1.A study on the applicability of polymer(polyglactin)for the artificial dermis.

The result of artificial skins made with collagen is poor after grafting over the full thickness wounds due to their rapid degradation by enzymatic cleavage. This study is an in vivo study of an artificial skin made with a biodegradable polymer, which can better address the problem of the collagenous artificial dermis. To investigate the availability of a biodegradable polymer for an artificial dermis and to get an information about the optimal degradation rate of a polymer for an artificial dermis, we made an artificial dermis by seeding of fibroblasts within the vicryl mesh and made a bilayer artificial skin by covering the artificial dermis with cultured keratinocytes. And these artificial dermis and artificial skin were evaluated in a full thickness wound model. The results are as followings: 1. The artificial dermis was available for grafting for 1 week culture of vicryl mesh-fibroblast. 2. The artificial dermis retarded the contraction of full thickness wounds. 3. The artificial dermis generated the granulation tissue and accepted the STSG completely. 4. The generated tissue from the artificial dermis had incorporated into the surrounding tissue by 4 weeks postgrafting. 5. Vicryl in the artificial dermis became to biodegrade from the culture period and absorbed completely by 5 weeks. 6. The epidermal portion was poorly differntiated during in vitro culture period. In conclusion, the polymer-fibroblast graft can retard the wound contraction and generate a new tissue permitting a useful dermal replacement. And to get more optimal results, another polymer which has slower biodegradation rate than vicryl should be used for the artificial dermis and the epidermal portion should be differentiated after in vivo grafting.

Trichloroethylene is a very common chemical used principally as a solvent and a degreasing agent in industry, We have experienced a case of toxic erythema, toxic hepatitis, and subsequent exfoliative dermatitis possibly due to trichloroethylene in a 23-year-old male. He had dealed with a cleaning process with trichloroethylene in a factory of stainless steel materials. We report our case with the review of the toxicity of trichoroethylene.

4.A Case of First Branchial Cleft Anomaly Type II.

First branchial cleft anomalies constitute less than one percent of all branchial arch anomalies. First branchial clefi anomalies are classified into 2 digerent types with different embryologic, anatomical and histologic features by Work (1972)1. Among 2 types, type II anomaly is less common than type I and there has been no report of type II anomaly in the Korean literatures. We experienced a 5-year-old female who had a inflammed cystic mass over neck which was extended into parotid area After elevation of parotidectomy skin incision and superficial parotid gland, the cystic mass was observed to be extended upward into the parotid gland, medial to the facial nerve and ended at the external auditory canal. Microscopic examination shows that cyst wall is lined with squamous epithelium and contains some mesodermal tissue such as pilosebaceous gland and muscle. In conclusion, we can present this case as the first branchial cleft anomaly type II.

5.A Case of Syringoma Limited to the Vulva.

Vulva syringomas are rare in reviewing literature. In our case, a 29-year-old had multiple papular lesions involving both sides of the vulva. Microscopically the lesion showed dilated cystic sweat ducts some of which have small comma like tails resembling tadpoles. It is the purpose of this report to present a rare case of syringoma which was limited to the vulva without involvement of other sites.

7.Surgical Treatment of Sacrococcygeal Pilonidal Sinus using Wide Excision and Coverage with Fasciocutaneous Advancement Flap.

PURPOSE: Sacrococcygeal pilonidal sinus is a chronic inflammatory disease that mostly affects young people, which warrants surgical intervention. Although many surgical methods have been suggested, an optimal surgical method remains controversial because of high recurrence rates and postoperative complications. The objective of this study is to evaluate the results of wide excision and coverage with fasciocutaneous advancement flap for the treatment of sacrococcygeal pilonidal sinus, and to assess the usefulness of this method METHODS: From May 1995 to October 2006, the authors treated 19 patients with the use of coverage with fasciocutaneous flap after wide excision. The results were evaluated regarding recurrence rates, complications, and the change in sensitivity of the gluteal region after surgery. The follow-up period was 7 to 142 months (mean, 76 months). All patients were male. RESULTS: Postoperative complications were wound infection at the suction drain insertion site and skin necrosis around the suture margin in one case, respectively. During the follow up period, only one recurrence (5.3%) was seen in fifth postoperative month, which was successfully treated by the same operative procedure. There was no other complications such as seroma, hematoma, wound dehiscence and flap loss. Extensive scarring and anatomic distortion did not occur in the reconstructed area. In addition, sensitivity of the gluteal region did not diminish. The aesthetic results were satisfactory for all patients as well. CONCLUSION: The authors advocate that fasciocutaneous flap closure be a good alternative method to cover defects after the excision of sacrococcygeal pilonidal sinus.

10.Acute Compartment Syndrome Induced by Rhabdomyolysis Due to Antipsychotic Drug Overuse

A 49-year-old male was found unconscious at his accommodation and visited the emergency room. He was on antipsychotic and antidepressant drugs (vortioxetine hydrobromide, mirtazapine, sertraline hydrochloride, quetiapine, and alprazolam) for schizophrenia and major depression. At the time of discovery there were signs of overdose of the drugs around the patient. A physical examination revealed, pain, pallor, and edema in the left buttocks and lateral thigh. Active ankle movements below the left ankle were not possible and sensations in the tibia and peroneal nerves were lost. The pressure in the buttock compartment was measured at 42 mmHg. Magnetic resonance imaging revealed edema and high intensity signals in the left hip muscles and surrounding soft tissue. An emergency fasciotomy was performed and partial restoration of the lower extremity sensation and muscle strength were achieved after 24 hours.
