupational injury.

5.Psychometric Properties of the Korean Version of the Clinical Language Disorder Rating Scale (CLANG).

OBJECTIVE: Our study aimed to measure inter-rater and test-retest reliability, concurrent and convergent validity, and factor solutions of the Korean version of the Clinical Language Disorder Rating Scale (CLANG). METHODS: The Korean version of the CLANG for assessing thought, language, and communication, the Brief Psychiatric Rating Scale, Young Mania Rating Scale, and Calgary Depression Scale for Schizophrenia were used to evaluate language disorder, formal thought disorder, positive and negative symptoms, manic symptoms, and depressive symptoms, respectively, in 167 hospitalized patients with schizophrenia. The factor solution was obtained by the direct oblimin method. A receiver operating characteristic curve was used to find the optimal cut-off score for discriminating schizophrenia patients with and without disorganized speech. RESULTS: Inter-rater reliability was considered moderate (intraclass coefficient=0.67, F=3.30, p=0.04), and test-retest reliability was considered high (r=0.94, p<0.001). Five factors, namely, pragmatics, disclosure, production, prosody, and association, were identified. An optimal cut-off score of 7 points with 84.5% sensitivity and 81.7% specificity was proposed for distinguishing schizophrenia patients with and without disorganized speech. CONCLUSION: Our findings suggest that the Korean version of the CLANG is a promising tool for evaluating language disorder in patients with schizophrenia.

6.A Case of Posterior Ischemic Optic Neuropathy after Endoscopic Sinus Surgery.

Recently, endoscopic sinus surgery (ESS) has been acknowledged as a standard surgical procedure for the treatment of chronic rhinosinusitis. Orbital complications of varying degrees that may occur during ESS have been widely reported. Blindness, one of the major complications that can occur during or immediately after ESS, is mainly attributed to orbital hematoma or direct injury to the optic nerve. In contrast to such direct mechanical trauma caused during ESS, we report a case of acute loss of vision that followed ESS without a definite cause. A postulated mechanism for idiopathic optic neuropathy is that it is ischema resulted from vasospasm in the branches of ophthalmic artery due to topical use of vasoconstrictive agents. Otorhinolaryngologists should be aware that this condition may occur following an uncomplicated ESS procedure. When loss of sight is diagnosed postoperatively, patients should be given prompt ophthalmological consultation and possible causes should be considered.

7.Gastric Ulcer Perforation to the Liver Diagnosed by Endoscopic Biopsy

Peptic ulcer disease is common and can be diagnosed easily if the patient has an ulcer history or characteristic abdominal symptoms. On the other hand, it may take a long time for the patient to visit the hospital due to severe complications if the patient is old or insensitive to symptoms caused by peptic ulcers. In the present case, a 72-year-old female visited the hospital due to general weakness and inadequate oral intake, which started two weeks ago. Endoscopy and abdominal CT revealed huge gastric ulcer findings. Through a tissue examination by endoscopy, hepatic cells were identified, and the patient was diagnosed with peptic ulcer perforation to the liver and later received surgical treatment.
8.Primary Gastric Lymphoma:Clinicopathologic Review and Prognostic Factors.

BACKGROUND: Primary gastric lymphoma is a rare neoplasm that represents 1~4% of all gastric malignancies. The optimal treatment of primary gastric lymphoma remains controversial. This study was designed to examine the prognostic significance of various clinicopathologic factors in patients with gastric lymphoma and to evaluate the role of surgery in the treatment of gastric lymphoma. METHODS: Retrospective study of 71 cases of primary gastric lymphoma which had been undergone laparotomy at the Department of Surgery, Seoul National University Hospital from Jan. 1982 to Dec. 1996 was performed. The authors reviewed clinicopathologic factors of patients and evaluated their prognostic significance. RESULTS: 1) The mean age of the patients was 50 years, and the most prevalent ages occurred in the 6th & the 7th decade. The ratio of males to females was 1.3:1. 2) The presenting symptoms and signs on admission were epigastric abdominal pain & discomfort, weight loss, anorexia, indigestion, and abdominal mass in decreasing order of frequency. 3) The diagnostic accuracy of the gastrofiberscopic biopsies was 67.1% preoperatively. 4) The most common pathologic type was diffuse histiocytic type (57.1%) in Rappaport classification, and diffuse large cell type (47.1%) in NCI Working formulation classification. 5) The distribution of stages according to modified TNM classification was 47.9% stage I, 36.6% stage II, 11.3% stage III, and 4.2% stage IV. 6) Overall, a potentially curative resection was performed in 92.9% of the cases. Treatment modalities included surgery (S), chemotherapy (CT), and radiotherapy (RT) in the following proportions: 33.8% only S, 53.5% S CT, 11.3% S RT, and 1.4% S CT RT. 7) The overall 5-year survival rate was 78.9%. The 5-year survival rates according to stage were 94.1% in stage I, 80.4% in stage II, 30.0% in stage III and 0% in stage IV.

10.A Case of Leptomeningeal Carcinomatosis Presenting as a Neurological Complication of Stomach Cancer.

Leptomeningeal carcinomatosis, an important and serious neurological complication of malignant cancers, is asso- ciated with severe disability and high mortality. It is most commonly seen in patients with breast cancer, lung cancer and malignant melanoma. However, leptomeningeal carcinomatosis induced by gastric cancer has been rarely reported, although gastric cancer is one of the most common causes of cancer-related deaths in Korea. We report a case of a 76- year-old woman with leptomeningeal carcinomatosis presenting as a neurological complication of gastric cancer and con- firmed by cytology examination of the cerebrospinal fluid and brain magnetic resonance imaging with gadolinium enhan-cement.
