BACKGROUND: Although several kinds of treatment methods(destructive therapy, immunotherapy, etc) for plane warts have been attempted but there have been no entirely satisfactory treatments, because the plane wants are seen to recur frequently. OBJECTIVE: This study was aimed to evaluate the combination therapy of griseofulvin and immunotherapy(dinitrochlorobenzene=DNCB, diphenylcycloprope none=DPCP) on plane warts. METHOD: Sixteen patients(age range, 9 to 41; mean age, 21.8 years) with VPJ were treated with the combination therapy with griseofulvin(500mg/day) and single contact immunotherapy(DNCB or DPCP).

2.Diagnostic Value of Immediate CT after Chemoembolization in Patients with Hepatocellular Carcinoma: Comparison with 2-3 Week Delayed CT.

PURPOSE: Lipiodol CT is a important modality for the diagnosis of hepatocellular carcinoma and compared is usually performed at 2-3 week after Lipiodol injection. Therefore, we assessed and the diagnostic value and merits of immediate CT after chemoembolization from there of 2-3 week delayed Lipiodol CT. MATERIALS AND METHODS: Thirty three cases of chemoembolization which were performed both immediate CT after chemoembolization and 2-3 week delayed Lipiodol CT were reviewed retrospectively. They were divided into four grades according to pattern of lipiodol uptake by three radiologists. The diagnostic value of immediate Lipiiodol CT was compared to delayed Lipiodol CT. RESULTS: Grade 0 was two cases(3.0%) and Grade 1 was seven cases(21.2%). In the cases of Grade 2(23/33, 69.7%) tumor uptake could be dishng wished from parenchymal uptake of Lipiodol by its density and pattern. Tumor uptake showed dense and homogeneous pattern, but parenchymal uptake revealed less dense and hepatoram-like wedge shaped pattern. Lipiodol uptake of tumor in Grade 3(1/24, 3.0%) was equal to that in delayed Lipiodrl CT. Grade 3 and 2(72.7%) of the immediate Lipiodol CT were not inferior to delayed Lipiodol CT in its diagnostic value. CONCLUSION: The immediate Lipiodol CT could make rapid establishment of treatment plan and are expected to be more convenient than delayed CT for the patients.

3.A Case of Mycosis Fungoides.

We describe a case of mycosis fungoides (infiltrative plaque type) in an 86-year-old male. He had experienced dark erythematous round plaques on the face, trunk, buttock, upper and lower extremities. Histopathologic findings revealed band-like superficial dermal infiltration of atypical lymphocytes, epidermotropism, and Pautrier's microabscess in the epidermis. The infiltrative cells were positivly stained with CD3, but not with a CD20. He was treated with a topical application of 0.2% carmustine, but he died due to poor general condition and septicemia.

4.A Case of Cervical Tab.

Cervical tab is one of rare developmental anomalies of the brachial apparatus which appears as firm, protruding papule or nodule on the neck along the line of anterior border of the sternocleidomastoid muscle. A 5 year-old male patient developed a pea sized, movable, semi-solid mass on the lower portion of right sternocleidomastoid muscle area. On microscopic examination, well-marginated oval shaped cartilage was present. Individual cells in the lacunae had ample cytoplasm and eccentric nucleus. Verhoeff-van Gieson stained section of the mass showed cartilage with a rich elastic network in the matrix, most heavily concentrated about the chondrocyte. We report a rare case of cervical tab manifesting as a skin-colored nodule on the sternocleidomastoid muscle area.

5.Natural History of Unilateral Ureteropelvic Junciton Obstructed Kidney : Five Cases of High Grade Hydronephrosis.

To evaluate the sensitivity and specificity of transthoracic fine needle aspiration cytology(TFNAC) in the preoperative diagnosis of pulmonary nodules, a retrospective analysis was carried out on a consecutive series of 200 TFNACs. They included 186 primary malignant tumors, 66 squamous cell carcinomas, 65 adenocarcinomas, 36 small cell carcinomas, 7 large cell carcinomas, 4 carcinoids, 8 others, 9 metastatic tumors, and 5 benign tumors. On cytohistologic correlation of malignant pulmonary tumors, the pro- cedure had a sensitivity of 97.3% and a specificity of 100%. A 86.6% correct correlation between the cytologic and histologic diagnoses was achieved. Five out of the 7 undifferentiated large cell carcinomas, 10 out of the 65 adenocarcinomas, 2 out of the 36 small cell carcinomas, and 2 out of the 66 squamous cell carcinomas were turned out to be mistyped in cytologic diagnosis. We concluded that TFNAC is a highly sensitive and specific preoperative diagnostic procedure in the investigation of patients with discrete pulmonary nodules in whom the specific cell type of the malignant neoplasm has important implications in treatment modality and prognosis.

6.A Case of Nasopharyngeal Teratoma Showing Upper Airway Obstruction.

Neoplasms of the nasophrynx are rare in neonates and infants. The most common nasopharyngeal tumor encountered in this age group is teratoma-neoplasm containing multiple heterotopic tissues. Four histologic types of nasopharyngeal teratoma occur-dermoid, teratoid, true teratoma, epignathiof which dermoid is most frequently seen. We have experienced a case of nasopharyngeal teratoma in a female neonate with signs and symptoms of upper airway obstruction. It was diagnosed by cervical. CT and confirmed as ture teratoma by histological examination. Nesopharyngeal teratoma was removed by contact Nd:YAG laser. In addition, we briefly reviewed the literature of neonatal nasopharyngeal teratoma.

7.A Case of Acute Myeloid Leukemia with an Unusual Phenotype.

We introduce an unusual case of acute myeloid leukemia (AML) showing strong myeloperoxidase (MPO) positivity without any lineage-specific cell surface marker expression, i.e. myelomonocytic antigens CD13, CD14, CD15 and CD33; or B-lymphoid antigens CD10, CD19, CD20, CD22 and surface immunoglobulin; or T-lymphoid antigens CD2, CD3, CD5 and CD7. The case was morphologically AML with maturation (FAB M2) and the cytogenetic study showed t (8; 21) (q22; q22) and missing Y. But Immunophenotypic studies by flow cytometry showed positive reaction ony for CD34 and HLA-DR. We considered this case as a 'presence of t (8; 21) in MPO (+), CD antigen (-) AML'. The myeloid lineage antigens are known to be expressed very early during myeloid differentiation whereas MPO (in its functional form) is viewed as being expressed relatively late in the process. Therefore, this case could be an example of 'asynchronous differentiation' in leukemia.

8.A Case of Linear IgA Bullous Dermatosis.

We describe an 18-year-old male with generalized pruritic, tense annular or polycyalic blistera on normal-appearing skin or erythematous bases. Histopathological fmdings of an iatact early blistering showed subepidermal blistering and papillary neutrophilic gnicroabscesses. A direct immunofluctescent study of a section from the skin adjacent to a lesioti revealed linear deposits of IgA at the dermoepidermal junction. There were no deposits of IgG, lgM, or C3. In 1.0M NaC1 eplit skin, IgA was found to be bound linearly on the epidermal roof. The patient respmded weIl to eteroid and dapsone therapy.

9.Chronic Recurrent Folliculitis with Atypical Hyperpigmented Scarring in an AIDS Patient.

Bacterial, fungal, and viral infections of the skin with extended skin involvement can occur during the early phase of human immunodeficiency virus infection. A significant reduction in circulating CD4+ lymphocytes in the late stage of the disease may cause tumors of the skin such as Kaposi's sarcoma. A 40-year male patient, a former sailor who had multiple sexual contact with native African women, presented with multiple tender follicular pustules and fibrotic brown patches on both his legs. these had been present for 6 months. The skin lesions were healed leaving brown pigmentation. Laboratory examinations revealed the presence of leukopenia, thromocytopenia and a reversed T4/T8 ratio. The ELISA and Western blot analysis to human immunodeficiency virus were positive. A skin biopsy from a brown patch showed early stages of scar tissue and perivascular hemosiderin deposition. We herein report a case of acquired immunodeficiency syndrome patient with atypical dark brown scarring atrophic patches on the lower legs following purulent bacterial folliculitis. This may have been an early manifestation of Kaposi's sarcoma from a preceding skin lesion.
