1.A Case of Kaposi's Varicelliform Eruption Occuring in Pemphigus Foliaceus.

This report presents a case of a 17-year-old-girl who developed a, Kaposis varicelliforrn eruption(eczema herpeticum) during oral prednisolone therapy in order to control pemphigus foliaceus. Diagnosis was made by the history, clinical feature and histologic examination. Within 24 hours after the initiation of therapy with ribavirin(Viramid), new lesions had ceased to develop.

2.Patients' Satisfaction after Reverse Total Shoulder Arthroplasty Is Affected by Preoperative Functional Status.

BACKGROUND: The purpose of this study is to evaluate the functional outcomes of reverse total shoulder arthroplasty (RTSA) and to assess factors affecting the patients' subjective satisfaction after RTSA. METHODS: Forty-three patients (mean age, 75.0 ± 5.2 years) who underwent RTSA for cuff tear arthropathy or irreparable cuff tears with preoperative magnetic resonance imaging and pre- and postoperative radiographs at 1 year, and whose various functional outcomes including pain visual analogue scale (VAS), simple shoulder test, Constant score, American Shoulder and Elbow Surgeons score, and active range of motion were evaluated preoperatively and at the last follow-up (>12 months) were enrolled. The outcome parameter was set as a satisfaction scale. Various clinical and radiographic factors were analyzed, and their correlations with postoperative satisfaction were evaluated. RESULTS: All functional scores, VAS pain score, and active forward flexion showed significant improvement after surgery (all p<0.001). Twenty-nine patients were satisfied with the results and 14 were dissatisfied. The presence of pseudoparalysis (p=0.028) and worse preoperative function (all p<0.05) were related with higher satisfaction. Any radiologic parameters did not affect patients' postoperative satisfaction. CONCLUSIONS: All patients showed a good functional outcome after RTSA, however the patients' subjective postoperative satisfaction was affected by preoperative functional status (higher satisfaction in poor preoperative function), not by radiological findings.

3.Physico-chemical and Microbiological Analysis of Tap Water in the Apartment in Taegu City.

The drinking water in apartment is stored in reservoir tank and thereafter supplied each household. In case the drinking water in stored too long or insanitary, tap water might change in quality. This study was carried out to evaluate the physic-chemical and microbiological conditions of 36 samples of tap water of apartments compared to 18 samples of tap water of independence houses in Taegu City from May to October, 1987. Main findings are summarized as follows: PH values were within normal range in all samples. Samples less than 0.2ppm in residual chlorine accounted for 16.7% in the apartment and 5.6% in the independence house. Nitrite nitrogen was detected 2.8% of apartment samples only. Among the total of 54 samples, 11.1% of the apartment and independence samples were over the normal value in standard plate count. Samples more than 1.8 of most probable number (MPN) accounted for 27.8% in the apartment and 33.3% in the independence house. In result of test for residual chlorine, nitrite nitrogen, standard plate count, and MPN, the proportion of the samples revealed inappropriate in respect of drinking water standards. In case of all sorts of tests was 2.7% in apartment and nothing in independence houses, in case of 3 sorts of tests was 2.7% in apartment and 5.6% in independence houses, in case of 2 sorts of tests was 8.3% in apartment and 5.6% in independence houses, and in case of only 1 test was 25.0% in apartment and 22.2% in independence houses. Correlation between residual chorine and standard plate count, and between standard plate count and MPN were observed. It was found that the more years since construction of apartment were, the worse tap water was in quality.

5.Enhancement of Thrombolytic Therapy by Transcutaneous Ultrasound and Perfluorocarbon Exposed Sonicated Dextrose Albumin in Thrombotic Arterial Occlusion.

BACKGROUND AND OBJECTIVES: Perfluorocarbon exposed sonicated dextrose albumin (PESDA) microbubbles has been suggested to facilitate thrombus disruption under the transcutaneous ultrasound (US). Thus, we investigated whether such a noninvasive approach could augment thrombolytic effect of fibrinolytic agent in an experimental thrombotic model. MATERIALS AND METHODS: Thrombus formation was induced with electrical injury in the rabbit iliofemoral arteries (n=20): Thrombus occlusion was documented by angiography in all arteries. In the control group, only tissue plasminogen activator (t-PA, 3 mg/kg) was administered intrav-enously in five rabbits. In the Group 1 (n=9), injured arteries were exposed to transcutaneous US (20 kHz, 30 W/cm2, continuous mode) with t-PA (3 mg/kg). In the Group 2 (n=6), the same treatment was given while administering PESDA continuously (10 ml/min, intravenous). Angiographic results were evaluated at 10 minute interval for 1 hour respectively. RESULTS: In the control group, two of five iliofemoral arteries (40.0%) were recanalized and one of nine iliofemoral arteries (11.1%) was recanalized in Group 1. In contrast, four of six iliofemoral arteries (66.7%) were recanalized angiographically in Group 2 (p=0.392 vs. control group: p=0.047 vs. Group 1). However, late reocclusion occurred in all iliofemoral arteries of Group 2. CONCLUSION: Although PESDA with transcutaneous US significantly enhanced initial angiographic patency rate of t-PA, it was associated with high rate of reocclusion. Further studies will be necessary for clinical application of this noninvasive method in acute arterial occlusion.

6.Craniometaphyseal Dysplasia: A Case Report.

Craniometaphyseal dysplasia is a rare genetic bone disorder characterized by cranial hyperostosis, sclerosis, and failure of normal modeling of the tubular bones. Both autosomal dominant and recessive forms have been described. Diagnosis of the craniometaphyseal dysplasia is mode on the basis of characteristic radiographic findings. Radiologic findings are thickening and hyperostosis of the skull, and Erlenmeyer flask deformity of the metaphysis on the long bones. We report a case of the craniometaphyseal dysplasia.

8.A case of mixed connective tissue disease.

We report a case of mixed connective tissue disease. A patient, 25-year-old woman, presented Raynaud phenomenon, proximal muscle weakness, sclerodactyly, fever, and diffuse alopeeia. In the serologic examination, antinuclear antibody and anti-RNP antibody were positiv but anti-native DNA antibody was negative. In direct immunofluorescent study of biopsy specimen of the skin, speckled epidermal nuclear staining of IgG and granular deposits of IgM at the dermoepidermal junction were seen: Electromyographic finding of the right deltoid and right gastrocinemius muscle was consistent with myopathy.

9.Five Cases of Acquired Port-Wine Stains.

Acquired port-wine stain is rare. It mimics a congenital port-wine stain morphologically but the pattern of onset is acquired after birth. It represents a progressive ectasia of vessels located in the superficial vascular plexus. The exact mechanism is unknown but some reported cases have occurred after trauma. We herein report five cases of acquired port-wine stains and they were idiopathic in nature.

10.Pachydermoperiostosis Accompanied by Hypertrophic Gastritis.

We report a patient with pachydermoperiostosis accompanied by hypertrophic gastritis. A 26-year-old man showed deep folds and furrows of the face and scalp, and terminal spade-like expansion of fingers and toes. Physical examination revealed no abnormalities except a grotesque appearance. Results of routine laboratory tests were unremarkable. Mild periosteal reactoins of both femurs and humeri were noted on long bone series, and hypertrophic gastritis on fibroscopic examination drew our attention. The relationship between pachydermoperiostosis and hypertrophic gastritis is uncertain. However it is probable that hypertrophic gastritis may be the endodermal counterpart of the ectodermal manifestations of pachydermoperiostosis.
