STUDY DESIGN: Thirty-eight patients with thoracolumbar and lumbar burst fractures were evaluated according to the treatment methods of the retropulsed fragments. OBJECTIVE: To confirm the pure effect of ligamentotaxis according to the approaching methods to tile involved area. SUMMARY OF BACK GROUND DATA: Retropulsed fragments were effectively treated by indirect posterior reduction. Regardless of size of the retropulsed fragments, remodelling process of the retropulsed fragments was progressed during follow-up period. METHODS: 38 patients with burst fractures of thoracolumbar and lumbar spine were divided into two groups according to tile reduction method of the retropulsed fragments ; the one group consisted of 18 cases which were treated by posterior instrumentation and indirect reduction(Group I: ligamentotaxis) and the other groups consisted of 20 cases which were treated by posterior instrumentation and posterior direct reduction(Group II). Change of neural canal compromise rate, neurologic status, remodelling process of the retropulsed fragments were measured using pre-, post-operative and follow-up radiographs and CT. RESULTS: Comparing the two groups, neural canal compromise rates were decreased at postoperative period but, there were no significant differences in both groups. There were no correlation of neurologic status and neural canal compromise rate. We could confirm the remodelling process of the retropulsed fragments, but there were no significant changes according to the size of the retropulsed fragments between two groups. CONCLUSIONS: Indirect reduction by posterior instrumentation( ligamentotaxis ) could effectively treat the retropulsed fragments in burst fractures of thoracolumbar and lumbar spine without directs posterior reduction.

Sclerosing polycystic adenosis (SPA) of the salivary glands is a rare entity analogous to fibrocystic disease of the breast. Less than 50 cases of SPA have been published in the literature. We report the first Korean case of SPA of the right parotid gland. A 34-year-old man presented with a slowly growing right parotid mass. Computed tomography showed a relatively well-demarcated, heterogeneously enhancing mass with multiple small calcifications. Fine needle aspiration showed cohesive sheets of epithelial cells with granular oncocytic cytoplasm and scattered lymphocytes. The parotidectomy specimen showed a 3 cm-sized solid nodular lesion with small cysts. Microscopically, the lesion was an unencapsulated mass of sclerotic fibrous tissue with cystic ducts, multiple calcifications, and lymphoplasma cell infiltration. Sclerosing adenosis and cystic ducts with frequent apocrine-like cells were noted. Familiarity with the cytologic and histological features of SPA is very important making the correct diagnosis.

Fifty cases of hepatocellularcarcinoma were studied using retrospective flow cytometric(FCM) and image cytometric(ICM) DNA analysis to determine the prevalence of aneuploid cell population and whether they were associated with any particular clinico-pathologic findings. At the same time, we compared the difference between FCM and ICM. The materials were prepared from 50 micrometer cut of paraffin embedded blocks. The DNA modal values, which could be defined in 42 cases by FCM with 74% aneuploidy and in 50 cases by ICM with 76% aneuploidy. So 95% of the cases had concordant DNA ploidy results by both techniques of FCM and ICM. Abnormal DNA pattern was correlated with age(<50), presence of cirrhosis, pathologic grade and some pathologic types(p<0.05), but was not correlated with presence of HBsAg, sex, alphafeto protein, and alcohol history(p>0.05). Also we found that ICM technique was easier to perform and interprete.

Oxidative stress has been considered to be a major contributor to the pathogenesis of the diabetic macrovascular and microvascular complications. In the absence of an appropriate antioxidant defense mechanism, increased oxidative stress leads to the activation of stress-sensitive intracellular signaling pathways and the formation of gene products that cause damage and contribute to the late complications ofdiabetes. The source of reactive oxygen species (ROS) in the pancreatic beta cells and insulin sensitive cells has postulated to be the mitochondrial electron transport chain. NAD(P)H oxidase-dependent ROS production is also important as the source both in pancreatic beta cells and other cells. NAD(P)H oxidase mediated ROS can alter parameters of signal transduction, insulin secretion, insulin action, cell proliferation and cell death. Additionally, oxidative stress as the pathogenic mechanism linking insulin resistance with dysfunction of both pancreatic beta cells and endothelial cells, eventually leads to diabetes and its complications. Further investigation of the mechanisms and its therapeutic interventions based on focusing NAD(P)H oxidase associated ROS production in the islet cells and other islet cells are needed

PURPOSE: Authors reviewed CT and MR findings of patients with parapharyngeal masses, and their incidence and characteristic findings were studied. MATERIALS AND METHODS: We reviewed 27 CT and 9 MR of pathologically proven 29 patients with primary parapharyngeal mass Neurogenic tumor (17 cases) and other rare masses (3 carotid body tumors, 2 pleomorphic adenomas, 2 cavernous hemangiomas, 1 cavernous lymphangioma, 2 carotid artery pseudoaneurysms, 1 non-Hodgkin's lymphoma, 1 idiopathic cervical fibrosis) were included and the characteristic CT and MR findings were studied. RESULTS: Neurilemmoma was seen as a well-demarcated, solid mass of heterogeneous attenuation or intensity, so that pleomorphic adenoma of minor salivary gland origin could not be differentiated by demarcation, attenuation, or signal intensity of the tumor itself. The direction of carotid artery displacement was not constant in cases of neurilemmomas while the styloid process was constantly displaced laterally in all cases. Characteristic CT and MR findings of other rare tumors were described. CONCLUSION: Neurogenic tumor is the most common parapharyngeal space mass and the direction of displacement of styloid process might be helpful in differential diagnosis from pleomorphic adenoma. Other rare tumors showed very characteristic CT and MR finidings and could be differentiated easily.

PURPOSE: Neonatal cranial sonongraphy performed through the mastoid fontanelle is more useful to evaluate the peripheral structures at the convexity of the cerebral hemispheres and brainstem rather than that performed through the anterior fontanelle. The purpose of this study is to demonstrate the anatomy of the extracerebral CSF space and brainstem and to suggest appropriate scan planes for performing neonatal cranial sonography through the mastoid fontanelle using MRI and multiplanar reconstruction programs. MATERIALS AND METHODS: A neonate with normal features on ultrasonography and good image quality on MRI, including the 3D-SPGR axial scans, was selected. We made the reconstructed MR images corresponding to the sonongraphic planes and the anatomic models of the neonatal cranial sonographic images by using axial MRI as the standard reference on the same screen. We demonstrated the sonographic images at the levels of the body of the caudate nucleus and lentiform nucleus, the head of the caudate nucleus and thalamus, the third ventricle and midbrain, and the midbrain and cerebellar vermis on the oblique axial scans. Four oblique coronal images at the levels of the periventricular white matter, basal ganglia, thalamus and tentorium were also obtained. RESULTS: We illustrated the anatomic atlas with including four oblique axial scans and four oblique coronal scans that corresponded to the neonatal cranial sonographic images through the mastoid fontanelle. CONCLUSION: We objectively analyzed the anatomy of the extracerebral CSF space and brainstem by using MRI and multiplanar reconstruction programs and we provided the standardized sonographic scan planes through the mastoid fontanelle. This study will be very helpful for evaluating the abnormalities of the peripheral structures at the convexity of the cerebral hemispheres and brainstem.

The methods of treatment of spondylolisthesis range from posterior fusion to instrumentation, and circumferential fusion. Combined anterior and posterior fusion with instrumentation had been carried out in one or two stages and satisfactory results were obtained in nighteen cases with follow-up period of one year to four years at department of orthopedic surgery of Ewha Womans University Hospital from 1987 to 1991. The results were as follows: 1. Of 19 patients, 7 patients were male and 12 patients were famale. 2. The isthmic type was 12 cases (63.1%) and the 5th lumbar vertebral involvement is common (12 cases, 63.1%). 3. The operative treatment was as follows: Posterior fusion with instrumentation was 8 cases, and anterior fusion with instrumentation was 11 cases. 4. The combined anterior and posterior fusion with instrumentation had been carried out in one stage operation at 8 cases, and two stage operation was done at 11 cases. 5. The average operation time was 3.0 hrs in one stage operation, 3.1 hrs in two stage operation, and the average amount of blood loss was 22cc/Kg in one stage, 17cc/Kg in two stage. 6. The satisfactory results were obtained in 19 cases, but there was no specific difference between one stage and two stage operation.
