1.Lymphomatoid Papulosis in a 10-year-old Boy.

Lymphomatoid papulosis (LyP), first described by Macaulay in 1968, is a paradoxical skin disease characterized by a generally clinical benign course, in contrast to malignant histological features. LyP is rarely seen in children. We herein report a case of LyP in a 10-year-old boy who had a 4-week history of multiple, scattered erythematous papules, crusted papules or ulcerated papules on the trunk, arms and thighs. Histopathologically, there was a superficial and midderma1 perivascular and interstitia1 infiltrate composed primarily of lymphocytes, which were admixed with large, atypical, mononuclear cells with pleomorphic nucleus. These atypical cells expressed CD30 (Ki-1) antigen. This is the first reported case in Korea of LyP present in a child.

Kaposis sarcoma is well recognized in patient,s with imrnunodef cienicy states such as organ trans plantation, malignancy, and acquired irnmunodeficiency syndrome(tILS). Kaposis sarcorna associated with organ transplantation is, however relatively uncomrnon in Konea. We report a case of Kaposi's sarcorna in a 23 year old male kidney recipient., who was on cyclosporine and prednisolone. The tumor involved the larynx and long as well as the skin. He was treated with radiation therapy and chemotherapy, and the dosage of immunosupressants were reduced.

5.Transurethral Prostatectomy with Advanced Technique and Instruments.

PURPOSE: We studied the therapeutic effect and safety of transurethral prostatectomy (TURP) performed with improved instruments by a moderately experienced resectionist. PATIENTS AND METHODS: Clinical data of the 95 consecutive patients who underwent TURP by one surgeon from March 1995 to June 1997 were analyzed. During this period, a continuous-irrigation 26F TUR sheath with a 30-degree lens attached to a monitor screen was used in most patients. RESULTS: The average weight of the resected adenoma was 10.8 7.4 gm, and the resection time was 47.2 21.2 minutes. The average weight of tissue resected per minute was 0.23 0.13 gm. Six patients required transfusion, and four suffered operative complications (one bladder perforation, one bladder neck injury, and two urethral injuries that enforced prolonged catheterization). Postoperative complications developed in seven cases. The duration of postoperative urethral catheterization averaged 2.8 days. The maximal flow rate was improved from 11.3 mL/sec to 20.6 mL/sec. Subjective satisfaction was mentioned by 95.6% of patients (91/95). In comparison with data from the first authors former report, all differences in terms of resection speed, safety, and morbidity were statistically significant. The therapeutic efficacy of the operation with the new equipment seems to be superior to that of other less invasive procedures, and even in terms of complications and duration of catheterization, these results are comparable to those of other procedures. CONCLUSIONS: We think that TURP deserves to be the primary treatment modality for the patients with benign prostatic hyperplasia who need surgical therapy if the procedure is performed by a properly trained surgeon equipped with modern TUR instruments.

6.The Clinical analysis of Tibial Shaft Fracture at the Primary Trauma Hospital

Tibia fractures are frequently occurred with increased traffic accident and industrial accident recently. And there are controversies in treatment of tibia fractures. From Jan. 1986 to Oct. 1988, tibial fractures of 107 cases in 105 patient, treated at the Department of Orthopedic Surgery of Dae Han Hospital, were analyzed. Results were obtained as follows. l. Of 105 patients, 85 patients were male & 20 patients were female. 2. The most common causes of fracture was traffic accident. 3. The most common level of fracture was middle one-third of tibial shaft. 4. Of 107 cases, conservative treatment was 35 cases and operative treatment in 72 cases. 5. Average union time was 16 weeks in conservative treatment & 18.5 weeks in operative treatment.

8.Cutaneous Immune Reactions to Trichophytin in Dermatophytosis.

BACKGROUND: The cell mediate immunity is of importance for the development of host resistance to dermatophytic infection. OBJECTIVE: The aim of this study was to estimate the clinical usefulness of purified trichophytin and to correlate cell mediate immunity to the clinical parameters of dermatophytosis, i. e. duration of infection, localization of infection and the type of dermatophyte involved. METHODS: For evaluation of cell mediate immunity in dermatophytosis, cutaneous immune reaction was measured in 102 patients with dermatophytosis (75 patients with chronic dermatophytosis and 27 patients with non-chronic dermatophytosis) by means of intradermal injection of purified trichophytin extracted from Trichophyton(T.) mentagrophytes. RESULTS: The results are summarized as follows: Patients with chronic dermatophytosis were positive in 10.7% of cases, while 51.9% of the patients with non-chronic dermatophytosis showed positive delayed cutaneous reactions (p<0.05). Of the dermatophytes isolated, 81.4% of the patients were chronically infected by T. rubrum. Delayed cutaneous reactions occured in infections with T. rubrum in only 17.1% of cases, but this difference was not statistically significant (p=0.06). Of the patients with tinea cruris, delayed cutaneous reactions were registered in 50%, but only in 15.1% those with tinea unguium (p<0.05). The significant difference in the rate of positivity in delayed reactions was not shown between the patients group with nail infection (15.1%) and the patients group without nail infection (28.6%). The rate of positivity in immediate and delayed reactions of the patients group in the multiple lesions (50%, 13.2%, respectively) was higher than that of the patients group in the single lesion (46.9%, 26.6%, respectively). CONCLUSION: The present study reinforced the hypothesis that susceptibility to chronic dermatophytosis is related to a lack of cell mediate immunity to the infectious agents and clinically, purified trichophytin is good for the evaluation of host sensitization to dermatophyte antigens as well as cell mediate immunity in dermatophytosis.

10.A Case of Nevus Sebaceus Associated with Trichilemmoma.

We reported a case of nevus sebaceus associated with trichilemmoma in a 19-year- old female patient. The lesion was a 2 x 2 cm sized yellowish verrucous plaque with two 0.3 x 0.3 cm sized dome-shaped papules. Histopathological examinat,ion revealed hyperkeratosis, papillomatosis and acanthosis, proliferation of sebaceus glands, presence of apocrine gland and mild inflammatory cellular infiltration around superficial vessels. In the epidermis of the lesion, there were two foci of lobulated mass consisted of clear cells with columnar basal cells and thick basement membrane.
