Dermoid cysts develop from sequestration of epithelium along lines of embryonic fusion.The most common locations are the lateral third of the eyebrows, nose, and scalp. These cysts are located in the subcutis; they are often adherent to periosteum, and may invade or erode underlying bane. A 34-year-old female presented with a solitary, skin colored, dome-shaped, child fistsized, subcutaneous mass on her right occiput. At operation, keratinous material was discharged and tufts of hair projected from opening of the cyst wall.The base of the cyst was firmly adherent to periosteum and diffuse depression with focal deep erosions of the outer table of skull was found.

In Korea, there are about 8 million motor vehicles and one-fourth of it is motorcycle. Last year Korean government report on the motor vehicle accidents claimed 12,781 cases of motorcycle accidents and it account for 5.1% of all of motor vehicle accidents occurred in Korea. But this is too broad analysis and lack of statical values. In general, most of people think motorcycle rider will suffer or die from head injuries sustained with motorcycle accident. Even though mandatory law on wearing helmet during motorcycle ride, but we see too many motorcycle riders not wearing helmet, especially in teenagers. The loss of young person by accident is great loss for family, society and economy of nation. No one will doubt the effectiveness of helmet on reduced incidence of head injuries. And multiple studies from United States of America have clearly demonstrated the benefit of helmets in motorcycle accidents. The following study was undertaken to determine the relationship between head injuries and helmet use in motorcycle accident, as well as analysis of general motorcycle accident.

Cutaneous metastasis from renal cell carcinoma is not unusual, occurring in 3 to 7%, although it may be overlooked. We report a case of metastatic renal cell carcinoma in a 74-year-old man who was presented with a 1.5 x 1.5 x 0.5 cm-sized nodule on the right zygoma for 3 months. A skin biopsy demonstrated the typical clear cell adenocarcinoma with PAS(+) granules in the cytoplasm and honeycombed or glandular configuration, compatible with metastatic renal cell carcinoma. Kidney ultrasonogram and abdomina1 CT scanning showed a large cystic mass on the upper pole of the left kidney. Furthermore radiologic studies revealed multiple metastatic lesions in the lung, liver and brain. He died 4 months later.

Secondary cutaneous cryptococcosis may occur earlier than other manifestations of disseminated cryptococcosis. A 68-year-old woman presented with multiple ulcerative lesions on the right calf of 2 weeks duration. She had been treated with antibiotics, but the lesions spread rapidly. The initial clinical impression was necrotizing fasciitis, but routine KOH mounting from the ulcerative lesions showed numerous budding yeast cells with peripheral clear zones and further investigations including a skin biopsy, tissue cultures and India ink preparations allowed a rapid and definitive diagnosis of cutaneous cryptococcosis. Studies for other evidence of infection elsewhere revealed an asymptomatic pulmonary lesion. We report a case of secondary cutaneous cryptococcosis clinically mimicking necrotizing fasciitis that occured before other manifestations of disseminated cryptococcosis.

We report herein a gigantic case of verrucous hemangioma occuring in a 15 year-old school- boy. The lesion was a dark red colored, 10.0 x 6.0 x 0.8cm sized, verrucous surfaced, hemangiomatous tumor affecting the posterior aspect of the right thigh. At birth an erythematous macule appeared and insidious]y increased or extended with age. In the last 6 months, it had grown rapidly to the size of the present lesion with a smaller satellite one, assumably ascribing to the repeated minor trauma or pressure. Histological findings from the totally excised lesion showed epidermal hyperplasia with irregular acanthosis and papillomatosis and diffuse marked proliferation and dilatation of blood vessels from the upper dermis to subcutaneous fatty tissue. Wide excision was done. There was no relapse after about one years postoperative follow up.

A 28-year-old man presented with a 6.0 * 5.0 cm sized ulcer on the anterior chest and widespread copper-red colored maculopapular eruptions. There was no past or family history of syphilis sign or STS. He had been taken imposition of hands twice to treat chronic hepatitis about 4 months prior to presentation. Thereby, the ulcerative lesion on the anterior chest developed as a result of erosive trauma through contact with infected fingernails. The clinical features and serologic tests as well as biopsy findings were consistent with syphilis. Spirochetes were observed under the dark field examination of the anterior chest lesion. On Warthin-Starry staining, spirochetes were identified in the epidermal area of the papular lesion. We report a case of unusual transmission of syphilis.

PURPOSE: Metastatic prostatic adenocarcinoma can be controlled by androgen ablation through the active process of programmed cell death in androgen responsive cells. However, about 20-30% of patients have no clinical response to androgen withdrawal. Because of the importance of apoptosis in effecting tumor control, factors involved in this process may be helpful in predicting androgen insensitivity. So, we evaluated the significance of bcl-2 protooncogene expression pattern with therapeutic response of prostatic cancer MATERIALS AND METHODS: We examined the cellular expression of bel-2 protein using immunohistochemical stain in tumor samples from 40 patients with metastatic prostatic cancer(stage D) and determined whether expression of blc-2 protein has related to the therapeutic response of prostatic cancer. RESULTS: The hormonal status of the patient's tumor was determined by the clinical response to therapy. Androgen independent cancer was defined as that subset of patients who experienced no initial response to androgen ablation, or who experienced disease relapse following an initial response to androgen ablation. So, we found that androgen dependent prostatic cancer was 22 patients and androgen independent prostatic cancer was 18 patients. The positive staining for bcl-2 was 27.3%(6/22) and 83.3%(15/18) in androgen dependent and independent prostatic cancer, respectively. It was significant difference(p<0.05). CONCLUSIONS: These results suggest that bcl-2 expression is associated with androgen independent prostatic cancer and used one of the factors to predict which patient with prostatic cancer will respond to androgen ablation.
