5.A Clinical Study of Surgically Treated Supracondylar Fractures of Femurs

Supracondylar fracture of femur is not well suited to internal fixation. A wide canal, thin cortex, comminution and compound wound make open reduction difficult. And the associated injuries, such as popliteal artery rupture are not infrequent. Some authors reported the favorable results with open reduction and rigid internal fixation. But still we have had failure cases with loosening or breakage of plate and screws, infection, post-operative joint stiffness and non-union etc. The present paper reports the result of surgically treated supracondylar fracture. From March 1982 to February 1989 25 patients who were treaed at Department of Orthopedic Surgery, St. Vincent Hospital were analysed. The results obtained as follows:l. Age distribution was between 29 and 74 years of age, and the ratio between males and females was 1.8:1. The most common of injury was traffic accident. 2. According to Neer's classification, there were 5 patients of type I, 7 patients of type II

9.Dissemination of Plasmid-mediated qnr, aac(6')-Ib-cr, and qepA Genes Among 16S rRNA Methylase Producing Enterobacteriaceae in Korea.

Plasmid-mediated quinolone resistance (PMQR) genes: qnr, aac(6')-Ib-cr, and qepA were investigated among 153 armA and 51 rmtB-positive transconjugants and their 204 clinical isolates of Enterobacteriaceae. Overall, qnrB4 and aac(6')-Ib-cr genes were identified in 52.3% (63 K. pneumoniae, 10 E. coli, 4 E. cloacae, and 3 E. aerogenes) and 24.8% (16 K. pneumoniae, 8 E. coli, 6 S. marcescens, 4 E. cloacae, 3 C. freundii and 1 K. oxytoca) of 153 armA-positive isolates, respectively. Four isolates of K. pneumoniae and two isolates of E. coli positive for armA co-harbored both qnrB4 and aac(6')-Ib-cr. The qepA gene was detected in 11.8% (5 E. coli and 1 K. pneumoniae) of 51 rmtB-positive clinical isolates and their transconjugants. Southern hybridization confirmed the co-localization of qepA and rmtB on a large conjugative plasmid of size between 90 to 170 kb. Inc replicon typing showed that qnrB4/6, aac(6')-Ib-cr, and qepA genes were principally disseminated by IncFIIAs, IncL/M, and IncF plasmids, respectively. This study constitutes the first report of the three known PMQR genes among the 16S rRNA methylase producing Enterobacteriaceae isolates of human origin from Korea.

10.Study of Functional Appliance for Treatments of Children and Adolescents with Class II Malocclusion

The purpose of this study was to evaluate the skeletal and dentoalveolar effects and optimal timing for treatment of class II malocclusion with functional appliances in children and adolescents. A group of 30 patients with class II malocclusion were divided into 3 groups according to their use of functional appliance: Twin block, Activator, Fränkel appliance. The group was also divided into 2 groups according to the cervical vertebrae maturation method. Lateral cephalometric radiographs were analyzed pretreatment (T0) and osttreatment(T1). Among the functional appliances, treatment with Twin block and Activator showed significant increase in the length of the mandible (Co-Gn) and the lower anterior facial height (ANS to Me), whereas the overjet and overbite were significantly reduced. Treatment with Fränkel appliance showed significant improvement in the relationship of maxilla and mandible. In addition, if the functional appliance was used during the period of pubertal growth peak, there was a significant increase in mandibular length, improvement in the relationship of maxilla and mandible, labial inclination of lower incisors and decrease in overjet compared to the treatment before pubertal growth peak. Therefore, this studyindicates that using functional appliances for patients with class II malocclusion is effective and the optimal timing for using functional appliances is during pubertal growth peak.
