Calcaneus is the largest tarsal bone and serves weight bearing of the body. The calcaneus has a pattern of cancellous bone enclosed with a very thin cortical shell. It is difficult to reduce the bony fragments accurately when there are comminution and displacement on the calcaneal fractures. Retrospective study was carried out of 18 patients(21 feet) of calcaneus fractures after axial fixation in Soonchunhyang University Hospital from July, 1982 to July, 1988. Minimum follow up period was 8 months(average 2 years 6 months). The clinical analysis of them are reported with a review of the literature and the results are follows:l. According to the Rowe's unit system, the results of treatment were excellent or good in 9 cases of 13 tongue types and 5 cases of 8 joint depression types. 2. The results were satisfactory in the calcaneal fractures of well reestablished Bohler angle to 21-30 degrees. 3. The results were excellent or good in 5 cases operated within post-trauma 1 week and in 9 cases operated from 1 week to 2 weeks. 4. The results were poor in the 2 joint depression type with severe comminution. And subtalar joint fusion was performed on the previous 2 cases. Then pain on the lateral malleolar area was subsided.

BACKGROUND: The pediatric CPR is rather uncommon practice than adult CPR in emergency room. Therefore, few interest and studies were focused in pediatric CPR. The object of this study is to make an analysis about pediatric CPR performed within hospital setting. METHOD: From January 1. 1990 to December 31. 1996, a total of 59 patients, less than 15-year-old, who received cardiopulmonary resuscitation were subject of this study. The study was done retrospective chart analysis for sex, age, weight, the place of cardiac arrest, initial EKG rhythms, endotracheal tube size, the causes of arrest, CPR time, ROSC, the amount and types of CPR drugs used during resuscitation. Wilcoxon rank sum test and chi2 test were used to compare the dose of drugs during CPR in each group. RESULTS: There were no statistical difference between ROSC and non-ROSC group in place of arrest, initial EKG rhythms, CPR time, dosage of CPR drugs. CONCLUSION: The uniform reporting guideline of pediatric advanced life support should be used for future pediatric CPR study, such as Utstein style guideline in pediatric CPR.

Journal of the Korean Ophthalmological Society 1993;34(3):235-238
Horizontal incomitancy is a difference of deviation angle between right and left gaze and monocular recession-resection operation may cause horizontal incomitancy. We reviewed 85 patients of intermittent exotropia without oblique dysfunction and vertical deviation who underwent monocular recession-resection operation. The preoperative horizontal in comitancy was 1.0 delta in average with a range of 0 delta to 5 delta. The postoperative horizontal incomitancy after 7 months (mean) was 4.8 delta in average with a range of 0 delta to 17 delta. The postoperative remained deviation and horizontal incomitancy were significanty smaller in the group of patients who had fusion with worth 4-dot test (p

Infection in primary total hip arthroplasty may cause catastrophic results and is the major reason for implant failure. The purpose of this study was to evaluate the utility of the hip joint fluid culture as a method of predicting the possibility of a hip joint infection by calculating the sensitivity, specificity and accuracy. We performed 628 primary total hip arthroplasty and aerobic and anaerobic bacterial cultures for hip joint fluid between January 1989 and June 1996. The hip joint fluid culture was routinely performed to evaluate the utility of the femoral head for bone banking. Thirty-two cases out of the 628 hips showed positive intraoperative culture and 596 cases showed negative intraoperative culture. The isolated organisms from 32 positive cultures were 11 for Staphylococcus aureus, eight for Staphylococcus coagulase negative, seven for Enterococcus, three for E.coli and one each for Enterobacter, Acinetobacter and Pseudomonas. Anaerobic culture was negative in all cases. In the positive intraoperative culture cases, none had delayed infection during the follow-up period. But in the negative intraoperative culture cases, one case had acute infection and two cases had delayed infection. In the case with acute infection, Staphylococcus aureus was isolated and on two cases with delayed infection, Enterococcus and Staphylococcus coagulase negative were isolated, respectively. All 32 positive culture cases were fa~lse positive and 595 negative culture cases were true negative and one negative culture case was fa~lse negative. The sensitivity of the hip joint culture was 0%, the specificity was 94.9% and the accuracy was 0%. The specificity of hip joint fluid culture in primary total hip arthroplasty was high, hut the sensitivity score was zero. Therefore, the hip joint fluid culture should not be used for a routine check of infection status in primary total hip arthroplasty. We recommend the hip joint fluid culture in revision arthroplasty or hips in which infection is clinically suspected.

The Journal of the Korean Orthopaedic Association 1990;25(1):258-261
In 1941 Wiberg discribed morphologic classification of patella in axial plane and the correlationship between its shape and dysfunction. Here the authors reviewed 120 cases and divided them into four groups. Group I is the one with no knee joint pain or patella dysfunction, group II is the patients with meniscus injury, gruop III is the patients with chondromalacia of patella and group IV is the patients with osteoarthritis of knee joint. We analysed them according to the ratio between the length of the patella and the articular surface in lateral X-ray films of patella. The results were as follows:1. The ratio between the length of the patella and the articular surface is 1.45±0.13(mean±SD) in group I, 1.47±0.18 in group II, 1.56±0.12 in group lll and 1.57±0.15 in group IV. 2. Statistically(P<0.05), no significant difference between group I and group II, but group III and group IV is significantly different from the group I. 3. The ratio between the length of the patella and the articular surface is thought to be helpful for diagnosing the patient with vague knee joint pain.

The Journal of the Korean Orthopaedic Association 1990;25(2):353-358
The various methods of treatment of the supracondylar fractures of the femur were adapted by many authors according to degree of the involvement of the articular surface, displacement of fragments, degree of comminution and external forces. Authors experienced the supracondylar fracture of the femur in 31 cases who were treated at Koryo General Hospital from March 1984 until March 1988 and among them, 14 cases were treated by operative methods and 17 cases by conservative methods. The results were as follows:1. The male was more common, and the 3rd & 4th decades were most. 2. The most common cause of injuries was traffic accident. 3. The most common fracture type was type I by Neer's classification. 4. The type I was treated mostly by conservative methods and the type II & III by operative methods. 5. In the operative treatments, after anatomical reduction and rigid fixation, the duration of the rehabilitation was shortened by using the C.P.M. exerciser. 6. The complications were partial ankylosis of the knee, chronic osteomyelitis, nonunion and delayed union.

Journal of the Korean Radiological Society 1993;29(1):14-19
A retrospective analysis on the MRI findings of 24 pathologically contirmed craniopharyngiomas was performed with emphasis on signal intensity of cystic portion and its chemical components. Axial, coronal MR scans were ontained, and in six patients, post-contrast study was done with Gadopentetate Dimeglumine(Gd-DTPA). In 14 cystic portions were aspirated to correlate the amount of chemical components such as calcium, glucose, protein, cholesterol, lipid, and iron with the signal intensity GT scans were also performed in 20 patients. In eight cases, tumors extended to subfrontal, third ventricle, and retrosellar area the extensions were well demonstrated in sagittal and coronal images. The signal intensity of cystic lesions on T2WI were variable but tended to be higher in high protein and iron contents. Calcifications were present in 20 cases and out of these, MRI detected calcification as very low signal intensity or signal void on T1WI and T2WI in 12 cases. Multiplanar imaging of MRI was very useful for the evaluation of tumor extension and the signal intensity of the cystic lesion tended to be higher on T1WI when the contents had high protein or iron components.

Korean Journal of Urology 1996;37(3):281-285
Transurethral resection of Prostate(TURP) is the current optimal therapy for benign prostatic hypertrophy. But in some cases of BPH patients, TURP cannot be performed because the patients have medical disorders not to be operated. Thus several alternatives have been used in the management of BPH patients, especially who had anesthetic contraindications to an operation. We experienced 12 cases of BPH patients applied with Intraurethral Cath(IUC) from April 1991 to September 1995. Eleven patients became able to void after IUC insertion. The maximal flow rates were 5.1 to 15.7 ml/sec and average value was 11.8 ml/sec. Almost all patients showed irritative voiding symptoms such as urgency, urge incontinence, frequency, and dysuria at first. In 7 patients, these symptoms disappeared spontaneously within 2 to 5 days and oxybutynin chloride was used orally in 2 patients. Suprapubic cystostomy was done in three patients because two patients had severe irritative voiding symptoms and one patient had persistent urinary retention. The stent was changed every 3 to 6 months. Stone formation around the device was noted in 2 patients and migration of device into the bladder was found in 2 patients. In 4 patients, TURP was performed when the patient's condition improved enough to be operated under epidural anesthesia. In conclusion, the use of IUC is thought to be a valid, cost effective and safe alternative to TURP in the treatment of highly operative risk patients with benign prostatic hypertrophy.
