1.Extraocular Sebaceous Carcinoma.

Sebaceous carcinoma is an uncommon neoplasm usually associated with ocular adnexa. This malignancy may occur at any anatomic site that contains sebaceous glands. Despite the widespread anatomic distribution of sebaceous glands, extraocular sebaceous carcinoma occurs with far less frequency. We report a case of an 87-year-old Korean woman with extraocular sebaceous carcinoma treated with total excision.

2.A Case of Nasopharyngeal Teratoma Showing Upper Airway Obstruction.

Neoplasms of the nasophrynx are rare in neonates and infants. The most common nasopharyngeal tumor encountered in this age group is teratoma-neoplasm containing multiple heterotopic tissues. Four histologic types of nasopharyngeal teratoma occur-dermoid, teratoid, true teratoma, epignathiof which dermoid is most frequently seen. We have experienced a case of nasopharyngeal teratoma in a female neonate with signs and symptoms of upper airway obstruction. It was diagnosed by cervical. CT and confirmed as ture teratoma by histological examination. Nesopharyngeal teratoma was removed by contact Nd:YAG laser. In addition, we briefly reviewed the literature of neonatal nasopharyngeal teratoma.

3.Electronmicroscopic Study of the Effect of Hexamethonium on Serous Choriretinopathy in Rabbits.

Stress has been regrarded as one of the causes of central serous chorioretinopathy. We studied the effect of Hexamethonium (ganglionic blocking agent) on experimentaI serous chorioretinopathy following epinephrine injection in rabbits. In group 1, we injected the 0.1 % epinephrine into the rabbits through IV route for 10 days and in group 2, we addited Hexamethonium subcutaneously prior to injection of the epinephrine same term of gorup 1. We could find slight histological changes in gorup 2 as com paired with group 1, which produced serous chorioretinopathy, under the electronmicroscopic study so we would expect the preventive effect of Hexamethonium on the stress-induced serous chorioretinopathy.

5.Changing pattern of serum leptin concentration in women undergoing clomiphene citrate challenge test or controlled ovarian hyperstimulation.

OBJECTIVE: To evaluate the changing pattern of serum leptin level and the correlation between estradiol level during the clomiphene citrate challenge test(CCCT) or controlled ovarian hyperstimulation(COH) cycle. METHODS: Twenty-seven women who underwent CCCT and fourteen women who underwent controlled ovarian hyperstimulation were recruited to measure the serum leptin level. After correction of serum concentration with body mass index(BMI), changes of leptin level and correlation with serum estradiol level during CCCT and COH cycle were analyzed. RESULTS: Circulating leptin levels were significantly correlated with BMI at each time point in CCCT cycle(P<0.01). In CCCT cycle, leptin/BMI level was significantly increased at midluteal phase compared to that of menstrual cycle day 3 and 10(p<0.05). In women with regular menstruation, leptin/BMI level at midluteal phase was significantly higher than that of menstrual day 3 and 10, but this difference was not seen in women with irregular menstruation. The leptin/BMI level in COH cycle showed increasing tendency throughout ovarian stimulation. But there was no significant correlation between leptin/BMI and estradiol level in CCCT and COH cycle. CONCLUSION: There is a significant correlation between BMI and circulating leptin level. Midluteal leptin level is significantly higher than that of follicular phase in CCCT cycle, and there is an increasing tendency in leptin level after ovarian stimulation in CCCT and COH cycle without statistical significance. These findings suggest that circulating estradiol concentration has no major influence on circulating leptin level.

6.A Case of Nevus Sebaceus Associated with Trichilemmoma.

We reported a case of nevus sebaceus associated with trichilemmoma in a 19-year- old female patient. The lesion was a 2 x 2 cm sized yellowish verrucous plaque with two 0.3 x 0.3 cm sized dome-shaped papules. Histopathological examinat,ion revealed hyperkeratosis, papillomatosis and acanthosis, proliferation of sebaceus glands, presence of apocrine gland and mild inflammatory cellular infiltration around superficial vessels. In the epidermis of the lesion, there were two foci of lobulated mass consisted of clear cells with columnar basal cells and thick basement membrane.

8.Comparison of Laparoscopy-assisted Distal Gastrectomy with Open Distal Gastrectomy for Early Gastric Cancer: The Experience of a Group that Have Overcame a Learning Curve.

PURPOSE: Due to the increasing number of early gastric cancer (EGC) cases, laparoscopic procedures have focused on in the quality of life. Although several studies have compared the surgical results of laparoscopic and open gastric resections, these are only the early initial experiences of the laparoscopic procedure. However, there has been no study following the overcoming of the learning curve. Between October 1998 and April 2003, we performed 83 laparoscopic radical gastrectomies, and compared the laparoscopy-assisted distal gastrectomy (LADG) with the conventional open subtotal gastrectomy after obtaining the learning curve. METHODS: Between May 2003 and February 2004, 35 patients who diagnosed with EGC by gastroscopy underwent LADG with lymph node dissection. Conventional open gastrectomy were performed in 14 patients preoperatively diagnosed advanced gastric cancer, but confirmed to EGC inform the pathology. Various clinicopathological parameters were retrospectively evaluated from the medical records. RESULTS: Statistically significant differences (P<0.05) were present between the laparoscopy-assisted and conventional open gastrectomies; white blood cell count on day 2 (10687 vs. 13053, P=0.033), liquid diet start day (4.0 vs. 6.0, P<0.001) and days of hospital stay (7.6 vs 12.1, P=0.005). For the LADG without complication, the group with more than a 7 day hospital stay had a delayed liquid diet start day (4.19 vs. 3.18, P=0.006). A strong positive correlation was found between the day of hospital stay and the start of a liquid diet for LADG without complication there is strong poitive correlation (correlation coefficient=0.77). CONCLUSION: A LADG with lymph node dissection for EGC has several advantage compared with a conventional open gastrectomy; less inflammatory reactions, a rapid return of gastrointestinal function, and a shorter hospital stay with no decrease in operative curability.

9.The State of Patient Satisfaction after Hernioplasty on an Ambulatory Basis.

PURPOSE: There have been no studies employing a specific questionnaire relating to patient satisfaction following ambulatory hernioplasty. Via the production of a novel specific questionnaire, attempts were made to determine the factors associated with patient satisfaction following hernioplasty on an ambulatory basis. METHODS: Patient satisfaction was evaluated via cross-sectional telephone surveys administered 10.5 (range of 2~23) months after their operations, consisting of six questions, regarding; anesthetic technique, surgical method, necessity for admission, necessity for follow-up, intraoperative pain, and postoperative pain. Each of the questions was then scored using a 4-point scoring system, with global satisfaction determined via the addition of each score. Factors related to global satisfaction were determined among preoperative, intraoperative and postoperative factors. RESULTS: Telephone questionnaire interviews were conducted on all 131 consecutive patients. Four respondents (3.1%) expressed dissatisfaction with the ambulatory surgery. Twelve (9.2%) had been admitted overnight after the operation. Thirteen (9.9%) required analgesics for over 3 days. No patients required a re-operation, although 20 (15.3%) experienced minor postoperative complications. Significant factors for global dissatisfaction were analgesic requirement for over 3 days and the presence of surgical complication (P value <0.05). Time until return to work and required overnight admission were important factors for patient satisfaction, but these were not significant. CONCLUSION: Patient satisfaction was associated with postoperative pain and surgical complications. Therefore, a more appropriate method for pain control and prevention of minor surgical complication are suggested might serve to enhance patient satisfaction after hernioplasty on an ambulatory basis.

Active infective endocarditis, involving native, is often complicated by conduction abnormalities. These conduction disturbances are considered to represent an extension of the infection from the valve to the annulus, and to the surrounding myocardium. We describe a case of a 59-year-old woman who presented with dyspnea due to staphylococcal endocarditis, which was complicated by a complete heart block.
