1.Clinical Application of Acute Ischemic Stroke in Perfusion Computed Tomography.

Recent advent of 64-multidetctor (MD) CT enables more coverage of Z-axis in the perfusion imaging. The purpose of this study was to evaluate the clinical usefulness of perfusion CT by using 64-MD CT in detecting the lesion in patients with acute stroke. The perfusion CT was performed by using 64-MD CT in 62 consecutive patients who were initially suspected to have subacute ischemic stroke symptoms during the period of recent 9 months. These patients had subacute stroke (n=62). CT scanning was conducted with Jog Mode which provided 16 imaging slices with 5 mm of slice thickness, and 8 cm of coverage in Z-axis. Scan interval was 1 seconds for each imaging slice and total 15 scans were repeated. After CT scanning, perfusion maps (CBV, CBF, MTT and TTP) were created at Extended Brilliance Workstation. The CBV and CBF maps showed that lesions were smaller images. While on the MTT and TTP map lesions were seen to be larger fifty-one were large than they appeared on these images. Two slices of perfusion maps obtained at the level of the basal ganglia were chosen to simulate conventional older perfusion CT with 8 cm of coverage in Z-axis. TTP and MTT maps may be clinically useful for evaluation of the penumbral zone in cases of aubacute cerebral ischemic stroke. The perfusion CT is useful in the assessment of acute stroke as an initial imaging modality.

2.Bone infection associated with pressure sores: a clinical study.

Pressure sores are common complications in patients with spinal injuries or cerebrovascular accidents. The management of pressure sores requires prolonged hospitalization and repeated surgical interventions with a high recurrence rate. Particularly osteomyelitis following initial pressure sores persistently complicates wound healing and recovery. Furthermore, it may also ultimately result in serious sepsis; thus an early diagnosis of osteomyelitis in pressure sore patients is crucial along with appropriate surgical and antibiotic therapy. Although many diagnostic methods were reported, there is no single 'gold' standard for early diagnosis of pressure sore-associated osteomyelitis. Authors reviewed pre-operative CBC, plain X-ray, Tc-99m bone scan, and post-operative biopsy results in 37 patients who had received surgical treatment for pressure sores in a 5-year period from September 1991 to August 1996. Based on these reviews, authors compared and analyzed the sensitivity and specificity in diagnosing osteomyelitis.The results were as follows: 1. Of 37 patients studied, 25 cases were confirmed histologically as osteomyelitis. Ischial region revealed the highest incidence of pressure sore-related osteomyelitis(78%, 14 out of 18 cases) while the regional incidence was as following in descending order : ischial, sacral, and greater trochanteric area. 2. Tc-99m bone scan had superior sensitivity(100%) and specificity(83%) to other pre-operative studies in diagnosing osteomyelitis.In conclusion, authors suggest that Tc-99m bone scan is the best diagnostic method because of its high sensitivity and specificity if osteomyelitis is clinically suspected in pressure sore patients. And in case the test result is positive, agressive surgical bone resection and appropriate antibiotic therapy based on bacterial culture from deep bone specimen should be combined to prevent complications or recurrences.

3.MTF Evaluation and Clinical Application according to the Characteristic Kernels in the Computed Tomography.

Our objective was to evaluate the clinical feasibility of spatial domain filtering as an alternative to additional image reconstruction using different kernels in CT. Kernels were grouped as H30 (head medium smooth), B30 (body medium smooth), S80 (special) and U95 (ultra sharp). Derived from thin collimated source images, four sets of images were generated using phantom kernels. MTF (50%, 10%, 2%) measured with H30 (3.25, 5.68, 7.45 lp/cm), B30 (3.84, 6.25, 7.72 lp/cm), S80 (4.69, 9.49, 12.34 lp/cm), and U95 (14.19, 20.31, 24.67 lp/cm). Spatial resolution for the U95 kernel (0.6 mm) was 33.3% greater than that of the H30 and B30 (0.8 mm) kernels. Initially scanned kernels images were rated for subjective image quality, using a five-point scale. Image scanned with a convolution kernel led to an increase in noise (U95), whereas the results for CT attenuation coefficient were comparable. CT images increase the diagnostic accuracy in head (H30), abdomen (B30), temporal bone and lung (U95) kernels may be controlled by adjusting CT various algorithms, which should be adjusted to take into account the kernels of the CT undergoing the examination.

4.A Clinical and Radiological Study of the hallux Valgus Angle , Intermetatarsal Angle and Hallux Valgus of Koreans

The first ray, including the hallux and first metatarsal, has historically been the most common location of congenital and functionally acquired deformities of the lower extremity. It has been the subject of extensive historical review and study as an untold number of authors and surgeons have directed their talents toward the comprehension and correction of the condition. But, few reports have been made on that subject in Korean population. So, we made a clinical and radilogical study of the hallux valgus angle (HVA), intermetatarsal angle (IMA) and hallux valgus in 200 Korean population. The results obtained were as follows: 1. The mean value of HVA was 14.8°±4.6° in male and 17.6°±5.4°in female. The mean value of HVA of the hallux valgus patient was 33° 2. The mesn value of IMA was 8.6°±1.9°in male and 10.2°±2.2°in female. The mean value of IMA of the hallux valgus patient was 14° 3. The incidence of hallux valgus was 11% in male, 37% in female and female was 77% of all hallux vslgus patient. 4. The family history of hallux valgus was found in 63% and 94% of patients has bilateral involvement. 5. The most common chief complaint was pain over the medial side of the first MP joint or base of the other metatarsal bone. Symptoms were not correlated with angle value. 6. In the first MP joint of hallux valgus by Piggott, the deviated joint was most common (52%). 7. In the three basic MP joint types by Haas, the round metatarsal head was most common. 8. DASA was ranged from 0°to 7°in normal persons and from 2°to 16°in hallux valgus patients. 9. PASA was ranged from 1°to 9°in normsl persons and from 3°to 35°in hallux valgus patient. 10. The tibial sesamoid displacement more than grade 4 by Haas rating system was found 63% of hallux valgus patients.

A rare case of traumatic bilateral anterior and posterior dislocstions of the hip wss occured by autamobile accident as a passenger injury. The right hip was dislocated anteroinferior to the righ scetabulum(neer the right obturator froamen), and the left hip was dislocated posterosuperior to the left acetsbulum with a ipsilateral femur head fracture caudsd to the fovea centralis(Fig. 1).

6.Risk Factors of Dislocation Occurring after Acetabular Component Revision.

Purpose: To analyze the causes and to prevent dislocations of the hip joint that occur in patients who underwent revisions of total hip arthroplasties by changing only the polyethylene liners and femoral heads, subsequent to primary total hip arthroplasties. Materials and Methods: We evaluated 28 patients who underwent acetabular component revisions of total hip arthroplasties subsequent to primary total hip arthroplasties. The average age of the patients was 55 years old and all 28 patients had operations through the posterolateral approach. In 17 of the cases, the acetabular cups, polyethylene liners, and femoral heads were changed; in 10 cases, just the polyethylene liners and femoral heads were changed; and in 1 case, only the polyethylene liner was changed. Results: We observed 7 cases of hip dislocations that occurred among a total of 28 cases after revisions of total hip arthroplasties. In all 7 cases, the polyethylene liners and femoral heads were changed, but not the acetabular cups. In additional, in all 7 cases of dislocation, the patients were non-compliant and started weight-bearing prematurely after revisions of the total hip arthroplasties Conclusion: We observed new dislocations in the 7 cases in which the polyethylene liners and femoral heads were changed. The main reasons were inadequate soft tissue tension and hip joint laxity. Therefore, it is necessary to increase the neck length, use elevated rim polyethylene liners, readjust the acetabular cups to their optimal positions, and apply hip abduction braces to patients early after revision of total hip arthroplasties in order to lessen the danger of dislocations.

8.A Case of Angioleiomyoma in Nasolacrimal Duct

Epiphora is a symptom in which tears overflow onto the face, with its most common cause being the obstruction of the nasolacrimal system. As a cause of nasolacrimal system obstruction, nasolacrimal duct tumors are very rare. Angioleiomyoma, which is a benign tumor, arises from vascular smooth muscle, occurs in the nasolacrimal duct, and is extremely rare. With the development of endoscopic intranasal approach for the treatment of nasolacrimal duct obstruction, there is increased importance for the consideration of otorhinolaryngological evaluation and treatment as well as dacryocystorhinostomy by conventional external approach. In this article, we introduce a case in which angioleiomyoma in the nasolacrimal duct was detected by nasal endoscopy and was successfully treated with endoscopic surgery.
9.A Case of Visual Loss Caused by Invasive Fungal Sinusitis of the Onodi Cell

Invasive fungal sinusitis is common in immunodeficiency patients and can spread into the orbit or intracranial cavity. The Onodi cell, which is one of the anatomical variations of the ethmoid sinus, refers to the space that has been pneumatized superolateral to the sphenoid sinus. We experienced a case of invasive fungal sinusitis that caused vision loss by invading the Onodi cells. Endoscopic sinus surgery and antifungal treatment successfully recovered the patient’s vision and we herein report on the case with a review of the literature.
