3.A Case of Dermoid Cyst Causing Deep Erosion of the Skull.

Dermoid cysts develop from sequestration of epithelium along lines of embryonic fusion.The most common locations are the lateral third of the eyebrows, nose, and scalp. These cysts are located in the subcutis; they are often adherent to periosteum, and may invade or erode underlying bane. A 34-year-old female presented with a solitary, skin colored, dome-shaped, child fistsized, subcutaneous mass on her right occiput. At operation, keratinous material was discharged and tufts of hair projected from opening of the cyst wall.The base of the cyst was firmly adherent to periosteum and diffuse depression with focal deep erosions of the outer table of skull was found.

4.Effect of Extracorporeal Shock Wave on Ovary of Sprague-Dawley Rat.

Ovaries lie near the distal ureter where extracorporeal shock wave lithotripsy (ESWL) of calculi is performed. The question whether ESWL may induce morphological changes in ovary should be investigated in animal experiments. After applying shock waves to ovary of Sprague Dawley female rat, the histopathologic changes within the tissue of ovary were observed immediately after shock waves and after recovery. For animal subjects, 25 healthy rats weighing about 250 grams each were used. Five rats were designated as control, and remaining 20 received shock waves at 20 kV for 1,000 times using Pendulum-ESWL (Lithoring), a third generation lithotriptor. The 20 rats were divided into 4 groups before shock waves, and each group was sacrificed after 1, 7, 14, and 28 days, respectively, in order to compare the observed histopathologic changes. Edema, congestion and hemorrhage were observed in ovary after 1 and 7 days of shock waves, but the edema and congestion were significantly reduced on the 14th day and completely disappeared on the 28th day. There was no ovarian follicular changes or fibrosis on the whole. It is thought that the extracorporeal shock waves have no specific effects on the tissues of ovary, but further studies ate needed to determine its long term effects.

5.Cutaneous Cryptococcosis Clinically Mimicking Necrotizing Fasciitis.

Secondary cutaneous cryptococcosis may occur earlier than other manifestations of disseminated cryptococcosis. A 68-year-old woman presented with multiple ulcerative lesions on the right calf of 2 weeks duration. She had been treated with antibiotics, but the lesions spread rapidly. The initial clinical impression was necrotizing fasciitis, but routine KOH mounting from the ulcerative lesions showed numerous budding yeast cells with peripheral clear zones and further investigations including a skin biopsy, tissue cultures and India ink preparations allowed a rapid and definitive diagnosis of cutaneous cryptococcosis. Studies for other evidence of infection elsewhere revealed an asymptomatic pulmonary lesion. We report a case of secondary cutaneous cryptococcosis clinically mimicking necrotizing fasciitis that occured before other manifestations of disseminated cryptococcosis.

6.A Case of Verrucous Hemangioma.

We report herein a gigantic case of verrucous hemangioma occuring in a 15 year-old school- boy. The lesion was a dark red colored, 10.0 x 6.0 x 0.8cm sized, verrucous surfaced, hemangiomatous tumor affecting the posterior aspect of the right thigh. At birth an erythematous macule appeared and insidious]y increased or extended with age. In the last 6 months, it had grown rapidly to the size of the present lesion with a smaller satellite one, assumably ascribing to the repeated minor trauma or pressure. Histological findings from the totally excised lesion showed epidermal hyperplasia with irregular acanthosis and papillomatosis and diffuse marked proliferation and dilatation of blood vessels from the upper dermis to subcutaneous fatty tissue. Wide excision was done. There was no relapse after about one years postoperative follow up.

7.Epidemiologic Study of Paederus Dermatitis Prevailing in the Midwest Area of Kyungpuk Province.

BACKGROUND: Paederus derriattis is a peculiar linear lesion caused by an insect belonging to genus Paederus, family Staptylinidae, order Coleoptera. The deliberate or accidental contact of crushed coelomic fluid of th insect, on the skin causes acute lesions due to a t.oxin(pederin) being released. OBJECTIVE: The study was nade in order to investigate an epidemic status of the peculiar der matitis and its causative agent of factor prevailing in the midwest area of Kyungpuk province. METHODS: We performed field approaches on 633 cases(M: 303, F: 330) including cases enrolled in a local clinic, teamiring up with a local skin clinician over the 3-month period of August November in 1994. RESULTS: 1. The captive insect causing the present dermatits, was identified as Paederus fuscipes(PF). The epidemic might be attributeid to the climatic conditions with higher temperature than usual during the period of July-eary September in the past year by 3-4degrees C as well as a lower relative humidity. The active temperature zone for PF was presumed to be about 15.5+1.2degrees C to 28.9+1.0degrees C 2. There was the highest p;evalence of 412 cases(65.1%) in early September, followed by mid September, late August, early October and mid August in a small number of cases in decreasing order. The cases were not found after mid October. 3. All age groups were evealy represented. The sex ratio was 1: 1.1 with little difference. The majority of cases lived in Kiriich on city(80.3%), with a lesser outbreak in the neighboring coun ties and Kumi city. 4. The preferred site was the mainly exposed area such as face, neck and extremities. The number of lesion was single(60%) and multiple(40%). The lesion showed a common linear con figuration and acute irritant contact dermatitis consistent with the histologic finding. CONCLUSION: The unusual wiather conditions and poor environmental hygiene help to develop an ecosystem breakdown, which, in turn, sometimes influences the innocuous insects life to be harm ful being on humans, as like outbreak of PF dermatitis.

8.A Case of Multiple Appendage Tumors in Nevus Sebaceus.

Nevus sebaceus is a common hamartoma that has a variety of appendage tumors. We report a case of nevus sebaceus with multiple appendage tumors on the scalp of a 43-year-old male. The exudative inflamed patches were admixed with darkly pigmented small nodules which had developed secondarily within a 4.0 × 1.5cm lesion 2 years previously. Histologically, on serial sectioning, there were various types of basal cell carcinoma, syringocystadenoma papilliferum, tubular apocrine adenoma and sebaceous adenoma. Some parts of a biopsy specimen, showed a tumor of the follicular infundibulum-like epidermal changes. There were also calcium depositions in the stroma and apocrine tumors, There was no relapse at 1-year follow-up after surgical treatment.

9.Expression Pattern of bel-2 Protooncogene in Metastatic Prostate Cancer.

PURPOSE: Metastatic prostatic adenocarcinoma can be controlled by androgen ablation through the active process of programmed cell death in androgen responsive cells. However, about 20-30% of patients have no clinical response to androgen withdrawal. Because of the importance of apoptosis in effecting tumor control, factors involved in this process may be helpful in predicting androgen insensitivity. So, we evaluated the significance of bcl-2 protooncogene expression pattern with therapeutic response of prostatic cancer MATERIALS AND METHODS: We examined the cellular expression of bel-2 protein using immunohistochemical stain in tumor samples from 40 patients with metastatic prostatic cancer(stage D) and determined whether expression of blc-2 protein has related to the therapeutic response of prostatic cancer. RESULTS: The hormonal status of the patient's tumor was determined by the clinical response to therapy. Androgen independent cancer was defined as that subset of patients who experienced no initial response to androgen ablation, or who experienced disease relapse following an initial response to androgen ablation. So, we found that androgen dependent prostatic cancer was 22 patients and androgen independent prostatic cancer was 18 patients. The positive staining for bcl-2 was 27.3%(6/22) and 83.3%(15/18) in androgen dependent and independent prostatic cancer, respectively. It was significant difference(p<0.05). CONCLUSIONS: These results suggest that bcl-2 expression is associated with androgen independent prostatic cancer and used one of the factors to predict which patient with prostatic cancer will respond to androgen ablation.
