1.A Pathologic Study of Renal Cell Carcinoma: Correlation between clinical and morphologic parameters and prognosis.

The prognostic significance of morphologic parameters was evaluated in 36 cases of renal cell carcinoma diagnosed during five years(1986~1990). We reviewed and classified on the basis of pathologic stage, tumor size, histologic pattern, cell type and nuclear grade. Mean age was 51 years old. Average tumor size was 7.3 cm in diameter. Six of 35 patients died of disease. Overall mean survival was 43.3+/-7.3 months. An increasing nuclear grade was generally correlated with a decrease in cummlative survival rate. Similarly, a higher stage at the time of diagnosis could predicated a low survival rate only for high nuclear grade carcinoma. There was an apparent positive correlation between grade and age, grade and size, grade and cell type, cell type and histologic pattern as well as stage and age. This positive correlations are in part a function of nuclear grade; only 20% of grade 3 & 4 tumor consisted of clear cells whereass 71% of grade 1 & 2 consisted of clear cell type. All 6 cases of granular cell types and 50% of mixed cell type were grade 3 & 4. The tumor size of the primary was well correlated with the nuclear grade. Nuclear grade was the most significant factor among the morphologic parameters studied.
2.Sequential Studies of Glomerular Crescent Formation in Rabbits with Anti-Glomerular Basement Membrane(GBM) Antibody Induced Glomerulonephritis(GN).

To investigate the mechanism of crescent formation, sequential pathologic changes from the New Zealand White rabbits with anti-GBM antibody induced GN by administration of guinea pig anti-GBM IgG were studied by light (LM), immunofluorescent (IF) and electron (EM) microscopy. Although no glomerular changes were observed in LM, swelling of the endothelial cells and the epithelial cells were noted in EM by day 2. By day 7, early and cellular crescents were evident. Proteinaceous materials and fibrins were noted in the glomerular capillary lumina (GCL) and Bowman's space (BS) associated with segmental hypercellularity. The GBM damage became progressively severe, followed by focal detachment of the visceral epithelial cells from the GBM. At day 14, fibrin strands, mononuclear cells and collagen fibrils were present between the proliferating extracapillary cells. At day 31, fibrocellular crescents were predominated. Elongated spindle cells, morphologically resembling myofibroblasts, were noted near the Bowman's capsule (BC). A degree of tubular atrophy, interstitial fibrosis, and inflammatory infiltrates increased as it did with fibrous organization of crescent. Intense linear IF staining for IgG and C3 were seen throughout the experiments along the GBM. In conclusion, the progression of crescent from an early "proteinaceous" stage through cellular, fibrocellular and fibrous stages was well documented in this study. Inflammatory cells and coagulation mechanism may activate the initiation of the GBM damage at the early stage. Activated periglomerular mononuclear cells may also cause disruption of BC which facilitates entry of activated periglomerular cells and fibroblasts into BS leading to progressive fibrous crescent formation.

3.A Statisitical survey of Skin Tumors.

The 447 cases of skin tumors were studied at the departments of dermatology and surgical pathology, National Medical Center in Seoul during the period of 18 years and 8 months from January, 1959 to August, 1977. The results were as follows: 1) the total number of specimens surveyed in the study was 447, among which benign tumors accoungted for 234 cases and malignant tumors for 213. 2) Male to female ratio for benign tumors was 1.2:1 and for malignant tumors 2.2:1. 3) Among the 234 cases of benign tumors, 67 cases were epidermal cyst. 53 cases of hemangioma and 34 cases of pigmented neni. 4) Among the 2 cases of malignant melanoma. 5) The peak age incidence for benign tumors were below the 2nd decade, The peak ago incidence for malignant tumors were 50th decade. 6) the sites of predilection of the epidermal cysts were trunk, face and neck. The sites of predilection of the hemangiomas and pigmented nevi were face. 7) The sites of peredilection of the squamaous cell carcinomas were penis(27.1%), lower extremities(25.7%) and face(21.5%) The sites of predilection of the basal cell carcinomas were face(83.3%) 8) The ratio of squamous cell carcinoma ws basal cell careinoma was 4:1. 9) The accuracy of clinical dianosis were 42.7% for benign tumors and 79.3% for malignant tumors.

4.A Structural Equation Model of General High School Students’ Happiness:Mediation Effect of Self-esteem and Resilience

Purpose:
This study was conducted to develop a predictive model for happiness of students in general high schools.The study was based on the theory of happiness integration and the literature review. Methods: Data were collected using a convenience sample of 231 first and second grade students in five general high schools in D city and K Province. The exogenous variables were optimism as personality factors, parenting attitude, academic stress and friend’s support as happiness. The endogenous variables were self-esteem and resilience as mediating variables, and happiness of students in general high schools. Data collection was done from March 14 to March 28, 2019. The AMOS 22.0 and SPSS programs were used to verify the validity of confirmatory factor analysis and hypothesis models.
Results:
The factors that have the greatest influence on happiness of high school students are self-esteem and resilience. The explanatory power of happiness by these variables was 98.1%. The individual mediating effect and double mediation effect of these variables were found to be significant.
Conclusion
As student's optimistic personality, parent's positive parenting attitude, and friend’s support improve student's self-esteem and resilience, ultimately positively affecting high school students' happiness, so it is necessary to consider these factors and try to develop a happiness improvement program.
5.Two Cases of Eruptive Vellus Hair Cysts.

We present two oases of Eruptive Vellus Hair Cysts(EVHC) in 30- year-old and 27- year old women. They had 6 to 10 discrete, asymptomatic, brown or reddish brown papules on the neck, axilla, chest and extremities. The clinical impressions were epidermal cyst and steatocystoma multiplex espectively, but histopathological exarnination revealed smal1 cysts in middermis containing numerous vellus hairs cut transversely or obliquely and surrounding granulomatous foreign body reaction typical to EVHC. Transepithelial elimination was not found.

6.A Case of a Solitary Type of Congenital Self-healing Reticulohistiocytosis.

Congenital self-healing reticulohistiocytosis (CSHRH) typically presents at birth or in the first few weeks of life as a widespread eruption of cutaneous red-brown papulonodules that resolve spontaneously without involvement of other organs. While multiple lesions are common, a solitary lesion is rare. We describe a solitary type of CSHRH in a full-term, male neonate. He had an erythematous papule with a yellowish crust on the left heel without any systemic symptoms. Four weeks later, the skin lesion had disappeared spontaneously.

8.Intestinal Colonization of Vancomycin-Resistant Enterococci.

BACKGROUND: In the United States, the Centers for Disease Control and Prevention recorded a 20-fold increase in the incidence of vancomycin-resistant enterococci (VRE) associated with nosocomial infections between 1989 and 1991. Although VRE has been reported in Korea since 1992, infections caused by these organisms are still extremely rare in Pusan, Korea. Therefore, a point prevalence culture survey was carried out to investigate the prevalence of intestinal colonization with VRE among patients admitted to Kosin Medical Center, which can predict the appearance of clinical infections with VRE. METHODS: Between July 1997 and August 1997, stool specimens were obtained from 303 patients. Specimens were placed in bile esculin azide broth containing vancomycin (64 microgram/mL) and aatreonam (60 microgram/mL). Cultures were done for 48 hours at 37degrees C, and turbid solutions were subcultured on blood agar. Minimal inhibitory concentrations (MIC) of vancomycin and teicoplanin to Enterococcus isolates were determined by Etest on Mueller-Hinton agar. For amplification of the vanA, vanB, and vanC genes, polymerase chain reactions were performed. RESULTS: VRE isolates were isolated from 6 of the patients (2%). Four of them were identified as E. faecium, and 1 was identified as E. avium, and 1 was identified as Enterococcus spp. All of them were highly resistant to vancomycin (MICs >256 microgram/mL), and they were also resistant to teicoplanin (MICs 32-->256 microgram/mL). All of 6 VRE strains carried vanA gene. CONCLUSION: The colonization of VRE was not infrequent among the patients of a university hospital in Pusan, Korea. Moreover, a large proportion of the colonizing VRE was revealed Enterococcus faecium with vanA gene, which implies quite a possibility of a sudden rising of infections by this organism in the near future. So we propose that the vancomycin susceptibility test should be done for every enterococcal isolate from clinical specimens and the intestinal colonization rate of VRE should be closely monitored at regular intervals for the purpose of surveillance 50 that proper establishment of plans for the prevention of this troublesome pathogen's spread can be promptly made.

9.A Clinical Study of Colorectal Cancer in Patients More Than 65 Years Old.

This report is a retrospective clinical analysis fo 84 cases more than 65 years of colorectal carcinoma treated surgically by the Department of General Surgery, College of Medicine, Soon Chun Hyang University from January 1991 to December 1995. The average age was 71.3 years; 49 patients were male and 35 were female. Tumor location was as follows; rectum 39(46.4%), sigmoid 22(26.2), ascending colon 12(14.3%), descending colon 8(9.5%), transverse colon 3(3.6%). The most frequent symptom in colorectal cancer was abdominal pain. The patients whose clinical symptom had been for less than 1 month before the hospitalization was about 31.0%. The rate of curative resection was 88.1%, Emergency operation was performed about 19.0%(16 cases). By Astler Coller classification, there were stage A 2 cases(2.5%), Bl 13 cases(16.5%), B2 28 cases(35.4%), Cl 2 cases(2.5%), C2 24 cases(30.4%), D 10 cases(12.7%). The average size of mass was 4.41 on. The most common pathologic type was moderately differentiated adenocarcinoma. Postoperative mortality rate was 4.7%(4 cases). The 5 year cumulative survival rate was 42.9%. In conclusion, the postoperative mortality and survival rates obtained in this study encourage us not to consider age as a limiting factor for curative surgical treatment. Early detection of colorectal cancer, adequate management of preoperative underlying disease and aggresive curative resection are improving outcome in the surgery of old aged patients with colorectal cancer.

10.A Case of Anencephaly Associated with Twin Pregnancy Conceived by In Vitro Fertilization and Embryo Transfer.

Twin pregnancies are increasing these days due to recent development of the technology in treating infertility and twin pregnancies tend to cause more congenital anomaly than singleton pregnancies do. Although anencephaly is not uncommon, occurring in about one in every 1,000 births, anencephaly developed in a twin pregnancy is very rare. We experienced a twin pregnancy which consisted of an anencephalic fetus and a normal one. This pregnancy was conceived by IVF and ET. This pregnancy was continued without having special problems and was delivered at 36 weeks of gestation by cesarean section due to SROM (spontaneous rupture of membrane). The anencephalic fetus weighed 1,430 gm and was already dead in the uterus. The other weighed 2,660 gm and showed no external anomaly. Its Apgar score was 8 in 1 minute and 9 in 5 minute. The pregnancy resulted in a cesarean section delivery of one dead anencephalic fetus and one normal healthy fetus. We report with a brief review of literature a case of a twin pregnancy, conceived by IVF and ET, in which anencephaly was associated with a normal fetus.
