1.Eczema Herpeticum in Darier's Disease.

Eczema herpeticum is a herpes simplex virus infection with disseminated skin involvement superimposed on a pre-existing dermatosis. Dariers disease has been reported to be among the dermatoses susceptible to the sudden onset of a widespread vesicular eruption accompanied by high fever known as Kaposis varicelliform eruption. We report a case of eczema herpeticum in a 46-year-old woman associated with Dariers disease.

2.A case report on human infection with Anisakis sp. in Korea.

Authors experienced a case of human infection with Anisakis sp. in left tonsil. The case was 27 year Korean female who visited Department of Otolaryngology, Severance Hospital with the chief complaints of swollowing difficulty, and the sensation of a foreign body in the throat, on November 20th 1968. Physical examination of the oral cavity revealed that both palatin tonsil were chronically enlarged. A larval worm was found in crypt of the upper portion of the left palatin tonsil. Chief complaints of the patient was improved after removal of it by forceps. The greyish white living worm was fixed in lactophenol solution for morphological study. The worm measured 36 mm in length by 0.6 mm in width, 2.4 mm in esophageal length, 1.4 mm in ventricule, 0.1 mm in tail length, and 0.025 mm in caudal spine length, respectively and histological study was carried out from the serial section of the larva which was stained with hematoxylin eosin. Authors identified that the larva was Anisakis species.

4.A case of malignant Mixed Germ Cell-Sex Cord stromal Tumor of the Ovary in a Normal Female Adult.

The authors observed a case of malignant mixed germ cell-sex cord stromal tumor occuring in a 43-year-old woman. A laparotomy perfomed on August 3, 1994, revealed a large roughly round, partially ruptured tumor in the right ovary. The tumor mass weighed 330 gm and its dimensions were 11X9X8 cm. Tumors composed of germ cells and sex cord stroma intimately associated togther are very rare. Interest in these gonadal tumors that are distinct from gonadoblastoma has mouted in recent years. The tumor behaves in a benign clinical course and occurs predominantly in female infats and children with normal 46.XX karyoytype. This tumor differs from gonadobalstoma in its histologic pattern, absence of regresive changes, lack of association with other neoplastic germ cell elements, and occurrence in normal gonads.Histologically, the mass was characterized by an intimate admixutr of germ cells and sex cord stromal cells. The lesion had many predominant setoli tuble formations and lipid cell changes. But it lacked Call-Exner like bodies, calcification and byalinization that charaterize gonadoblastomas. Typical areas of dysgemonoma and endodermal sinus tumor were also present the patient subsequelty received chemotherapy(blemycin,etoposied, and cisplatin administered in 5cycles), The patient was symptom free two yares after surgery and chemotherapy.

5.Choroidal Metastasis of Adenocarcinoma of the Lung: A case report.

Choroidal metastatic carcinoma is very rare. We recently experienced a case of lung adenocarcinoma which presented to the clinic with ocular symptoms. This 57-year-old Korean male patient visited the department of ophthalmology due to decreased visual acuity and pain of the left eye. On MRI scan, a nodule was attached to the retina of the left eyeball. On simple chest radiograph, a large amount of pleural effusion was noted in the left pleural cavity. Emergency enucleation of the left eyeball was done with an impression of malignant melanoma causing an intractable ocular pain. Grossly, the lesion in the eyeball was rising from the choroid. On histologic examination, tumor cells formed many irregular, small gland-like structures. The tumor cells showed alcian blue-positive mucin in the cytoplasm and glandular lumens and were positive for CEA. Chest CT scan was performed postoperatively and showed a huge mass in the left lower lobe and multiple nodular opacities in both lung fields. Bronchoscopic biopsy revealed moderately differentiated adenocarcinoma similar to that of the eyeball.

6.The Prevalence of BPH in Busan City over Age 40.

PURPOSE: We studied 278 men who are over 40 years of age to educate BPH and to evaluate the prevalence of BPH in Busan city of Korea. MATERIALS AND METHODS: Between December 1997 and February 1998, 278 men in Busan city were investigated. The screening tests included urinary symptom questionnaire (IPSS), digital rectal examination (DRE), prostate specific antigen (PSA) and uroflometry. RESULTS: Based on the IPSS, 36.3%, 49.7% and 14.0% of the men were mild (1-7), moderately (8-19) and severely (20-35) symptomatic. The mean maximal flow rate and PSA for men 40 to 49, 50 to 59, 50 to 69 and over 70 years old were 18.4 ml/sec, 14.2 ml/sec, 12.9 ml/sec, 10.6 ml/sec and 1.081 ng/ml, 1.195 ng/ml, 1.527 ng/ml, 2.352 ng/ml respectively. CONCLUSIONS: The prevalence rate of clinical BPH(IPSS>7 and peak urinary flow rate<10 ml/sec) in community-based study of 278 men was 25.5%.

8.Rocuronium and Lidocaine Pretreatment for Prevention of Biochemical Changes, Fasciculations and Myalgia following Succinylcholine Administration.

BACKGROUND: The purpose of this study was to assess the changes in serum potassium and creatine kinase concentrations and the incidence of fasciculations and myalgia when rocuronium and lidocaine were used in combination and separately as pretreatment before succinylcholine. METHODS: We studied 60 patients undergoing a minor elective surgery, in a prospective double blinded method. Three groups each with 20 patients were pretreated before a 1.5 mg/kg succinylcholine administration with 0.05 mg/kg rocuronium three min before (group R), 1.5 mg/kg lidocaine 30 sec before (group L), or both rocuronium and lidocaine (group RL). Serum potassium and creatine kinase were respectively measured 5 min after succinylcholine administration and 24 h after the operation. Fasciculations and myalgia on postoperative day 1 and day 2 were evaluated. RESULTS: The increase in creatine kinase and incidence of myalgia on postoperative day 1 was less in the RL group than in the R group and L group. The incidence of fasciculations was higher in the L group than in the R group and RL group. There was no increase in serum potassium in any group. CONCLUSIONS: The combined use of rocuronium and lidocaine was more effective in reducing creatine kinase and postoperative myalgia than when they were used separately. However, the fasciculations were only reduced by the use of rocuronium.

9.Lacrimal Passages Injury Associated With Eyelid Burns.

PURPOSE: To develop a protocol for treatment of injuries to lacrimal punctum and canaliculus associated with eyelid burns. METHODS: We retrospectively reviewed the records of seven patients who required the treatment of punctal and canalicular injuries caused by medial eyelid burns. RESULTS: Four patients were treated with punctoplasty and bicanalicular silicone tube intubation within 3 weeks of injury. One patient was treated at 2 months and punctum of upper eyelid was not found, requiring punctoplasty and monocanalicular silicone tube intubation of lower eyelid. Silicone tube removal was performed over a range of 3 to 6 months. Two patients were treated after 3 months of injury by conjunctivodacryocystorhinostomy. Six patients showed good passage by syringing after surgery. CONCLUSIONS: Early evaluation and treatment of canalicular burn injuries can avoid complex surgical procedures such as conjunctivodacryocystorhinostomy, and silicone tube removal is recommended before 6 months after surgery.

10.A Comparative Study of Immunohistochemical Expression of p53, bcl-2, c-erbB-2, and MIB-1 in Polypoid and Infiltrative Colorectal Carcinomas.

Almost all colorectal carcinomas have been thought to develop from pre-existing adenomas. However, some colorectal carcinomas can arise directly from normal flat mucosa, and usually form infiltrative mass at the early stage. The carcinogenesis of this infiltrative carcinoma may be different from the well-known adenoma-carcinoma sequence, which usually forms a polypoid mass. The purpose of this study is to investigate the different expression of various oncogenes in polypoid carcinoma and infiltrative carcinoma. We performed immunohistochemical staining on p53, bcl-2, c-erbB-2 and MIB-1 in 29 polypoid carcinomas arised from adenomas, and 21 infiltrative carcinomas. The average tumor size of infiltrative carcinomas (5.5 cm) was larger than that of polypoid carcinomas (3.1 cm), and the polypoid carcinomas were differentiated more than the infiltrative carcinomas. The results of p53, bcl-2, c-erbB-2, and MIB-1 antisera immunoreactivity in the polypoid carcinoma were 79%, 17%, 21%, and 100%, and those in the infiltrative carcinoma were 71%, 29%, 29%, and 100%, respectively. However the diffuse positivities of p53 and MIB-1 antisera were slightly higher in the infiltraive carcinomas (62%, 76%) than in the polypoid carcinomas (55%, 41%) (p=0.63, 0.01). And the results of p53 and c-erbB-2 immunoreactivity in the adenomas were 52% and 17%, respectively, which is significantly lower than that in the polypoid carcinoma(p=0.03, 0.74). The immunoreactivty of bcl-2 in the adenoma was 72%, which was significantly higher than that in the polypoid carcinoma (17%) (p<0.01). In summary, we did not show the significant difference in expression of p53, bcl-2, c-erbB-2, and MIB-1 proteins between polypoid and infiltrative carcinomas. However, the tendency of infiltrative carcinomas having a more aggressive nature suggests another carcinogenetic mechanism is involved in the colorectal carcinogenesis.
