1.A Clinical Study of Antihypertensive Effects of Doxazocin(Cardura(R)) in Essential Hypertension.

The antihypertensive effect of Doxazocin were evaluated in 20 patients with essential hypertension. Two to eight mg of Doxazocin per day were administered continuously for two or eight weeks. The results were as follows : 1) Doxazocin were effective in 18 out of 20 patients(90%) at the end of 8 weeks treatment. 2) The systolic blood pressure was signigicantly decreased(173+/-20mmHg VS 144+/-33mmHg, P+/-0.001), and the diastolic blood pressure was also decreased(110+/-10mmHg VS 93+/-19mmHg, P<0.01) after treatment. 3) Triglyceride showed a decreasing tendency but statistically significance was not observed. Total cholesteral, HDL-cholesterol, BUN, creatinine, and uric acid did not change significantly before and after treatment with Doxazocin. 4) The side effects were headache in one case and dizziness in to cases. These results suggest that Doxazocin is an effective antihypertensive agent and concerning lipid profile, further long term observation is needed.

2.The Effects of Captopril(Capril(R)) on Early Diastolic Time Intervals in Dilated Cardiomyopathy.

Time intervals and indices of left ventricular diastolic filling were studied by M-mode and pulsed Doppler Echocardiography in 18 dilated cardiomyopathy patients. After one two hours captopril(capril(R)) 25mg medication. 1)Systolic blood pressure, Diastolic blood pressure, Heart rate were significantly decreased. 2) Dimensions of LVSD, LVDD, LA, Aorta and EF were not changed measured by M-mode echocardiography. 3) IRT measured by M-mode and Pulsed Doppler echocardiography were significantly increased(45+/-33msec VS 74+/-35msec. 84+/-32msec VS 100+/-22msec P<0.05). 4) A2-E measured by M-mode and Pulsed Doppler echocardiography were significantly increased(114+/-45msec VS 134+/-46msec, 156+/-46msec VS 194+/-48msec, P<0.05). 5) Deceleration time was significantly increased(137+/-36msec VS 205+/-40msec P<0.05). 6) Transmitral peak flow velocities were not changed significantly. In summary captopril influences early diastolic time intervals with dilated cardiomyopathy patients and we interpreted these effects be beneficial on diastolic filling pattern.

3.A Severe Case of Hemolytic Disease of the Newborn due to Anti-M Antibody.

Anti-M antibody is usually a naturally occurring antibody reacting optimally 4 degrees C and is not considered to be clinically significant. Rarely has anti-M been implicated in hemolytic disease of the newborn(HDN) and the true incidence of HDN due to anti-M has not been well delineated. Authors report the second case of hemolytic disease of the newborn due to anti-M in Korea. A 3-days old baby boy was admitted due to jaundice and severe anemia which were developed at birth. The blood type of his mother was A, CcDEe, Ns, while the blood type of the infant of was A, CcDEe, MNs. The mother's serum had anti-M which wits strongnly positive in room temperature and albumin phase. The reaction was only weakly positive in the antiglobulin phase. Direct antiglobulin test of baby's red cells was negative, while the serum was weakly positive in polyethylene glycol-Coombs test. The antibody was found to be partially IgG through the treatment with dithiothreitol. After an exchange transfusion and phototherapy, the anemia and jaundice were corrected and and the patient discharged at the age of 16.

4.A case of malignant Mixed Germ Cell-Sex Cord stromal Tumor of the Ovary in a Normal Female Adult.

The authors observed a case of malignant mixed germ cell-sex cord stromal tumor occuring in a 43-year-old woman. A laparotomy perfomed on August 3, 1994, revealed a large roughly round, partially ruptured tumor in the right ovary. The tumor mass weighed 330 gm and its dimensions were 11X9X8 cm. Tumors composed of germ cells and sex cord stroma intimately associated togther are very rare. Interest in these gonadal tumors that are distinct from gonadoblastoma has mouted in recent years. The tumor behaves in a benign clinical course and occurs predominantly in female infats and children with normal 46.XX karyoytype. This tumor differs from gonadobalstoma in its histologic pattern, absence of regresive changes, lack of association with other neoplastic germ cell elements, and occurrence in normal gonads.Histologically, the mass was characterized by an intimate admixutr of germ cells and sex cord stromal cells. The lesion had many predominant setoli tuble formations and lipid cell changes. But it lacked Call-Exner like bodies, calcification and byalinization that charaterize gonadoblastomas. Typical areas of dysgemonoma and endodermal sinus tumor were also present the patient subsequelty received chemotherapy(blemycin,etoposied, and cisplatin administered in 5cycles), The patient was symptom free two yares after surgery and chemotherapy.

6.The Degree of ZMother's Accident Prevention Practices for Their Children.

Accidents are the leading cause of death in children worldwide. The purpose of this study is to use basic data of safety education, counseling, and information available regarding accident prevention to examine the degree of mother's accident prevention practices for their children. Data were collected from June to October, 1998 from 587 mothers including 2 University hospitals located in Seoul and KyoungKi-Do. By using the 30 item questionnaire, which was created by researchers through literature review, the degree of mothers' accident prevention practices for their children was measured. The degree of accident prevention practices of mothers shown ranged 70-118 and averaged 3.19. Compared to the composit area average score, drug keeping had the highest score of 3.57. The next highest scores and traffic safety(3.41), and super vision of child(3.30). Prevention against burning was the lowest point at 2.58. With the respect to the demographic characteristics, there were statistically significant differences in the mothers' education(F=4.291, p=.014), type of residence(F=3.979, p=.008), and child developmental age(F=5.275, p=.001). The degree of accident prevention practices of mothers were relatively high. But the area which showed the low degree of accident prevention practices, required nurses' active education, counseling, social interest and support, and mass media participation.

7.Epidemiology of Leisure Sports-Related Ocular Trauma.

PURPOSE: We investigated the epidemiological and clinical characteristics of leisure activity- and sports-related ocular trauma for application to its treatment and prevention. METHODS: A retrospective chart review was conducted on 121 patients who visited Gachon University Gil Medical Center from January 2006 to December 2007 because of leasure activity- and sports-related ocular trauma. The incidence of ocular injuries, sex and age, monthly and seasonal distribution, causes, diagnosis, and visual acuity were reviewed. RESULTS: Among the 1592 patients of ocular trauma, 121 patients (7.6%) were related to leasure activity- and sports. The incidence was higher in males (81.8%; 4.5-fold) than in females. The mean age of patients was 23.7+/-7.1 years. Ocular injuries occurred least in winter (19.8%) and occurred more during the week than on weekends (1.6-fold). The most common cause was soccer (39.7%), more specifically being hit with the soccer ball (47.1%). The initial LogMAR visual acuity was worse in the females and in the patients older than 30 years. Traumatic hyphema (19.7%) was the most common cause of visual disturbance, and corneal erosion (11.3%) occurred frequently. CONCLUSIONS: Leasure activity- and sports-related ocular trauma is becoming an increasingly significant cause of ocular morbidity, and trends relating to characteristics such as sex and age were found. These outcomes can be put to practical use in the prevention and treatment of sports-relatedocular injuries.

8.Choroidal Metastasis of Adenocarcinoma of the Lung: A case report.

Choroidal metastatic carcinoma is very rare. We recently experienced a case of lung adenocarcinoma which presented to the clinic with ocular symptoms. This 57-year-old Korean male patient visited the department of ophthalmology due to decreased visual acuity and pain of the left eye. On MRI scan, a nodule was attached to the retina of the left eyeball. On simple chest radiograph, a large amount of pleural effusion was noted in the left pleural cavity. Emergency enucleation of the left eyeball was done with an impression of malignant melanoma causing an intractable ocular pain. Grossly, the lesion in the eyeball was rising from the choroid. On histologic examination, tumor cells formed many irregular, small gland-like structures. The tumor cells showed alcian blue-positive mucin in the cytoplasm and glandular lumens and were positive for CEA. Chest CT scan was performed postoperatively and showed a huge mass in the left lower lobe and multiple nodular opacities in both lung fields. Bronchoscopic biopsy revealed moderately differentiated adenocarcinoma similar to that of the eyeball.

9.Development of Toxic Anterior Segment Syndrome Immediately after Uneventful Phaco Surgery.

PURPOSE: We report on 15 cases of suspected toxic anterior segment syndrome after uneventful phaco surgery. METHODS: We retrospectively reviewed the charts of patients who had developed toxic anterior segment syndrome (TASS) after uneventful phacoemulsification for senile cataracts between April and December of 2005. Clinical features and all possible causes were investigated including irrigating solutions or drugs, surgical instruments or intraocular lenses, sterilization techniques for instruments, or any other accompanying disease. RESULTS: The patients consisted of 2 males and 13 females with an average age of 64.7+/-10.9 years. Five different surgeons had performed their phaco surgeries. No abnormal preoperative or operative findings were reported. Nevertheless, all 15 patients developed a moderate degree of corneal edema. Ordinary treatments were not helpful. We suspect that lack of sterilization resulted in the development of the syndrome, because after ethylene oxide gas sterilization was replaced with autoclaving, no such incidents have occurred. CONCLUSIONS: Toxic anterior segment syndrome requires special attention and thorough management, including sterilization of reused surgical instruments.

10.A Case of Chronic Pancreatitis Complicated by Retroperitoneal and Intrasplenic Pseudocysts.

A 69-year-old-male patient was admitted because of left upper quadrant (LUQ) abdominal pain He had hard palpable mass (abaut 15x l0cm) in LUQ abdomen and mild elevated serum alkaline phosphatase (349 IU/L) and amylase (216 U/L) levels at the admission time. Abdominal CT scan revealed huge, well-marginated cystic mass in enlarged spleen with increased densities in dependent portion of cystic mass, about 8x13cm in size. Another small well-capsulated. cystic mass in anterior aspect of spleen was also seen. After ERCP, urgent surgical intervention was done. Final diagnosis was chronic pancreatitis with large intrasplenic (l2x9.5 cm) and retroperitoneal pseudocysts. So we are reporting above case with pertinent review of literatures.
