1.Ultrastructural Study on Basement Membrane Thickening of Iris Capillaries in Diabetic Patients.

The early ultrastructural changes of basement membrane thickening and degeneration of the pericyte were noted in retinal capillaries in diabetic patients. However no early ultrastructural changes were reported in diabetic iris capillary. The authors studied the ultrastructural features of basement membrane of iris capillaries which were obtained during cataract surgery in five diabetic patients and compared with those obtained in five nondiabetic patients. The endothelium basal lamina, pericyte basal lamina and membrane-like pericyte matrix in didabetic iris capillaries were thicker than those in nondiabetic Iris capillaries. The thickening of pericyte basal lamina is more prominant than the endothelium basal lamina in diabetes. The duration of diabetes had no relationship to endothelium basal lamina but, showed statistically significant relationship to thickness of pericyte bascal lamina and pericyte matrix.

2.A Study of Mucocutaneous Manifestations Due to Cancer Chemotherapy.

BACKGROUND: Recently, inireaing numbers of chemotherapeutic agens are being used to treat cancer patients. Mucocutaeous complications are commonly sen in association with the administration of these medicaticins. OBJECTIVE: We conducted a study in cancer chemotherapy patient so determine the suspected chemotherapeutic agents ancl frequency of various mucocutaneous side effects in these patients. METHODS: The study involved 140 patients admitted from Decerrae 1993 to September 1994 for cancer chemot herapy at Korea University Guro Hospital. RESULTS: The mucocutaneous side effects during chemotherapy were alopecia(55%), hyperpig mentation(32.9%), stomatitis(20%), phlebitis(12.9%), flushing(8.6%) descending order. Visual grade II, IV alopecia patients were most common, but as the chemot retpy cycle increased so, the severer the alopecia. Although the onset of the alopecia were variake many patients experienced alopcia within 17 to 24 days after the start of chemotherapy. Serpeitie supravenous fluorouracil hyperpigmentation were commor,(17.9%), Diffuse and longitudinal or horizontal band like nail pigmentation were observed, and, pigmented macules were also observei, epecially on the palms and digits. Stomatitis usually developed within 10 days after the start of chemotherapy and it persist ed for about 7 days and the most of the lesions were resolved spor Laeously. Other clinical manifestations accompanied with chmotherapy were tinea infection(16.4%), acne(7.1%), oral thrush (7%), white nail band(4.3%), pruritus(2.9%) in descending order. CONCLUSION: So, physiciars need to be aware of the widely divergnal cutaneous reactions which may occur with the use of antiancer medications.

5.Radiological Analysis of Aging Changes of the Lumbar Intervertebral Disc.

The present study was carried out to clarify the anatomical changes of lumbar intervertebral discs by aging. Anterior height, posterior height, anteroposterior diameter of intervertebral discs were measured on 512 normal plain lateral radiographs of lumbosacral spine. And the indices of disc wedging and relative disc height were calculated. There was a cephalocaudal gradient of increase in the indices of disc wedging in all age groups. The indices of relative disc height were constant at all lumbar levels. These suggest that the lower disc is more wedge shaped and the height of discs changes in the constant ratio with that of vertebral body.

6.Dapiprazole for the Reversal of Mydriasis.

Some patients are very sensitive to glare after diagnostic mydriasis with tropicarnide or phenylephrine. It would be desirable to reverse the effects of mydriasis rapidly and safely after diagnostic mydriasis. We assessed the effect of topical dapiprazole, an alpha adrenergic receptor blocker, in reversing mydriasis in 40 dark brown Iris subjects who received tropicamide 1% or phenylephrine 2.5%. There was a significant difference in the decrease in pupil size between the eyes that received dapiprazole and the control eyes. This study suggests that dapiprazole 0.5% eye drops are effective and safe in reversing mydriasis after instilation of tropicamide 1% or phenylephrine 2.5% into dark brown irides.

7.Causes and Trauma Apportionment Score of Chronic Subdural Hematoma.

OBJECTIVE: The pathophysiology of chronic subdural hematoma (CSH) is not yet clear. Trauma alone is not sufficient to result in CSH in young individuals, while a trivial injury can result in CSH in older adults. Although the causality and apportionment of trauma are important issues in CSH, especially in terms of insurance, it is too obscure to solve all struggles. METHODS: There are three key factors for producing CSH. First, CSH necessitates a potential subdural reservoir. Other important precipitating factors are trauma and coagulopathy. However, these factors are not sufficient to cause CSH development. The trauma apportionment score (TAS) can be used to compare the relative importance of these three factors. Here, we applied the TAS to 239 consecutive cases of CSH. We retrospectively obtained the patients' history and laboratory results from their medical records. RESULTS: The TAS ranged from −5 to 5. The most common score was 0. If we defined the cause of CSH as being combined when the TAS was 0, then the cause was combined in 30 cases (12.6%). If we extended the criteria for a combined cause from 0 to −1 to 1, the cause was combined in 107 cases (44.8%). Regardless of the criteria used, traumatic CSHs were more common than were spontaneous CSHs. Spontaneous CSHs were more common in older than in younger patients (p < 0.01, Fisher's exact test). CONCLUSION: The TAS is a useful tool for differentiating the causality of CSH.

8.A Case of Stenotrophomonas Maltophilia Endophthalmitis after Cataract Operation.

It has been reported that the cause of the bacterial endophthalmitis following cataract extraction and intraocular lens implantation was G(+) organisms in most cases. However, the endophthalmitis caused by G(-) organisms has worse prognosis than that by G(+)organisms. We experienced a case of Stenotrophomonas maltophilia endophthalmitis which has not yet been reported in Korea. A 77-year-old female presented with blurred vision, conjunctival injection and ocular pain 5 days after cataract extraction and intraocular lens implantation. Ocular findings such as corneal edema, hypopyon and thick posterior capsular opacity were consistent with infectious endophthalmitis. Culture of vitreous aspirate isolated Stenotrophomonas maltophilia 15 days after cataract extraction.

10.Surgical Fixation of Sacroiliac Joint Complex in Unstable Pelvic Ring Injuries.

PURPOSE: To evaluate the effectiveness and clinical outcomes of surgical fixation of a sacroiliac joint complex in unstable pelvic ring injuries. MATERIALS AND METHODS: We selected sixteen cases in our hospital from 2006 to 2010 that underwent surgical fixation of the sacroiliac joint complex and had unstable pelvis ring injuries corresponding to B and C of Tile classification. Plate fixation through an intra-pelvic anterior approach was performed for 9 cases, tension band plate fixation for 2 cases, iliosacral screw fixation for 4 cases, and spino-pelvic fixation through extra-pelvic posterior approach for 1 case. Radiological and clinical evaluations were implemented to determine the results of treatment. RESULTS: Bone union was observed in all patients. For radiological evaluation using the Matta and Saucedo criteria, 15 cases were above the nearly-anatomic reduction. Out of 10 cases that performed rotational displacement analysis, 9 cases were above fair. All cases had above moderate clinical results as well. CONCLUSION: For unstable pelvic ring injuries, the satisfactory radiological and clinical results have been obtained through the anatomical reduction of the sacroiliac joint complex and firm internal fixation. And from the evaluation of fracture types, the Type C fracture and vertical shear type fractures showed relatively poor results.
