2.Contribution of Color Doppler Scoring System to Diagnosis of Ovarian Malignancy.

The purpose of this prospective study was to evaluate the diagnostic value of color Doppler scoring system in characterization of ovarian masses. We performed transabdominal or transvaginal color Doppler sonopaphy on 82 women with ovarian masses and analyzed the sonographic findings. Ovarian lesions were assessed by means of morphological and color Doppler scoring system by Kurjak. Tumors were characterized by ultrasonographic findings as benign or suspected of being malignant. Then the results of each scoring systems were correlated with histopathological findings. The results were as follows; Of 82 ovarian masses, 64 were benign lesions(13 mucinous cystadenomas, 16 endometriomas, 20 teratomas, 8 serous cystadenomas, and 13 other abnormalities), and 18 were malignant(12 cancers and 6 borderline tumors). The color Doppler scoring system was useful in distinguishing malignant from benign masses, with a sensitivity of 88.2%, compared with the morphological scoring system of 86.7%. The specificity of color Doppler and morphological scoring system were 95.4% and 92.5%, respectively. The sensitivity of combination of both scoring systems was 88.9%, with a specificity of 96.9%. In conclusion, color Doppler scoring system is a useful tool in predicting the malignancy of ovarian lesions especially combined with morphological scoring system.

3.Periosteal Reaction of Osteomyelitis: MRI Findings Compared with Plain Radiographs.

PURPOSE: To evaluate MR characteristics of periosteal reactions and subperiosteal abscesses in osteomyelitis as compared with radiographs. MATERIALS AND METHODS: We retrospectively reviewed 28 patients(18 males, 10 females) with osteomyelitis. Nineteen patients underwent MR imaging with 0.5 T. We analyzed for morphologic patterns, intervals of appearance and disappearance of periosteal reactions after symptom onset. Twenty-three patients were confirmed by surgery and 5 patients by the radiologic findings, clinical and laboratory data. RESULTS: Periosteal reaction appeared as low signal intensity arc on the T1- and T2-weighted axial images. In 3-7 days after symptom onset, periosteal reactions and subperiosteal abscesses were noted on MR imaging, and only two of eleven patients were noted in radiographs. Periosteal reactions of 8-14 days after symptom onset were demonstrated in all of 13 patient on MR, and 9 on plain radiographs. All patients with 2 weeks after symptom onset showed perioste.al reaction in plain film and MR. The periosteal reactions persisted until approximately 2 to 3 months after treatment on follow up radiographs. CONCLUSION: Periosteal reactions in osteomyelitis are detected on the MR imaging earlier than plain film. MR is valuable in detecting subperiosteal abscess which is not appecent in simple radiographs. Periosteal reactions can be seen on MR in 3 days after symptom onset and persist for 2-3 months after treatment.

4.A Case of Retinoblastoma Associated with Secondary Glaucoma.

Retinoblastoma is a rare, congenital, malignant tumor that arises from the retina. It is the most common intraocular tumor occurring during childhood. Retinoblastoma usually remains unnoticed until it has advanced far enough to produce a white pupil. But it may manifest as a painful, red, tearing eye due to secondary glaucoma. The most common mechanism inducing secondary glaucoma in retinoblastoma is neovascularization of iris with peripheral anterior synechiae formation. The next common mechanism is massive exudative retinal detachment causing pupillary block and angle closure. Uveitis and/or necrotic tumor tissue in anterior chamber acn contribute to the development of secondary glaucoma. A 9 month old Korean boy was seen to have somewhat enlarged cornea with massive exudation in anterior chamber OS. Intraocular pressure was 27.4 mmHg OS. The C-T scan was done because of hazy media OS. and showed calcification within left eye. Left eye was enucleated and found to have retinoblastoma which was confirmed microscopically.

5.Neonatal outcomes of very low birthweight infants from spontaneous and indicated preterm delivery.

OBJECTIVE: To analyse neonatal outcomes of the very low birthweight infants born to women who underwent an indicated preterm delivery and spontaneous preterm delivery. METHODS: We performed an observational study of 150 very low birthweight(<1500 grams) infants delivered at Samsung Medical Center. The study population was limited to singleton infants without major congenital anomalies. The primary reason for delivery was categorized as indicated preterm delivery or spontaneous preterm delivery. Selected neonatal outcomes were compared between infants born to women in each of these groups. RESULTS: Univariate analyses showed some survival benefits in infants born to women who underwent indicated preterm delivery. Selected neonatal outcomes, however, did not differ between the groups in the multiple logistic regression analysis. CONCLUSION: There was no survival advantage to the very low birthweight infants born to women who underwent an indicated preterm delivery compared to those born to women with spontaneous preterm delivery.

OBJECTIVE: The aim of this study was to determine whether fetuses delivered prematurely because of pregnancy complications had a different neonatal outcomes than that of those bom after either spontaneous preterm labor or after premature rupture of membrane. METHODS: The study design was a retrospective analysis of 150 very low birthweight(<1500 grams) infants and their mothers who delivered preterm neonates at Samsung Medical Center. The primary reason for delivery was categorized as preterm delivery because of pregnancy complications(indicated preterm delivery) or spontaneous preterm delivery. RESULTS: There were some survival benefits in infants born to women who underwent indicated preterm delivery from univariate analyses. In the multiple logistic regression analysis, however, selected neonatal outcomes did not differ between the groups. CONCLUSION: We concluded that a "stressed" pregnancies confer negligible survival advantage to the very low birthweight infants.

7.A case of nodal marginal zone B-cell lymphoma with macroglobulinemia.

Macroglobulinemia is the result of an uncontrolled proliferation of lymphocytes and plasma cells in which a large IgM M protein is produced. IgM monoclonal gammopathy is recognized in a variety of lymphoproliferative diseases and Waldenstr?m's macroglobulinemia (WM) is the most frequent disease in this group. We report a case of nodal marginal zone B-cell lymphoma (Nodal MZBCL) accompanied by monoclonal macroglobulinemia. A 58-year-old man was admitted to the hospital with chronic fatigue and dyspnea. Physical examination revealed cervical, subaxillary, and inguinal lymphadenopathy. Histopathologically, monocytoid B cells with abundant pale cytoplasm and small nuclei infiltrated the cervical lymph node. The neoplastic cells were positive for CD 20, bcl-2, and IgM. The serum and urine electrophoresis showed monoclonal spike in the globulin region and immunoelectrophoresis demonstrated immunoglobulin of IgM, kappa type. Immunohistochemically, this monoclonal gammaglobulinemia (IgM, Kappa) was produced and secreted from the nodal MZBCL. This is the first report of nodal MZBCL accompanying macroglobulinemia in Korea.

A metastatic melanoma to the gastrointestinal tract is observed in 1.5~4.4% of all melanoma patients. However, colonic and rectal involvement is less common. A 72-year-old woman was admitted due to abdominal pain and poor oral intake for 20 days. She had a 3 x 4 cm-sized mass on her right inguinal area 4 month ago, which was diagnosed as a malignant melanoma of the inguinal lymph node on excision biopsy. A large exophytic mass with an irregular ulcerlated, whitish patch, erythematous surface was observed in the hepatic flexure during colonoscopy. A histology diagnosis of a metastatic melanoma was made by an optical microscopy examination of the biopsies obtained during the colonscopy, and palliative right hemicolectomy was performed on account of a potential intestinal obstruction. We report a case of a metastatic melanoma of the colon with a review of the relevant literature.

9.Rhabdomyolysis Associated With Statin Medication, Exercise and Sauna.

Rhabdomyolysis is a syndrome involving the breakdown of skeletal muscle, and this causes myoglobin and other intracellular proteins and electrolytes to leak into the circulatory system. We report here on a case of rhabdomyolysis associated with statin, exercise and sauna exposure. A 63-year-old woman presented to our hospital, and she had been taking simvastatin for over 6 years due to dyslipidemia. She complained of developing myalgia and weakness of the lower extremities over the previous 5 days. She had used a sauna more than 4 hours daily for 20 years, and she had started unaccustomed exercise in the sauna 1 week prior to admission. Her serum creatine kinase concentration was 45,990 IU/L. Conservative treatment was started with the discontinuation of her statin medication, exercise and use of the sauna. Her symptoms and serum creatine kinase level decreased 1 week after admission and she was discharged with minimal residual muscle weakness.
