1.Job Satisfaction, Organizational Commitment and Turnover Intention among Male Nurses.

PURPOSE: This study aimed to investigate job satisfaction, organizational commitment and turnover intention among male nurses in South Korea. METHODS: Data were collected in May, 2013 and a total of 150 male hospital nurses participated in this study. Job satisfaction, organizational commitment and turnover intention were measured using a structured questionnaire. Collected data were analyzed using SPSS/WIN 20.0 for windows. RESULTS: There were statistically significant correlations between job satisfaction and organizational commitment (r=.74, p<.001), job satisfaction and turnover intention (r=-.56, p<.001), and organizational commitment and turnover intention (r=-.69, p< .001). There was a statistically significant difference in job satisfaction of participants according to education, work unit, and salary. Organizational commitment showed significant difference according to age, education, work unit, and salary. Turnover intention was significantly different according to duration of employment in the hospital, and salary. The most influential factor for turnover intention was organizational commitment (beta=-.69, p<.001). This factor accounted for 47.8% of the variance in turnover intention. CONCLUSION: The results of this study indicate that to reduce turnover intention for men in nursing, it is necessary to increase job satisfaction, and organizational commitment.

2.A Case of Aberrant Lacrimal Gland.

Ectopic or Aberrant lacrimal gland, which has been observed in the eye and in various sites on the ocular adnexa is an unusual congenital and developmental abnormality. The authors have recently experienced one case of Aberrant lacrimal gland in the upper and lateral quadrant of the bulbar conjunctiva involving the cornea on right eye in a 10 year old Korean girl. It was reviewed clinically with the literature.

4.Vaginal Birth after Cesarean Section.

Repeat cesarean section is one of the leading causes to make increasing a rate of cesarean section. Trial of labor TOL after cesarean section is attempt to reduce the rate of repeat cesarean section. Trial of labor has been well established as a safe alternative in carefully selected women with transverse scars in the lower uterine segment. This study was based on 104 cases of delivery with prior cesarean birth at the Chonbuk National University Hospital from January, 1991 to June, 1997. The results were as follows: 1) Among 1400 cases with previous cesarean delivery, trial of labor was done in 104 cases (7.4%). Among 104 cases, vaginal birth was successfully completed in 96 cases (92.3%). 2) Among 28 cases with PGE2, vaginal tablet, vaginal delivery was done in 23 cases (82.1%). 3) Among 22 cases with more than 4 of Bishop score, vaginal delivery was done in 21 cases (95.5%), and among 6 cases with less than 3 of Bishop score, vaginal delivery was done only 2 cases (33.3%). 4) There was no matemal death or uterine rupture in the cases of trial of labor, But in 2 cases, uterine dehiscence was observed in 4 cases and postpartal bleeding (more than 400ml) was developed. 5) In the cases of cesarean section, mean of hospital day was 7 days and medical fee was about 400,000 won. In the cases of vaginal birth after cesarean section, mean of hospital day was 2.6 days and medical fee was about 100,000 won. In condusion, vaginal birth after cesarean section is safe and effective alternative to elective repeat cesarean section and also the use of PGE, vaginal tablet is so effective to increase success rate of vaginal delivery. After all, positive these trials might decrease cesarean rate and increase maternal health and quality of medical care.

5.Agreement and Sensitivity of Sacroiliac Joint Pain Provocation Tests.

The purpose of this study is to assess agreement and sensitivity of sacroiliac joint pain provocation tests with fluoroscopically guided, diagnostic sacroiliac joint block. Thirty patients with sacroiliac joint area pain and without other obvious sources of pain were selected. The patients were tested with 11 kinds of sacroiliac pain provocation tests before and after sacroiliac joint block and their pain relief scales were measured before block, after 30 minutes, 1 week, and 2 weeks. According to pain relief scale at 30 minutes after sacroiliac joint block, the patients were divided into 3 groups - response group that was 100-70% pain relief; 9 cases(30%), partial response group that was 69-30% pain relief; 15 cases(50%), and nonresponse group that was 29-0% pain relief; 6 cases(20%). In the response and the partial response group, the sensitivity of pain provocation tests were 88.5% in FABERE, 53.9% in Erichen, 50.5% in Yeoman, 43.3% in Minor, and 38.5% in Gaenslen test. Gamma statistic for agreement in the 5 superior sensitive tests was 0.80 between Minor and Gaenslen test, 0.67 between Yeoman and Minor test, 0.48 between Yeoman and Gaenslen test, and 0.44 between FABERE and Minor test. Pain relief scale 2 weeks later were above 60% in response group and above 50% in partial and nonresponse group. In conclusion, combining the FABERE test and another sensitive test, we enhance diagnostic power for sacroiliac dysfunction.

Cholangiocarcinoma is a form of adenocarcinoma arising from the intrahepatic or extrahepatic biliary epithelium. Apparent predisposing factors include some chronic hepatobiliary parasitic infsttations, congenital anomalies with ectatic ducts, sclerosing cholangitis and chronic ulcerative colitis and occupational exposure to possible biliary tract carcinogens. Patients with cholangiocarcinoma usually present with biliary obstruction, painless jaundice, pruritus, weight loss and acholic stools. Because the obstructing process is gradua1, the cholangiocarcinoma is often far advanced by the time it presents clinically. The diagnosis is most frequently made by cholangiography following ultrasound demonstration of dilated intrahepatic bile ducts. We report a case of common bile duct cancer spreading to gallbladder and common hepatic duct along the biliary mucosal epitherium and submucosal tissue in 68-year-old female patient along with a review of literature.

8.A Case of Acute Suppurative Labyrinthitis Complicated by Chronic Otitis Media.

With the development of antibiotics, tympanogenic labyrinthitis complicated by otitis media rarely occurs nowadays. Only few cases of acute serous or suppurative labyrinthitis in otitis media patients have been reported previously in the literature. Acute suppurative labyrinthitis caused by direct bacterial invasion into the inner ear leads to potentially severe, irreversible hearing loss, vertigo and spontaneous nystagmus toward the healthy side. Magnetic resonance image might be helpful to recognize the inflammatory lesions of the labyrinth. Treatment for tympanogenic labyrinthitis includes an appropriate use of antibiotics and establishment a draining route. Recently, we experienced an unusual case of acute suppurative labyrinthitis complicated by chronic otitis media without cholesteatoma. We also review and discuss the clinical manifestations, radiologic findings and treatment strategy of this disease.

9.An Anesthetic Experience of Intratracheal Double-Stents Insertion in an Infant.

The management of airway during anesthesia is mandatory but it is very difficult in certain diseases, especially when a tracheoplasty and bronchoplasty is done. Tracheal stenosis is caused by congenital factors, trauma, physical compression, infection, intrabronchIal granuloma, tumor etc. Among those causes, congenital tracheobronchial stenosis is very rare and it accompanies pulmonary arterial sling and cardiovascular anomaly. Our anesthetic experience of succesful tracheal management during tracheobronchoplasty (3 times), bronchoscopic examination and bougienation (7 times), insertion of a single stent (1 time) and double stents into the trachea and the main bronchus (2 times) on a child who has a congenital complete circular tracheal cartilage and pulmonary arterial sling which caused tracheal stenosis will be reported, along with a literature review.

10.Relationship among the Expression of Cyclin D1, p21, and p53 Protein, and Prognosis in Non-Small Cell Lung Carcinomas.

Recently, cell cycle regulators have been suggested as new prognostic factors of the lung cancer. In this study, we evaluated the expression of cyclin D1, p21, and p53 using the X2-test, with regard to the stage of the patients, histologic type, and histologic differentiation in the 135 cases of non-small cell lung carcinomas (NSCLC). To evaluate the confounding effects among cyclin D1, p21, and p53 on X2-test analysis, we used the Mantel-Haenzel test. The NSCLC in this study included 82 cases of squamous cell carcinoma and 53 cases of adenocarcinoma. Each nuclear staining of cyclin D1, p21, and p53 was observed in 65 cases (48.1%), in 54 cases (40.0%), and in 81 cases (60.0%) of NSCLCs, respectively. Only p53 expression was significantly associated with the stage (stage I, II, IIIa) (p<0.05) and squamous cell carcinoma (p<0.05). On the other hand, cyclin D1 expression was significantly associated with the histologic differentiation. The confounding effects among cyclin D1, p21, and p53 revealed that only p21 expression changed the relationship between p53 and stage. In this regard, further study is needed.
